Electroacupuncture alleviates depression-like behavior after spinal cord injury via SIRT1/HMGB1 signaling axis mediated ferroptosis.

BACKGROUND: Spinal cord injury (SCI) induces neuroinflammation and brain alterations that contribute to depression-like behaviors. Ferroptosis plays a critical role in neuronal injury following SCI. HMGB1 regulates ferroptosis, while SIRT1 inhibits HMGB1 activity. Electroacupuncture (EA) has demonstrated neuroprotective and antidepressant effects, but its mechanisms in SCI remain unclear. This study investigated whether EA alleviates post-SCI depression by activating SIRT1, retaining HMGB1 in the nucleus, and limiting ferroptosis in the prefrontal cortex (PFC). METHODS: A moderate SCI model w

Silencing of Ifi27l2a Attenuates Inflammation After Spinal Cord Injury by Regulating Microglial Polarization via JAK2/STAT3 Signaling.

Microglial polarization toward M1/M2 phenotypes is crucial in modulating neuroinflammation following spinal cord injury (SCI). This study aimed to investigate the role of interferon alpha-inducible protein 27-like 2A (Ifi27l2a) in regulating microglial polarization in SCI. The expression of Ifi27l2a were analyzed using single-cell RNA sequencing. C57BL/6 mice that underwent SCI were pretreated with adeno-associated virus (AAV) carrying sh-Ifi27l2a. In vitro, BV-2 cells were transfected with si-Ifi27l2a and stimulated with lipopolysaccharide (LPS). The effects of Ifi27l2a silencing were assesse

Sex differences in pain-related behaviour and the endocannabinoid system following hind limb ischemia-reperfusion injury.

Ischemia-reperfusion (I/R) injury causes tissue damage in organ transplantation, stroke and myocardial infarction, and underlies chronic wound formation. The endocannabinoid system (ECS) is involved in pain modulation, maintenance of skin homeostasis, and ischemic preconditioning, an endogenous protective mechanism against I/R-mediated damage. The rodent hind limb I/R model is a model of chronic post-ischemia pain, complex regional pain syndrome-type 1 (CRPS-1), and a potential model of wound-related pain. Research into sex differences in pain-related behaviour and the ECS following hind limb

Exosomes-mediated delivery of miR-27a-3p antagomir alleviates white matter injury by regulating PPARγ/PRDX1/JNK pathway after subarachnoid hemorrhage in rats.

White matter injury (WMI) is a critical factor contributing to poor neurological outcomes following subarachnoid hemorrhage (SAH). MicroRNAs (miRNAs) are key regulators of WMI-related pathology and can be delivered via exosomes, yet their mechanisms and therapeutic potential remain largely unexplored. In this study, miRNA sequencing revealed a significant upregulation of miR-27a-3p in peripheral blood exosomes after SAH, which was further confirmed in white matter tissue. BV2 cell-derived exosomes loaded with miR-27a-3p antagomir were administered intranasally and effectively targeted oligoden

Development and clinical validation of a stroke-specific Gait Deviation Index.

Stroke affects over 101 million individuals worldwide, leaving many to cope with long-term consequences. These often include physical and cognitive impairments such as muscle weakness, loss of coordination, and challenges in performing daily activities. Several quantitative indices have been developed to objectively quantify gait deviations with a focus on ensuring ease of use and accessibility within clinical settings. Among those, the most cited one is the Gait Deviation Index (GDI), originally developed with data from a population of children with cerebral palsy. This characteristic probabl

High-Frequency Spinal Cord Stimulation for the Treatment of Spasticity: A Preliminary Case Series.

Background: Spasticity is a complex and multifactorial condition resulting from upper motor neuron injury. It manifests through muscle contractions, pain, limited range of motion, and clonus, which significantly impair daily activities and quality of life. High-frequency spinal cord stimulation (HF SCS) has shown optimal results in treating chronic neuropathic pain, but its potential role in spasticity remains underexplored. This study aimed to evaluate the efficacy of HF SCS in patients with spasticity. Methods: From April 2021 to July 2024, six patients with spasticity from various etiologie

Peripheral immunosuppressive and immunostimulatory signatures of severity and pain in spinal cord injury.

Spinal cord injury is a severe neurological condition associated with an increased risk of infection, an elevated mortality rate, and often results in chronic neuropathic pain. The peripheral immune changes in the chronic phase of spinal cord injury are largely unknown, as is their capacity to facilitate neuropathic pain. The current study used a high-dimensional single-cell analysis of peripheral blood to evaluate major immune populations and functional markers in individuals with chronic spinal injury. Spinal cord injury was associated with a shift towards classical monocytes and an increase

Effects of Robot-Assisted Gait Therapy on Urodynamic Changes in the Subacute Phase After Spinal Cord Injury: A Prospective Study.

BACKGROUND Approximately 70% to 84% of patients with spinal cord injury (SCI) have varying degrees of bladder dysfunction, which can lead to upper urinary tract complications. Urodynamics is the standard method for evaluating bladder function after SCI. The effect of robot-assisted gait therapy (RAGT) on urinary function is rarely reported in the literature. The aim of this study was to evaluate the changes in urodynamics in SCI patients undergoing RAGT. MATERIAL AND METHODS The study included 35 SCI patients (26 males and 9 females) with neurological impairment at between 3 months and 2 years

Neuropathic pain after spinal cord injury: Mechanisms, animal models and pain assessments.

Neuropathic pain after spinal cord injury (SCI-NP) often has lifelong and significant negative effects. Therefore, understanding its underlying mechanisms is a current research priority. SCI-NP involves central sensitization, neuroinflammation and functional remodeling in the brain, hyperexcitability of primary sensory neurons, and peripheral-central interactions. The mechanism of SCI-NP at the spinal cord level is an important one to study. Glial cell activation and proinflammatory pathways in the spinal cord are the key drivers that lead to central sensitization at the spinal cord level, and

Impact of trauma center volume on treatment strategies and outcomes of blunt traumatic aortic injuries.

OBJECTIVE: Management of blunt traumatic aortic injuries (BTAIs) has changed significantly over time. There is now increasing support for medical management of minimal aortic injuries (MAIs; Grades I and II) and thoracic endovascular aortic repair (TEVAR) in more severe injuries (Grades III and IV). Data on real-world management of BTAIs, from medical therapy to surgical intervention, remains limited. Available literature predominately focuses on outcomes after intervention, rather than the spectrum of BTAI care that includes medical management. Furthermore, it remains unclear whether the trau

Dendritic spine dysgenesis in spinal cord injury: A structural contributor to pain and spasticity.

Pain and spasticity are common consequences of spinal cord injury (SCI) that profoundly diminish quality of life. Although pain arises from sensory pathways and spasticity from motor pathways, both reflect post-injury mechanisms that renders spinal circuits hyperexcitable. Dendritic spines-specialized protrusions on neuronal dendrites that mediate excitatory synaptic transmission-undergo striking structural remodeling after SCI. Abnormal spine morphology has been documented in both the superficial and deeper laminae of the dorsal horn, correlating with pain, and on motor neurons in the lumbar

Spinal Cord Stimulation in Complex Regional Pain Syndrome: A Narrative Review.

Complex regional pain syndrome (CRPS) is a chronic, debilitating pain disorder that primarily affects the limbs and often arises after injury, surgery, or trauma. Spinal cord stimulation (SCS) has emerged as a promising intervention for patients with refractory CRPS, offering targeted modulation of pain pathways in the spinal cord. Evidence suggests that SCS can provide significant pain relief, improve functional outcomes, and enhance quality of life. While generally safe, the therapy carries potential risks, including surgical complications, hardware issues, and infection. Advances in neuromo

Gut-lung axis perturbation and Bifidobacterium potential after spinal cord injury in humans and mice.

Spinal cord injury (SCI) predisposes patients to severe respiratory complications. Integrating clinical observations with a mouse model, we identified gut-lung axis perturbation as a key mechanism. SCI increased the similarity between gut and lung microbial communities, consistent with cross-compartment convergence driven by intestinal dysbiosis. Fecal microbiota transplantation from SCI donors reproduced this convergence in healthy recipients. Crucially, oral Bifidobacterium supplementation restored intestinal community structure, reduced gut-lung similarity, and attenuated pulmonary inflamma
