The value of intraoperative neuromonitoring and neurostimulation in thyroid surgery: A single-center retrospective analysis and prospective follow-up.

This study aims to determine the accuracy of neurostimulation with laryngeal palpation (NSLP) and intermittent intraoperative neuromonitoring (I-IONM) to predict the postoperative function of the recurrent laryngeal nerve during thyroid surgery and to follow up patients with vocal fold paralysis (VFP). This was a historical single-center retrospective cohort study with a prospective follow-up. A total of 1471 patients (2408 nerves at risk) comparing I-IONM and NSLP for patients undergoing hemi-, total, or completion thyroidectomy from January 2009 to January 2024. The I-IONM group included 810

Comparison of Suprasellar Cistern Height Measurements Between Coronal and Sagittal Planes.

BACKGROUND: The Bern score was developed for cerebral spinal fluid leak determination and spontaneous intracranial hypotension (SIH) diagnosis. Suprasellar cistern height (SCH) is an important imaging sign in Bern score. However, the way to measure SCH is vague, and results from different studies vary. METHODS: This case-control study included SIH patients presented to our institution. Blind reviews were conducted by 2 readers for magnetic resonance images of SIH patients and control participants. SCHs on coronal and sagittal sections were measured. Intermethod agreement was evaluated with in

Impact of intraoperative recurrent laryngeal nerve monitoring on minimally invasive esophagectomy outcomes for esophageal cancer: a meta-analysis of case-control studies.

Esophagectomy and lymphadenectomy for esophageal cancer carry an inherent risk of recurrent laryngeal nerve (RLN) injury. Intraoperative nerve monitoring (IONM) may help prevent RLN damage, though evidence on its effectiveness is still limited. This systematic review and meta-analysis (SRMA) evaluate the feasibility and efficacy of IONM during minimally invasive esophagectomy (MIE) for esophageal cancer. This SRMA followed Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) and Meta-Analysis of Observational Studies in Epidemiology (MOOSE) guidelines, with a registered

Impact of different lingual split patterns on inferior alveolar nerve recovery after bilateral sagittal split osteotomy in patients with skeletal class III malocclusion.

This study investigated the impact of different lingual split patterns on inferior alveolar nerve (IAN) recovery following bilateral sagittal split osteotomy (BSSO). Patients with skeletal class III malocclusion who underwent BSSO between April 2021 and December 2023 were included. Lingual fracture lines were categorized into four types using the Lingual Split Scale (LSS). Nerve recovery in the chin and lower lip was assessed through monofilament detection, and dual logistic regression was used to analyze the relationship between lingual split patterns and IAN recovery. Among the 114 lingual f

Comparison of two surgical techniques for the extraction of mandibular horizontally impacted third molars near the mandibular canal: the displacement reduction method and the away-from-nerve method.

OBJECTIVES: To introduce two surgical techniques for extracting horizontally impacted wisdom teeth near the mandibular nerve canal and to assess their effectiveness. MATERIALS AND METHODS: This study involved patients requiring extraction of horizontally impacted wisdom teeth near the mandibular canal from January to December 2024. Cases were grouped into three categories: close, compressing, and intruding into the mandibular canal. Depending on the tooth's position relative to the nerve canal, it was classified as lingual, middle, or buccal. Extractions were carried out using either the disp

Selective visualization of the recurrent laryngeal nerve during parathyroidectomy.

INTRODUCTION: Direct visualization of the recurrent laryngeal nerve (RLN) is considered the gold standard for avoiding nerve injury during parathyroidectomy. We hypothesize that with routine intraoperative nerve monitoring (IONM), RLN visualization is not mandatory for safe parathyroidectomy. METHODS: Following institutional review board approval, adult patients undergoing parathyroidectomy using IONM during a two-year period were reviewed. The RLN was only visualized if exposed during parathyroid gland dissection. Outcomes included the rate of nerve visualization and rates of RLN injury. RE

Intraoperative mapping of the recurrent laryngeal nerve and parathyroid glands to improve thyroid surgery outcomes.

PURPOSE: To perform an intraoperative topographic analysis of the recurrent laryngeal nerve (RLN) and parathyroid glands during thyroid and parathyroid surgery, focusing on their relationship with the inferior thyroid artery (ITA), RLN branching patterns, and glandular positioning. Based on a high-volume surgical cohort, the study proposes a standardized dissection approach to reduce RLN injury and postoperative hypoparathyroidism. METHODS: A retrospective cohort of 500 patients undergoing thyroidectomy and/or parathyroidectomy between 2011 and 2021 was analyzed. All cases followed a uniform

Comparative analysis of facial nerve outcomes in petroclival versus posterior petrous meningioma surgery: A systematic review and meta-analysis of 2884 patients.

BACKGROUND: Even conservative resection of petroclival (PC) and posterior petrous (PP) meningiomas may cause facial nerve (FN) morbidities. While inconsistent classifications by tumor's dural origin circulate among different cohorts, a comprehensive assessment of postoperative FN outcomes is lacking. We systematically evaluate early and long-term FN outcomes related to surgical extirpation of PC and PP meningiomas. METHODS: A PRISMA-tailored survey of PubMed, Embase, Ovid/Medline, and Scopus was used to assess pooled perioperative FN deficit rates in PC and PP meningiomas. Outcomes were dicho

Evaluation of the Incidence of Thyroidectomy-induced Recurrent Laryngeal Nerve Injuries and Their Risk Factors Using Acoustic Analysis: An Observational Study.

BACKGROUND: Thyroidectomy is one of the most common operations performed, with recurrent laryngeal nerve injury (RLNI), both complete and incomplete, being one of the most serious potential complications. In this study, we used acoustic analysis of jitter, shimmer, noise-to-harmonic ratio (NHR), and maximum phonation time (MPT) as an objective method of diagnosing RLNI. MATERIALS AND METHODS: We evaluated the mean values of jitter, shimmer, NHR, and MPT in patients undergoing thyroidectomy. These values, in addition to laryngoscopy, were measured preoperatively, at 1 week post operatively, an

A rare mental foramen variation and the role of cone-beam computed tomography in preventing nerve injury: A case report.

BACKGROUND: The interforaminal space is frequently used for dental implant placement due to its strategic location between the bilateral mental foramina. However, neurosensory disturbances following surgical intervention near the mental foramen (MF) can occur in up to 43% of cases. Understanding anatomical variations of the MF is crucial to preventing surgical complications. Traditional two-dimensional radiographs have limitations in accurately visualizing anatomical structures, whereas cone-beam computed tomography (CBCT) provides high-resolution three-dimensional imaging that enhances diagno

[Surgical Strategies for Trigeminal Schwannomas].

Trigeminal schwannomas are rare, benign tumors with diverse extension patterns along the trigeminal nerve, which significantly influence surgical approach selection. This article presents strategies for selecting optimal approaches based on tumor location, classified into middle, posterior, and extracranial compartments. Commonly used approaches include the anterior transpetrosal, epidural and interdural approaches, and recently introduced minimally invasive techniques, such as the endoscopic endonasal transmaxillary-pterygoid and transorbital approaches. The anatomical accessibility, safety,

Recurrent laryngeal nerve paralysis as a potential mediator of complications in esophagectomy following lymph node dissection.

Complications from esophagectomy often interact with each other, with those related to recurrent laryngeal nerve (RLN) paralysis (RLNP) being particularly significant. Aggressive dissection of RLN lymph nodes (RLN-LNs) is considered a major contributing factor to RLNP. This study seeks to validate the hypothesis that RLNP acts as a mediator, not only resulting from RLN-LN dissection but also amplifying the likelihood of other postoperative complications. Data were retrospectively extracted from the Chinese 12th Five-Year Major Science and Technology Project on esophageal diseases, including a

Development and Validation of a Clinically Applicable Nomogram for Predicting Inferior Alveolar Nerve Injury After Mandibular Cystectomy.

BACKGROUND: Mandibular cystectomy carries a considerable risk of inferior alveolar nerve injury (IANI); however, current evidence on modifiable preoperative imaging-based risk predictors remains limited. This knowledge gap underscores the critical need for developing advanced risk stratification models to optimize surgical decision-making. PURPOSE: The purpose of this study was to systematically identify key imaging-derived risk factors for IANI following mandibular cystectomy, quantify their associations, and construct/validate a predictive nomogram for personalized preoperative risk assessm

Reducing complications using minimally invasive retrosigmoid approach for microvascular decompression in patients with trigeminal neuralgia.

BACKGROUND: The most effective treatment for a typical trigeminal neuralgia is microvascular decompression of the trigeminal nerve. An approximately 3-5 cm in diameter retrosigmoid craniotomy with an approximately 12-14 cm long skin incision is considered the standard approach. Modern neurosurgery strives for minimally invasive surgical approaches, which in turn reduces both intraoperative and postoperative complications. The use of minimally invasive approaches in skull base neurosurgery and, in particular, in microvascular decompression of the trigeminal nerve can reduce the risk of complica

[Management of accidental tooth root displacement into the mandibular canal during tooth extraction].

The accidental displacement of tooth roots into the mandibular canal is a serious complication during tooth extractions in oral and maxillofacial surgery, often resulting in direct damage to the structural and functional integrity of the inferior alveolar neurovascular bundle (IANB). This article reviews the anatomical features of the mandibular canal, the IANB, and adjacent tooth roots; identifies high-risk factors and anatomically vulnerable sites for root displacement; and outlines the clinical manifestations and radiographic characteristics of intraoperative root intrusion into the mandibu

Dysfunction of pancreatic exocrine secretion after experimental spinal cord injury.

Pancreatic exocrine dysfunction is an underdiagnosed comorbidity in individuals living with spinal cord injury (SCI) who often present cholestasis, acute pancreatitis or high levels of serum pancreatic enzymes. Parasympathetic control of pancreatic exocrine secretion (PES) is mediated in the medullary dorsal vagal complex in part through cholecystokinin (CCK) release. Our previous reports indicate high thoracic (T3-) SCI reduces vagal afferent sensitivity to GI regulatory peptides, like CCK and thyrotropin releasing hormone (TRH). To date, the effects of experimental SCI on PES are unknown. He
