A 60-year-old man fell; a subdural hematoma developed and he lost consciousness. Visual evoked potentials demonstrated a total conduction block in the left optic nerve and slowed conduction on stimulation of the right eye. He died four weeks later, and necropsy showed focal infarcts in the intracranial portion of both optic nerves.

During the past seven years 347 patients have been entered into a data bank at the Duke University Medical Center for evaluation of transient neurologic ischemia. One hundred fifty eight of these patients had carotid endarterectomies of whom 24 (15.1%) developed 26 (16.4%) peripheral cranial nerve palsies. Injury to the peripheral portion of the hypoglossal nerve was noted in 13 patients, to the cervical branch of the facial nerve in five and to the recurrent laryngeal nerve branch of the vagus in eight. Complete recovery of nerve function usually occurred within four months but residual defic

A patient who was admitted with visual loss and, third and fourth cranial nerve paresis as only evidence of a recurrent epidural hematoma is presented. A review of some of the mechanisms suggested in the literature to explain the etiology of ocular signs in the presence of extracerebral collections is offered. We propose an additional possible mechanism and review the anatomical explanation for the development of these symptoms.

A case of posttraumatic unilateral enlargement of the optic nerve sheath associated with a venous obstructive retinopathy and progressive visual loss responded with marked improvement in visual function after nerve sheath decompression. Indirect (closed-head) trauma may be related to the progressive nerve sheath enlargement, venous obstructive retinopathy, and progressive visual loss. It is apparent that a venous stasis retinopathy can result from a posttraumatic optic nerve sheath lesion. Prompt recognition by echographic and/or roentgenographic investigation and subsequent decompression may

The authors approbated in the clinic a new method for restoring vision in 22 patients in damage of the optic nerves caused by pathological processes in the opticochiasmic area of the brain (tumors of the hypophysis, arachnoiditis, the sequelae of trauma). The novelty of the method lies in introducing electrodes under the nerve sheath during operation performed for the main pathological process and producing direct electrostimulation (ES) of the nerves for 3-4 weeks after the operation. The authors chose the optimum parameters of therapeutic ES under control of a direct record of the bioelectri

The occurrence of cranial nerve palsies in cervical injuries is described in eight patients. The clinical features were dramatic and usually presented soon after injury, the most common picture being that of a bulbar palsy, with acute respiratory distress and dysphagia. Neurologic recovery, both from cranial nerve palsies and motor and sensory deficits, was substantial in all instances, suggesting that brain-stem ischaemia due to vertebral artery spasm or compression had been a factor in their causation. Alternatively, differential movement between the base of the skull and the upper part of t

The management of facial nerve injury resulting from temporal bone fracture continues to be a topic of discussion. The indications for surgery as well as the proper timing and extent of surgical exploration are the principal questions debated. This study reviews a large series of patients (26), treated for facial paralysis following a longitudinal temporal bone fracture. The most frequent site of injury was the area of the geniculate ganglion. The injury to the facial nerve was often severe. We recommend early exploration in these cases when electrical studies indicate a severe injury. In most

Recordings were made from facial muscles and the facial nerve near its entrance into the brain stem in patients with hemifacial spasm (HFS). The purpose of this study was to determine if the synkinesis commonly seen in patients with HFS could be linked to ephaptic transmission at the presumed site of the lesion (at the root entry zone (REZ) of the facial nerve). When the mandibular branch of the facial nerve was electrically stimulated, a response could be recorded from the orbicularis oculi muscles during the operation. The latency of the earliest response was 11.03 +/- 0.66 msec (mean respon

Hypoglossal-facial anastomosis has been our procedure of choice in the repair of the permanently injured facial nerve in the cerebellopontine cistern, when the nerve cannot be primarily repaired. Total failures are few and complications are rare. Most results are good to excellent, if assessment is based upon realistic expectations. These include: 1. normal facial symmetry in repose, 2. good midface voluntary motion, 3. no reflex or emotional facial movement, 4. some synkinesis and donor-injected mass facial movement, and 5. surprisingly little functional loss from hypoglossal paralysis. Our e

Using computed tomographic scans and reformatted images of the optic canal region, we were able to define osseous abnormalities in nine patients with sudden traumatic blindness resulting from indirect trauma to the optic nerve. Six patients had optic canal fractures, two had fractures of adjacent structures, and one patient did not have a fracture. Patients received corticosteroid therapy, surgical decompression of the optic canal, both types of treatment, or no specific treatment for the nerve injury. Although there was no correlation between the abnormality identified by computed tomography

Two years after operative correction of a middle third fracture of the face, a patient suffered unilateral tearing and homolateral rhinorrhea as soon as he started sweating. This is explained by abnormal regeneration of autonomic nerve fibres. The possible anatomic pathways are discussed and compared with similar syndromes in the literature.

Acquired sixth-nerve palsies are relatively rare in younger adults. We reexamined 49 patients, aged from 15 to 50 years, with isolated sixth-nerve palsies who were seen between 1972 and 1982 at the Wills Eye Hospital in Philadelphia. In this group, the following etiologies were encountered: vasculopathy (14 patients [29%]), tumors (eight patients [16%]), multiple sclerosis (six patients [12%]), presumed inflammation (four patients [8%]), trauma (three patients [6%]), postlumbar puncture (two patients [4%]), and orbital amyloidosis (one patient [2%]). Eleven patients (22%) had no determined cau
