The initial results of trigeminal somatosensory evoked potentials (TSEPs) which were recorded in 17 patients, age range 16-64 years, presenting with established unilateral sensory impairment to the face or oral cavity, are described. Nerve potentials were evoked by stimulation of the normal healthy side and compared with those recorded from the side with disturbed sensation. The recorded waveforms allowed identification of a prominent sequence of events. Significant increases in the latencies compared to the events previously determined in normal subjects at 20, 30 and 39 msec. were found, and

Injury to the optic nerve may occur after seemingly minor trauma. Evulsion of the nerve from the globe may be partial or complete as a result of several concussive or rotational forces. A depressed or visibly absent lamina cribrosa indicates a disinsertion of the nerve fibers from the globe. Visual loss ranges from severe to complete. Transections of the optic nerve within the orbit generally occur after penetrating orbital injuries or surgical resection, resulting in complete blindness. Medical or surgical intervention has not been shown to improve the visual prognosis once such injuries occu

A six-year-old boy who developed optic atrophy after an injury with the inner tube stem of a truck tire is described. The air stem penetrated the inferonasal portion of the left orbit and discharged air. The patient presented for evaluation within one hour and had no light perception in the involved eye. Computed tomography of the orbit revealed multiple small air bubbles surrounding the optic nerve sheath without direct optic nerve edema. It is postulated that the etiology of optic atrophy is related to the air surrounding the optic nerve, which interrupted its vascular supply. Proper coverin

One hundred and thirty-eight patients with facial nerve paralyses were treated by various surgical methods. One hundred and six patients had total facial nerve paralyses and thirty-two had segmental paralyses. In the total paralysis group, 80 patients had oncologic procedures or tumor induced paralysis. In the segmental paralysis group, 21 patients had malignancies. Two patients had idiopathic facial nerve paralysis. In the total paralysis group, 75 had neural reconstructions, 7 neuromuscular pedicles and 26 muscle-fascia transpositions. The segmental paralysis group was divided into an upper

A series of 183 patients who underwent total thyroidectomy is presented. The operative method, which emphasizes visualization and preservation of the parathyroid glands and their blood supply, as well as exposure of the recurrent laryngeal nerves, is discussed in detail. Permanent hypoparathyroidism was a complication in six patients (3.3 per cent). Permanent nerve injury occurred in one patient (0.55 per cent). Postoperatively, 78 patients had radioactive iodine uptake studies to evaluate the amount of residual thyroid tissue in the neck. Sixty-four (82 per cent) had 24 hour uptake studies of

Intra-operative electrocochleography (ECochG) was used in an attempt to monitor the action potential of the cochlear nerve during acoustic neuroma surgery in 14 patients with useful pre-operative hearing. Five patients had ECochG potentials preserved and yet only three could hear when tested audiometrically later. Of those losing the potentials intra-operatively all were subsequently deaf and the pattern of waveform loss allowed determination of the probable cause of hearing loss. Complete excision of the tumour was achieved in each case regardless of the ultimate effect on the AP as it was no

This study analyzes the complications encountered in the surgical treatment of 17 patients with large glomus jugulare tumors. All 17 patients sustained either new cranial nerve palsies or exacerbation of preexisting palsies. These involved, in descending order of frequency, the facial nerve, the vagus and glossopharyngeal nerves, and the hypoglossal nerve. Postoperative palsies of the facial nerve involved 17 patients, as compared to 5 preoperatively. Fifteen patients had postoperative partial or complete paralysis of the vagus nerve as compared to eight preoperatively. Ten patients had postop

A retrospective review of 29 cases of intratemporal facial nerve injuries included 18 temporal bone fractures, 7 gunshot wounds, and 4 iatrogenic complications. Surgical exploration confirmed involvement of the fallopian canal in the perigeniculate region in 14 longitudinal and 3 transverse or mixed fractures of the petrous pyramid. Gunshot and iatrogenic injuries usually occurred within the tympanic and vertical segments of the facial canal and at the stylomastoid foramen. When hearing is salvageable, the middle fossa approach provides the best access to the perigeniculate region of the facia

Injury to the facial nerve is of concern in surgery of cerebellopontine angle tumors. The crossed acoustic reflex provides a way to monitor the ipsilateral facial nerve with the auditory stimuli delivered to the contralateral side. Using the method of optimum digital filtering, it is possible to monitor the resulting brain stem facial evoked response (BFER) in real time. This paper presents preliminary experiences in more than 18 such operations monitored using this method. This preliminary study demonstrates a trend for a high (88.8%) correlation between BFER and postoperative facial nerve fu

Peripheral nerve injuries associated with carotid endarterectomy are fairly common but not emphasized in reported results of carotid endarterectomy. The sensory nerves to the submandibular skin and ear lobe are often damaged. Motor nerves VII, IX, X, XI and XII may be injured at surgery. The commonest motor injuries involve the facial, vagus and hypoglossal nerves. Carotid endarterectomy was studied prospectively over 1 year to document the incidence of nerve injury. Nerve injury occurred in 12% of patients, with facial and vagus nerves being involved in 4% each. Careful surgical technique bas

Massive bleeding into an intracranial neurinoma is a rare event. The 12th case of this particular occurrence, which was precipitated by weight lifting, is described and a review of the literature is presented. Risk factors for bleeding appear to be tumor size and vascularity. Presenting symptomatology is abrupt and includes headache, nausea, vomiting, and depressed consciousness. Preexisting symptoms referrable to and marked dysfunction of the cranial nerve of origin are present. Deficits of neighboring cranial nerves are frequent. Computed tomography demonstrates the hemorrhages and the tumor

Changes in the response from the cochlear nerve in dogs resulting from cerebellopontine angle (CPA) manipulations were correlated with histological changes in the nerve. The aim of this study was to determine the mechanisms underlying hearing deficits incurred as a result of manipulations in the CPA. Compound action potentials (CAP) were recorded from the cochlear nerve in response to click stimulation before, during, and after cerebellar and eighth nerve retractions were performed under anesthesia. The retractions were carried out to elicit different degrees of change in the latency and wavef

The effect of manipulations in the cerebellopontine (CP) angle on the cochlear nerve was studied in 12 rhesus monkeys to elucidate how surgical procedures in the CP angle may affect the peripheral auditory system. Brain stem auditory evoked potentials and compound action potentials from the cochlear nerve were recorded throughout the experiments and were studied to determine the level of injury to the cochlear nerve. After the manipulations, the animals were perfused with fixatives and their temporal bones were removed and examined histologically. The monkeys showed electrophysiological change

A prospective investigation of 208 patients with painful, arthrographically verified temporomandibular joint disk displacement revealed that seven patients (3%) demonstrated lingual numbness associated with speech articulation impairment. The speech disorder was characterized primarily by a distortion in the production of /s,r,d, and t/. The lingual numbness and the speech disorder had in all cases started in association with a painful onset of permanent displacement of the temporomandibular joint disk. Local anesthetic blocking of the auriculotemporal nerve eliminated both joint pain and musc

The Cavitron ultrasonic surgical aspirator (CUSA) was used in a patient with an acoustic nerve tumour. After surgery she presented unilateral palsies of several cranial nerves. The nerve injuries were most likely induced by ultrasonic energy transmitted from the CUSA via the petrous bone. We suggest that when the CUSA is used in such operations care should be taken to avoid contact between the vibrating probe of the CUSA and the petrous bone.

Brainstem auditory evoked potentials (BAEPs) were used to monitor eight neurosurgical procedures involving posterior fossa tumors to assist the neurosurgeon in preservation of hearing postoperatively. The technique included placement of recording electrodes over the Cz (vertex) and both earlobes. Stimulation was accomplished intraoperatively with a specifically designed intraauricular click stimulator that did not interfere with surgical access to the suboccipital region. Continuous BAEP recording was performed with particular attention to the sequence of preincision, opening of the dura, tumo
