Endoscopic surgery for thoracolumbar spinal metastases: a technical case series.

OBJECTIVE: With its favorable profile of enhanced recovery and a reduced perioperative footprint, there is growing evidence for endoscopic spine surgery (ESS) in the management of thoracolumbar spinal malignancies. The aim of this study was to explore the conceptual framework of the endoscopic benefit zone in the management of metastatic spinal disease, and present an Australasian-first technical case series, establishing its feasibility and outcomes in this cohort. METHODS: Between 2021 and 2026, 7 patients (4 female, mean age 56) who were diagnosed with spinal metastases underwent ESS as pa

Beyond Clinical Predictors: Determining the Impact of Psychosocial Factors in Chronic Pain After Spinal Cord Injury.

UNLABELLED: Although chronic pain is common after traumatic spinal cord injury (SCI) and prognostic models have traditionally prioritized clinical injury characteristics, the relative contribution of sociodemographic and psychological factors remains undercharacterized. METHODS: Secondary analysis of a national longitudinal cohort from United States inpatient rehabilitation centers participating in the Spinal Cord Injury Model Systems. Adults with traumatic SCI (N=1303) completed Spinal Cord Injury Model Systems Form 1 and a 1-year follow-up Form 2 interview. Pain intensity at 1 year was asse

Relationship between orthostatic hypotension, internal carotid artery blood flow, and cognition in patients with cervical spinal cord injury.

INTRODUCTION: This study aimed to identify the distribution of orthostatic hypotension (OH) according to changes in internal carotid artery (ICA) blood flow in patients with cervical spinal cord injury (SCI), and to determine whether cognitive impairment is more prominent among patients with a certain level of ICA blood flow reduction. METHODS: Forty-one patients diagnosed with cervical SCI were retrospectively analyzed and classified into two groups based on the results of a head-up tilt test and carotid duplex ultrasonography to measure ICA blood flow: those with a reduction of 21% or more

Effects of an Adaptive Exercise Program on Inflammation After Spinal Cord Injury.

BACKGROUND: Chronic inflammation contributes to cardiovascular and respiratory diseases, the leading causes of death in individuals with spinal cord injury (SCI). People with SCI tend to have higher inflammation levels than able-bodied individuals and are often physically inactive despite the anti-inflammatory benefits of exercise. OBJECTIVES: This study evaluated whether an adaptive exercise program could reduce inflammation in individuals with SCI. METHODS: Twenty-four individuals with SCI were randomized to either an immediate start (intervention) or delayed start (control) group. The 12-

Transient Paraparesis from Post-traumatic Muscle Herniation Through Laminectomy Defect: Rare Sequela of Thoracolumbar Kyphosis Correction: A Case Report.

CASE: An 8-year-old girl with congenital thoracolumbar kyphosis underwent posterior instrumented correction with a closing-opening wedge osteotomy. Seven years later, minor trauma caused acute paraparesis. Imaging showed intact implants and solid fusion, but muscle herniation into the laminectomy defect had impinged the spinal cord. Steroids and physiotherapy led to full recovery within 1 month, confirmed at 1-year and 3-year postinjury follow-up. CONCLUSION: This case highlights transient paraparesis from soft-tissue impingement years after correction, underscoring the exposed apical cord's

Healthcare Professionals' Experiences of the Causes of Recurrent Pressure Injuries-A Phenomenographic Study.

There are few previous studies regarding recurrent pressure injuries, most of which investigate surgical outcomes. From the perspective of the healthcare system, recurrent pressure ulcers may indicate shortcomings in quality, as preventive measures have failed not just once, but on several occasions. We aimed to examine how healthcare professionals working with wound care in specialised inpatient care perceived the causes of recurrent pressure injuries in patients with spinal cord injuries. A phenomenographic study using in-depth interviews was conducted. We identified four categories of the e

Scrambler Therapy for Neuropathic Pain After Spinal Cord Injury: Two Case Reports.

Neuropathic pain after spinal cord injury (SCI) is common and often refractory to treatment. Scrambler Therapy (ST) is a noninvasive neuromodulation approach that delivers synthetic "nonpain" signals via cutaneous electrodes. We report 2 cases of SCI-related neuropathic pain treated with ST. One patient experienced transient and inconsistent relief, while the other achieved substantial, durable improvement with functional recovery and complete response after retreatment. ST was well tolerated with minimal adverse effects. These cases highlight variable response and support further study of ST

Use of fascial plane blocks for traumatic rib fractures: a scoping review.

BACKGROUND: The primary cause of morbidity and mortality in traumatic rib fractures is respiratory complications due to compromised respiratory mechanics secondary to pain and opioid-related respiratory depression. Thoracic epidural analgesia (TEA) provides effective analgesia but may not be possible in patients due to spinal cord injuries, thoracic vertebral fractures, and coagulopathy. New thoracic fascial plane blocks provide new options for patients with multiple rib fractures (MRFs). OBJECTIVE: Our primary objective was to assess the effectiveness of thoracic fascial plane blocks for pat

Assessing sexual function in neurorehabilitation: insights and challenges from a spinal cord injury cohort.

BACKGROUND: Although sexual dysfunction after spinal cord injury (SCI) is common and impacts quality of life, healthcare professionals may avoid this topic due to discomfort and knowledge gaps. Missing sexual health data can bias research findings, yet this issue remains largely unexplored in the context of SCI. AIM: To describe missing sexual function data compared to other clinical domains and evaluate the association between changes in data collection procedures and missing data in females and males during post-acute specialized SCI rehabilitation. METHODS: Data from the population-based,

Reduced Post-Activation Depression in Humans with Spinal Cord Injury Is Not Related to Stretch Reflex Hyperexcitability.

Post-activation depression of spinal reflexes is reduced following central motor lesions, including spinal cord injury (SCI), and has long been proposed to contribute to signs and symptoms associated with spasticity. To evaluate this hypothesis, we assessed post-activation depression in individuals with chronic SCI, both males and females with and without stretch reflex hyperexcitability (SRH) as well as in age-matched controls. Post-activation depression was assessed by recording the soleus H-reflex every 10 s using paired pulses at interstimulus intervals (ISIs) of 1, 2, and 4 s, and test re

Outcomes of Tracheostomy Decannulation: Study Protocol for a Longitudinal Observational Study.

BACKGROUND: Tracheostomy management aims to achieve optimal decannulation outcomes in tracheostomized patients. Successful removal of the tracheal cannula (decannulation) is key from a physical and psychological health perspective. Decannulation success is commonly defined as the absence of physical complications leading to re-cannulation within a few hours or days post-decannulation. However, this definition does not account for complications occurring weeks or months later, or for (serious) complications that require actions other than re-cannulation. Additionally, the definition neglects th

Family Partnership in Hypnosis to Improve Mechanical Ventilation Weaning in an ICU Patient With High Cervical Spinal Cord Injury-A Case Report.

Quadriplegia often leads to severe respiratory impairment, accompanied by distressing symptoms such as dyspnoea and breathlessness. Ventilator weaning remains a challenge in the acute phase of high cervical spinal cord injury. We present the case of a 27-year-old man with traumatic C5-C6 quadriplegia. His clinical course was complicated by recurrent respiratory issues and a prolonged ventilator weaning process. Medical hypnosis was introduced as a complementary strategy to support ventilator weaning. A nurse anaesthetist certified in medical hypnosis performed the intervention. As sedation lim
