Treatment of recurrent trigeminal neuralgia due to Teflon granuloma.

Recurrent trigeminal neuralgia after microvascular decompression (MVD) may be due to insufficient decompression, dislocation of the implant to pad the neurovascular contact, or the development of granuloma. Here, we report on our experience with Teflon granuloma including its treatment and histopathological examination. In a series of 200 patients with trigeminal neuralgia MVD was performed with Teflon felt according to Jannetta's technique. In three patients with recurrent facial pain Teflon granuloma was found to be the cause for recurrence. In each instance, the granuloma was removed for hi

Perioperative complications after translabyrinthine removal of large or giant vestibular schwannoma: Outcomes for 123 patients.

CONCLUSION: Large vestibular schwannomas are benign but dangerous tumors. The translabyrinthine approach allows the surgeon to limit vital and functional complications due to the disease itself or to its surgical removal. OBJECTIVE: Morbi-mortality study focused on large vestibular schwannoma surgically treated by translabyrinthine removal. METHODS: This was a retrospective review of prospectively collected data in a series of 123 patients who underwent translabyrinthine removal of a large vestibular schwannoma (>4 cm in the cerebellopontine angle, stage IV). All surgical and medical complic

[Identification techniques of the recurrent laryngeal nerve in endoscopic thyroidectomy].

OBJECTIVE: To investigate the application of identification techniques of the recurrent laryngeal nerve (RLN) in endothyroidectomies. METHODS: Routine identification of the RLN was performed in a series of 20 consecutive endothyroidectomies, and the clinical data were reviewed. RESULTS: Totally 20 RLNs were dissected. Neither transient nor permanent RLN injury occurred. CONCLUSION: To expose RLN, both sharp and blunt dissection should be applied skillfully. Thorough liberation of the thyroid lobe is essential for the identification of RLN. It is safe and feasible to remove the thyroid and i

Radial diffusivity in remote optic neuritis discriminates visual outcomes.

OBJECTIVE: Diffusion tensor imaging (DTI) quantifies Brownian motion of water within tissue. The goal of this study was to test whether, following a remote episode of optic neuritis (ON), breakdown of myelin and axons within the optic nerve could be detected by alterations in DTI parameters, and whether these alterations would correlate with visual loss. METHODS: Seventy subjects with a history of ON > or =6 months prior underwent DTI of the optic nerves, assessment of visual acuities (VA) and contrast sensitivities (CS), and laboratory measures of visual evoked potentials (VEP) and optical c

[Intraoperative neuromonitoring of thyroid gland operations : Surgical standards and aspects of expert assessment].

Intraoperative neuromonitoring (IONM) was introduced into thyroid surgery approximately 10 years ago for better identification of recurrent laryngeal nerve palsy. Since then several studies have been performed for evaluation of this new technology. IONM is superior to visual nerve identification alone for prediction of postoperative local cord function. Therefore, in bilateral procedures IONM enables intraoperative decision-making concerning resection of the second side. To avoid misinterpretation of the results of IONM a standardized approach including preoperative and postoperative laryngosc

Neurotoxicity of quinolinic acid to spiral ganglion cells in rats.

Our study investigated the neurotoxicity of quinolinic acid (QA) to spiral ganglion cells (SGCs), observed the protective effects of N-methyl-D-aspartate (NMDA) receptor antagonist MK-801 and magnesium ions on the QA-induced injury to SGCs, and analyzed the role of QA in otitis media with effusion (OME)-induced sensorineural hearing loss (SNHL). After culture in vitro for 72 h, SGCs were exposed to different media and divided into 4 groups: the blank control group, the QA injury group, the MK-801 treatment group, and the MgCl2 protection group. The apoptosis rate of SGCs was analyzed by Annexi

Can satellite glial cells be therapeutic targets for pain control?

Satellite glial cells (SGCs) undergo phenotypic changes and divide the following injury into a peripheral nerve. Nerve injury, also elicits an immune response and several antigen-presenting cells are found in close proximity to SGCs. Silencing SCG-specific molecules involved in intercellular transport (Connexin 43) or glutamate recycling (glutamine synthase) can dramatically alter nociceptive responses of normal and nerve-injured rats. Transducing SGCs with glutamic acid decarboxylase can produce analgesia in models of trigeminal pain. Taken together these data suggest that SGCs may play a rol

Endoscopic endonasal trans-sphenoidal optic nerve decompression for traumatic optic neuropathy--technical note.

The endoscopic endonasal trans-sphenoidal approach can decompress the traumatized and swollen optic nerve without brain retraction. Three cases of traumatic optic neuropathy (TON) were treated by this endoscopic approach. We performed 58 endoscopic endonasal trans-sphenoidal approaches in 52 patients between April 2006 and December 2007. Three patients, 2 men and a woman aged 42-66 years, with TON were treated by endoscopic endonasal trans-sphenoidal optic nerve decompression. Adequate decompression of the optic canal was observed in all three patients. Two experienced improved visual function

Brainstem infarction in a patient with internal carotid dissection and persistent trigeminal artery: a case report.

BACKGROUND: The primitive trigeminal artery (PTA) is the most commonly described fetal anastomosis between the carotid and vertebrobasilar circulations. CASE PRESENTATION: We report a 42-year-old patient presenting with internal carotid dissection, and imaging features of brainstem infarction. CONCLUSION: Based on the imaging studies we presume occlusive carotid dissection with extensive thrombosis within a persistent trigeminal artery as the cause of this brainstem ischemia.

Evaluation of recurrent laryngeal nerve monitoring in thyroid surgery.

AIM: Thyroidectomy creates a potential risk for all parathyroid glands and nerves. Nerve identification has decreased the rates of nerve injury during thyroidectomy. Intraoperative nerve monitoring (IONM) has been used as an adjunct to the visual identification of the nerve. The aim of this clinical trial is to evaluate the effect of the identification time of RLN during thyroidectomy using IONM. METHOD: Two hundred and thirty seven patients and 409 nerves at risk were enrolled in this prospective study. The nerves in Group 1 (n=210) were identified with IONM, whereas the nerves in Group 2 (n

Repair of the trigeminal nerve: a review.

Nerve surgery in the maxillofacial region is confined to the trigeminal and facial nerves and their branches. The trigeminal nerve can be damaged as a result of trauma, local anaesthesia, tumour removal and implant placement but the most common cause relates to the removal of teeth, particularly the inferior alveolar and lingual nerves following third molar surgery. The timing of nerve repair is controversial but it is generally accepted that primary repair at the time of injury is the best time to repair the nerve but it is often a closed injury and the operator does not know the nerve is inj

Binaural cochlear implantation after bilateral temporal bone fractures.

In this clinical note we discuss the indications, feasibility, and outcomes of binaural simultaneous cochlear implantation (CI) following bilateral transverse temporal bone (TB) fractures. A 41-year-old male, totally deaf after a bilateral TB fracture, underwent an audiological, electrophysiological, and imaging investigation in order to assess the integrity of the VIIIth cranial nerves. Five months later he received a simultaneous bilateral CI. Speech perception tests were conducted at different time points. A significant advantage by dichotic listening was observed since the beginning of the
