Endogenous GABA attenuates CNS white matter dysfunction following anoxia.

We studied the effects of GABA on anoxia-induced injury in CNS white matter using optic nerves exposed to 60 min of anoxia. Injury was assessed by recording pre- and postanoxic compound action potentials (CAPs). GABA (1 microM) significantly increased postanoxic CAP recovery when applied 60 min prior to anoxia. This effect was bicuculline (100 microM) insensitive, mimicked by baclofen (1 microM), blocked by GABA-B antagonists, and not mimicked by selective GABA-A agonists. GABA therefore acted at GABA-B receptors. High concentrations of GABA and baclofen did not influence recovery, possibly in

Neuromuscular dysfunction after surgical treatment of oral cancer.

Surgical treatment of oral cancer usually includes radical dissection of the neck, in which the sternocleidomastoid muscle is sacrificed. In modified neck dissection the accessory nerve is spared, but may be severed by e.g. manipulation. The facial nerve may also be severed by traction or distension. We have done an electromyographic study of 35 patients, 20 men and 15 women, aged from 21 to 92 yrs, 6 to 176 months after oral cancer operation. EMG findings of the upper trapezius muscle were normal in only six of 32 patients who had had a radical or upper neck dissection, although the accessory

Staged removal of acoustic tumors: techniques and lessons learned from a series of 83 patients.

The removal of large acoustic tumors is associated with increased mortality and cranial nerve injury. One method for treating these difficult lesions is staged resection. Between 1972 and 1992, more than 600 acoustic tumors were resected at our institution. Of these, 83 were removed in stages. This represents the largest series of staged acoustic tumor resections reported to date. A review of available films and patient records was performed for all acoustic tumors resected in stages between 1972 and early 1993 to analyze demographic information, tumor size, operative technique, outcome, and c

Oculocephalic sympathetic dysfunction in posttraumatic headaches.

Oculocephalic sympathetic functions were assessed in five patients with posttraumatic headaches using the thermoregulatory sweat test and biochemical pupillary responses. Four patients demonstrated bilateral sympathetic dysfunction following whiplash injury, and one patient demonstrated unilateral sympathetic dysfunction following forehead injury. Biochemical pupillary responses were diagnostic in the early posttraumatic period, while the thermoregulatory sweat test was abnormal up to 56 months following the injury. This study documents serious injury to the cervical sympathetic nerves in pati

[A case of abducens nerve palsy after whiplash injury].

We report a patient who developed abducens nerve palsy after whiplash injury. The patient was a 61-year-old man who was involved in a traffic accident on May 15, 1993. After the accident, he developed severe neck pain. Four days later, he still had neck pain and vertigo. On the fifth day after the accident, he noted diplopia on left gaze and visited our hospital. Neurologic examination revealed an isolated abduction deficit of the left eye. Head magnetic resonance imaging and angiography showed no abnormal findings. Ten days later, the ocular movements of the patient were full and the diplopia

Anterior segment ischemia after the Jensen procedure in a 10-year-old patient.

PURPOSE/METHODS: We recently encountered anterior segment ischemia in a young healthy girl with traumatic abducens nerve palsy after the Jensen procedure and ipsilateral recession of the medial rectus muscle. The patient was treated with topical dexamethasone and homatropine 2% and oral prednisone. RESULTS/CONCLUSION: The patient's condition improved, and six weeks postoperatively, visual acuity was 20/20. As this complication of anterior segment ischemia is potentially serious and unpredictable, we think that an alternative surgical approach should be used when treating patients with muscle

[Principles and results of treatment in acquired paralysis of III, IV and VI nerves].

Between 1979-1994 120 patients with acquired paralysis of cranial nerves of ocular muscles were treated: 33 cases with paralysis of oculomotor nerve, 43 cases with paralysis of trochlear nerve, 44 cases with paralysis of abducens nerve. The majority of our patients were males (84-70.0%) aged 21-40 years (65-54.2%). The paralysis was most frequently caused by traffic accidents (45 cases--37.5%) and assaults (26 cases--21.7%). The most common symptom of paralysis was diplopia (109 patients--90.8%). Only 12 persons (10.0%) were admitted during the first month of paralysis and visual disorders cau

Transient trochlear nerve palsy following anterior temporal lobectomy for epilepsy.

Three of 22 patients (14%) who underwent anterior temporal lobectomy for treatment of medically intractable epilepsy at our institution from July 1987 through July 1993 experienced diplopia immediately after surgery. We found ipsilateral paresis of the superior oblique muscle in all three patients. Their ophthalmoplegia resolved completely within 14 weeks. We did not observe any new structural or ischemic changes on postoperative MRIs to account for their deficits. Trochlear nerve palsy--not oculomotor nerve palsy, as is reported in most reference texts--is a relatively common cause of transie

Mental nerve dysfunction: a symptom of diverse mandibular disease.

Paraesthesia and anaesthesia of the mental nerve may result from a variety of pathological conditions, and in persistent cases of orofacial sensory disturbance thorough clinical assessment, including CT scanning, is vital to exclude underlying systemic or neoplastic disease. This paper presents three patients with right mental nerve dysfunction, and reviews the aetiology of mental nerve paraesthesia and anaesthesia.

[Intraoperative facial nerve monitoring in parotid surgery].

Neurophysiological monitoring of cranial motor nerves has proved to be of value in cerebellopontine and skull base surgery. Unfortunately, facial nerve monitoring has been used infrequently for routine parotid gland surgery because suspicion of expense, possible unreliability and the requirement for extra personnel. This study presents clinical experience at the University of Erlangen with facial nerve monitoring during parotid gland surgery done by residents. Advantages are also emphasized for the experienced ENT-surgeon for use during revision parotidectomy. In 35 consecutive patients with b

Cardiac arrest after traffic accident induced through vagal reflex in a case with bilateral stenosis of vertebral arteries.

An 84-year-old driver suffered cardiac arrest after a traffic accident. He was quickly resuscitated and transferred to a hospital where he was treated in a state of unconsciousness and respiratory failure for 20 days until his death. The brain stem was rendered anoxic during cardiac arrest, which caused the respiratory failure. Artificial ventilation and catecholamine infusion were carried out, resulting in myocardial degeneration. Bilateral stenosis of the vertebral arteries was disclosed, but no injuries or hemorrhage of the brain and spinal cord were detected. On days 3 and 4 after admissio

[Section of the facial nerve. Apropos of 40 cases].

From January, 1985 through December, 1992, 40 patients were referred to the Department of Otoneurology of the Lille University Hospital for traumatic facial nerve injury. The injury was located in the intracranial, intratemporal, or extracranial segments of the nerve in 19, 15 and 6 patients, respectively. The trauma was iatrogenic in 30 patients. It was the result of surgery for a tumor of the posterior fossa, of the temporal bone or of the parotid in 19, 2 and 3 patients, respectively. Nerve injury had occurred during surgery for chronic otitis or a parotid fistula in 5 and 1 patients, respe

New kinds of microneuroprotectors for microsurgery and endoscopy of cerebellopontine angle neurovascular decompression.

New microsurgical devices for neurovascular decompression--microneuroprotectors (MNP)--are described. Four constructive kinds of MNP have been developed according to topographic peculiarities of pathological neurovascular contacts. The hydrodynamic and biological testing of MNP has been concluded. The methods of microsurgical and endoscopic techniques of MNP insertion on the cranial nerves or posterior fossa vessels are reported.

Velopharyngeal insufficiency following palatine tonsillectomy.

Although adenotonsillectomy is usually considered a minor operation, numerous uncommon but severe complications have been described. Even tonsillectomy alone can cause velopharyngeal insufficiency (VPI). We describe two cases in which severe VPI was noted after palatine tonsillectomy was performed because of recurrent peritonsillar abscesses. The patients underwent clinical examination, nasalance measurements, videonasopharyngoscopy and videofluoroscopy. Findings in both patients were consistent with lesions of branches of the vagus and glossopharyngeal nerves through lingual rami, while one o

Chiasmal trauma: clinical and imaging considerations.

This report presents a patient who sustained closed head injury with chiasmal trauma. This uncommon injury may not be apparent on routine imaging studies. It is significant, not only from the visual standpoint, but also because of the association with serious conditions, such as panhypopituitarism, traumatic carotid aneurysm, carotid cavernous fistulae, and meningitis associated with leakage of cerebrospinal fluid. This report demonstrates that magnetic resonance imaging (MRI) is the best method for identifying chiasmal abnormalities.

Management of traumatic facial nerve paralysis with carotid artery cavernous sinus fistula.

Massive skull base injuries require detailed preoperative neurological and neurovascular assessment prior to undertaking surgical repair of isolated cranial nerve deficits. We present the management of a patient with traumatic facial paralysis, cerebrospinal fluid leak, and carotid artery cavernous sinus fistula as the result of a gunshot wound to the skull base. The carotid artery cavernous sinus fistula was ultimately controlled with super-selective embolization via the vertebral artery. The facial nerve injury was then safely treated with mobilization of the labyrinthine and vertical segmen
