[Cerebellopontine angle surgery. Part 1: General remarks].

OBJECTIVES: Cerebellopontine angle tumors, although uniform in location, are diverse with regard to the site of tumor origin and displacement of the neurovascular structures. With the increasing use of magnetic resonance imaging in the diagnosis of brain lesions, the treatment of tumors of the cerebellopontine angle has changed extraordinarily. An appreciation of the vascular and cranial nerve microanatomy and the intimate relationship between neurovascular structures and the tumor is essential for achieving optimal surgical results METHODS: A review was made of the current literature. RESUL

[Phanerogenetic retrocochlear low frequency hearing loss].

OBJECTIVE: To study the pathogenisis of retrocochlear low frequency hearing loss. METHODS: Clinical and audiologic findings [auditory brainstem response (ABR), evoked otoacoustic emission (EOAE), et al] of 29 cases with retrocochlear low frequency hearing loss were studied. RESULTS: The head injury, acoustic neuroma, peripheral neurophathy, hereditary hear loss, multiple sclerosis and brainstem disease can cause retrocochlear low frequency hearing loss. The typical clinical manifestations of retrocochlear low frequency hearing loss were normal EOAE which cannot be suppressed by contralateral

Neural-dural transition at the medial anterior cranial base: an anatomical and histological study with clinical applications.

OBJECT: Few anatomical studies have been focused on the morphological features and microscopic anatomy of the transition from the intracranial space to the medial anterior cranial base. The authors of the current study performed histological analyses to define the structure of the transition from neural foramina to the cranial base (neural-dural transition) at the cribriform plate, particularly as related to cerebrospinal fluid (CSF) fistula formation and surgical intervention in the region. METHODS: The medial anterior cranial base was resected in six cadaveric specimens. Histological method

Radiation exposure of normal temporal bone structures during stereotactically guided gamma knife surgery for vestibular schwannomas.

OBJECT: The dosimetry of radiation exposure of healthy inner, middle, and external ear structures that leads to hearing loss, tinnitus, facial weakness, dizziness, vertigo, and imbalance after gamma knife surgery (GKS) for vestibular schwannomas (VSs) is unknown. The authors quantified the dose of radiation received by these structures after GKS for VS to assess the likelihood that these doses contributed to postradiosurgery complications. METHODS: A retrospective study was performed using a prospectively acquired database of a consecutive series of 54 patients with VS who were treated with G

Unusual complication of tonsillectomy: taste disturbance and the lingual branch of the glossopharyngeal nerve.

Taste disturbance is an unusual complication of tonsillectomy of which there are very few reports in the literature. The possible causes of this rare complication are: (1) direct or indirect damage to the glossopharyngeal nerve or its lingual branch (LBGN), (2) lack of dietary zinc, and (3) habitual drug intake. We report a 41-year-old man, who complained of taste disturbance following tonsillectomy that was performed for chronic tonsillitis and unilateral (left) tonsillar hypertrophy. During surgery, hypertrophic tonsils were found to be sited deeply into the tonsillar bed, especially at the

Pathophysiology of trigeminal neuralgia: the ignition hypothesis.

There are no satisfactory animal models of trigeminal neuralgia, and it is difficult to obtain essential data from patients. However, trigeminal neuralgia presents with such idiosyncratic signs and symptoms, and responds to so distinctive a set of therapeutic modalities, that scientific deduction can be used to generate likely hypotheses. The ignition hypothesis of trigeminal neuralgia is based on recent advances in the understanding of abnormal electrical behavior in injured sensory neurons, and new histopathologic observations of biopsy specimens from patients with trigeminal neuralgia who a

Hyperglycemia impairs antro-pyloric coordination and delays gastric emptying in conscious rats.

Delayed gastric emptying has been shown in diabetes. Although it has been proposed that hyperglycemia, and not only autonomic neuropathy, contributes to the pathogenesis of delayed gastric emptying, the inhibitory mechanism of hyperglycemia on gastric emptying remains unclear. We studied the effects of hyperglycemia per se on gastric emptying and postprandial gastric motility in conscious rats. Liquid and solid gastric emptying were compared between saline-infused rats and D-glucose-infused rats. Two strain gauge transducers were implanted on the antrum and pylorus and the postprandial antro-p

Translabyrinthine approach for the management of large and giant vestibular schwannomas.

BACKGROUND: The removal of large vestibular schwannomas through the translabyrinthine approach is still controversial. MATERIALS AND METHODS: A retrospective review was performed of 81 patients (58 men and 23 women) with vestibular schwannomas 3 cm or greater, who underwent tumor removal via the translabyrinthine approach between 1985 and 2000. The mean tumor size was 3.7 +/- 0.81 cm, and the mean age of the patients was 47 +/- 16.1 years. The largest tumor was 6 cm. All surgical procedures were performed in collaboration with a neurosurgery team. RESULTS: Total tumor removal was accomplishe

Dissection of the intracranial internal carotid artery producing isolated oculomotor nerve palsy with sparing of pupil.

Dissection of the internal carotid artery usually occurs in the cervical segment, but rarely may involve the artery in the intracranial course (1). The clinical course of intracranial dissection is often catastrophic, with rapid onset of profound neurological deficit, as a result of middle and/or anterior cerebral artery involvement. When this occurs the mortality rate is generally considered high. We describe a case of intracranial internal carotid artery dissection following trivial trauma presented with an isolated painful pupillary sparing oculomotor nerve palsy.

Non-recurrent inferior laryngeal nerve.

Surgical damage to the inferior laryngeal nerve is one of the most feared complications of thyroid surgery. Prevention of surgical injuries requires systematic, early exposure of the inferior laryngeal nerve. A detailed knowledge of anatomical variations is necessary. Non-recurrent inferior laryngeal nerve is a rare anomaly on the right side and is exceptional on the left. Whereas the typical course of the inferior laryngeal nerve is due to the embryological development of the aortic arch and supra-aortic vessels, non-recurrence is associated with a vascular anomaly such as a right retro-oesop

Self-inflicted vocal cord paralysis in patients with vagus nerve stimulators. Report of two cases.

Vagus nerve stimulation for treatment of epilepsy is considered safe; reports of severe complications are rare. The authors report on two developmentally disabled patients who experienced vocal cord paralysis weeks after placement of a vagus nerve stimulator. In both cases, traction injury to the vagus nerve resulting in vocal cord paralysis was caused by rotation of the pulse generator at the subclavicular pocket by the patient. Traumatic vagus nerve injury caused by patients tampering with their device has never been reported and may be analogous to a similar phenomenon reported for cardiac

Vitamin B6 protects primate retinal neurons from ischemic injury.

Vitamin B6 derivatives protect the retinal neurons from excitotoxic injury in vitro. However, their in vivo role in a process involving excitotoxicity, such as ischemia, remains unknown. We studied potential protective effects of pyridoxal 5'-phosphate (PLP) and pyridoxal hydrochloride (pyridoxal) on the retinal neurons in a monkey model of transient global ischemia. Daily intravenous injections (15 mg/kg) of pyridoxal and PLP were performed for consecutive 10 days. On the sixth day, whole brain complete ischemia was produced by clipping the innominate and the left subclavian arteries for 20 m

Morphological studies of the optic canal.

BACKGROUND: In some cases of closed head injuries after minor blunt trauma, an amaurosis can occur. Our knowledge concerning the pathophysiological mechanisms of traumatic optic neuropathy is limited. The aim of this study is to analyze the morphology of the optic canal in order to understand the effect of mechanical forces in the optic canal and the consecutive injuries. METHODS: Forty-one optic canals from body donors were analyzed by light microscopic, polarization microscopic, immunohistochemical and scanning electron microscopic techniques. RESULTS: In the optic canal, collagen fibrils
