A Case of Isolated Cervicomedullary Injury With Extensive Wallerian Degeneration: The Multifactorial Nature in Making Decisions to Withdraw Life Support From a Pathology Perspective.

Patients suffering from traumatic spinal cord injury (SCI) constitute a unique patient population in critical care medicine due to the high rate of mortality, irrecoverable neurological damage, and dependence on advanced life support measures. In this report, we present a patient who sustained a non-survivable cervicomedullary injury due to motor vehicle collision who became dependent on maximal life support and subsequently expired from multiple complications 11 months after the initial accident. On neuropathological exam, extensive Wallerian degeneration in the spinal cord despite non-trauma

Posterior Facet Distraction and Fusion Technique: Driving a Paradigm Shift in the Surgical Algorithm for Atlantoaxial Dislocation.

BACKGROUND AND OBJECTIVES: Atlantoaxial dislocation (AAD) remains one of the most technically challenging disorders of the craniovertebral junction. Traditional combined anterior-posterior approaches, although effective, carry substantial morbidity. The posterior facet distraction and fusion (PFDF) technique achieves reduction through sequential facet release and distraction, offering a safer posterior-only alternative. This single-center retrospective study aimed to evaluate the reduction efficacy, clinical outcomes, and safety of a PFDF-centered surgical strategy, and to develop and validate

Selective spinal interneuron activation enhances the hypercapnic ventilatory response in chronic spinal cord injury.

The ability to sense and regulate breathing in response to changes in carbon dioxide (CO₂) levels is vital for maintaining respiratory and systemic homeostasis. Impaired CO2 chemosensitivity is a common feature of several debilitating conditions, including chronic obstructive pulmonary disease (COPD), central hypoventilation syndromes, and cervical spinal cord injury (cSCI) and contributes to significant respiratory complications such as reduced ventilatory drive, poor gas exchange, and diminished adaptability to metabolic demands. Chronic cSCI disrupts descending respiratory pathways, leading

Spinal Cord Stimulation for Pain Management Following Spinal Cord Injury: A Systematic Review.

PURPOSE: Chronic pain affects more than 80% of individuals with spinal cord injury (SCI), profoundly impairing mobility, rehabilitation, and quality of life. Conventional pharmacologic and non-pharmacologic treatments often fail to achieve adequate relief. Spinal cord stimulation (SCS) has emerged as a potential neuromodulatory therapy for neuropathic pain; however, its role in SCI remains insufficiently defined. PATIENTS AND METHODS: A systematic review was conducted in accordance with PRISMA 2020 guidelines and registered with PROSPERO (CRD42024541622). Comprehensive searches of PubMed, Emb

Hypoglossal motor output is altered by C4 epidural electrical stimulation via ascending spinal and peripheral feedback pathways.

After cervical spinal cord injury (cSCI), swallowing dysfunction is common and increases mortality via aspiration pneumonia. While these deficits have often been attributed to secondary damage from complications of injury management, there has recently been a greater appreciation for the modulatory role of spinal populations in swallow generation that are disrupted by injury. Here, we illustrate in a rodent model of cSCI that epidural spinal stimulation (ESS) of the phrenic motor nucleus at spinal segment C4 alters motor output at the hypoglossal motor nucleus through activation of excitatory

Is MRI required to assess CT-negative traumatic cervical spine tenderness without focal neurologic deficit?

OBJECTIVES: Acute cervical spine injury is an uncommon but serious sequela of blunt trauma. Patients presenting with multi-trauma to the Emergency Department usually receive CT imaging for cervical spine clearance; however, many of these patients have ongoing pain despite a negative CT and proceed to an MRI. This can entail delays in care, transfer between hospitals, prolonged periods of cervical spine immobilisation, and increased healthcare costs. The aim of the study is to identify the rate of acute cervical spine injury detected on MRI following negative CT and the impact of these findings

Many but not all pathogen-associated molecular patterns aggravate neurogenic heterotopic ossification after spinal cord injury.

BACKGROUND: Neurogenic heterotopic ossifications (NHOs) are heterotopic bones that develop in periarticular muscles after traumatic brain (TBI) and spinal cord injuries (SCI). The mechanisms leading to NHO are incompletely understood and the only effective treatment to-date remains surgical resection. We previously established that several inflammatory pathways drive NHO pathogenesis in injured muscles in a mouse model of NHO and in humans. We also demonstrated a functional association between gram-negative bacterial infections and NHO development via lipopolysaccharide (LPS), a pathogen assoc

Desynchronise, disconnect, reorganise, collapse (DDRC): A theoretical framework linking white matter pathology and cognitive dysfunction in vascular disease.

White matter health is closely associated with cognitive function, particularly across the spectrum of cerebrovascular health. However, inconsistencies in the cognitive domains reported to be affected by white matter pathology have complicated efforts to define clear mechanisms of dysfunction. This review synthesises neuroimaging and neuropathological evidence to reveal the cascade of cognitive and neural consequences that follow white matter injury, from microstructural alterations to macroscale white matter hyperintensities. We discuss the contributions of grey matter atrophy and disrupted f

Making the Most of Pharmacologic Interventions: Urodynamic Effects of Different Therapies for Neurogenic Lower Urinary Tract Dysfunction Following Spinal Cord Injury.

Management of neurogenic lower urinary tract dysfunction in individuals with spinal cord injury (SCI) involves consideration of patient-specific and bladder-specific factors. Although individuals with SCI prioritize treatment of their symptoms, providers should pay equal, if not more, attention to addressing urodynamic (UDS) parameters such as neurogenic detrusor overactivity (NDO) and impaired compliance due to the associated risk for upper tract deterioration. Anticholinergics, β-3 agonists, and onabotulinumtoxin A (BTX-A) are guideline-supported pharmacotherapies. Though each therapy variab

Development and internal validation of a risk prediction calculator for minor spinal cord injury in CT-negative blunt trauma.

BACKGROUND: Clinical calculators are used to determine which trauma patients require computed tomography (CT) scans of the spine. However, mild traumatic spinal cord injury (TSCI) may be present despite a negative CT scan. Therefore, the present study sought to internally validate the use of two calculators (whole spine and cervical-only) to identify such patients. METHODS: The Spinal Cord Injury Model System Program was used to conduct this retrospective cohort study. Patients at least 15 years old with an American Spinal Injury Association (ASIA) grade D injury due to blunt trauma were incl

Long-term complication rates and healthcare utilization after traumatic spinal cord injury are more strongly associated with insurance status than injury mechanism.

BACKGROUND: Injury mechanism influences early neurologic recovery after SCI, but long-term outcomes may also depend on access to post-acute care. The relative contributions of injury etiology and insurance status remain unclear. This retrospective longitudinal cohort study compares long-term complications and healthcare utilization after spinal cord injury (SCI) due to gunshot wounds (GSW) versus motor vehicle accidents (MVA) and evaluates the effect of insurance status on these outcomes. METHODS: Adults with traumatic SCI due to GSW or MVA who completed inpatient rehabilitation were followed

Imbalance of nociceptive homeostasis drives spinal cord injury pain.

Somatosensory synaptic transmission is an adapting phenomenon according to changes of synaptic environments composed of neurons and glial cells. Synaptic architecture in the spinal dorsal horn well recognizes and identifies sensory modalities, such as nociceptive and non-nociceptive, however, sensory neurons do not identify only single modality, therefore, synaptic transmission is dynamic and phasic events. More importantly, maladaptive synaptic plasticity developed chronically and critically drives the new setup of synaptic environments that results in enhanced or decreased synaptic transmiss

Remarkable Recovery After Delayed High-Dose Methylprednisolone in a Rare Case of Penetrating Spinal Cord Injury.

Traumatic spinal cord injury (TSCI) caused by sharp-force penetration is exceptionally rare, and the use of high-dose methylprednisolone (MP) remains highly controversial, especially beyond the conventional 8-h treatment window. This case report describes a 30-year-old male with acute incomplete TSCI following a knife stab wound to the right neck. Despite no initial improvement, high-dose MP (1000 mg/day for 3 days, tapered thereafter) was initiated 40 h post-injury, along with mannitol. Remarkable neurological recovery was observed, including independent ambulation by post-injury day 25. Foll
