Tourniquet-related complications in extremity injuries: a scoping review of the literature.

BACKGROUND: Tourniquets are crucial for controlling life-threatening hemorrhage and, therefore, in preventing avoidable deaths in both military and civilian settings. Its increased use since the launch of the Stop the Bleed campaign, however, has raised concerns regarding possible complications associated with its application, including limb ischemia and amputation. The objective of this study was to synthesize the existing evidence regarding complications associated with the use of tourniquets for extremity injuries and identify gaps in knowledge to guide future research on this topic. Do Gunshot and Explosion Injuries Differ in Severity and Management? A Multicenter Study of Upper Extremity Trauma in the 2023 Israel-Gaza Conflict.

BACKGROUND: In 2023, the Israel-Gaza conflict escalated with the October 7, 2023, attack that has led to prolonged warfare. Upper extremity injuries, often resulting from explosions or gunshot wounds, have been a major cause of disability in this conflict. While previous studies have examined blast and ballistic trauma separately, there is a lack of direct, large-scale comparisons of their injury patterns, severity, and clinical management in a real-world conflict setting. It remains unclear whether gunshot wounds or explosion-related injuries result in more intensive surgical interventions or

Combined Application of Negative Pressure Wound Therapy and Dermal Substitute for Coverage of a Complex Cutaneous Defect: A Case Report.

Abrasion burns are a type of skin injury commonly associated with friction trauma, such as those caused by automobile accidents. These wounds, although superficial in appearance, can involve large areas of the integument and require surgical treatment to achieve adequate functional and aesthetic healing. We present the case of a female patient who suffered an extensive abrasion burn on her upper limb following a high-energy vehicle accident. The initial approach included surgical cleansing and placement of negative pressure therapy (vacuum-assisted closure system (VAC®)) to prepare the wound b

Role of SU-Mag dressing in postoperative orthopaedic surgeries: A prospective study.

INTRODUCTION: Effective postoperative wound care is essential in orthopaedic surgeries to minimise complications such as infection, swelling, and compartment syndrome. Postoperative oedema can delay healing and restrict mobility. SU-Mag ointment, known for its hygroscopic and anti-inflammatory properties, has been used in conditions like thrombophlebitis and intravenous infiltration, but its role in orthopaedic postoperative care is not well studied. This study evaluates the effectiveness of SU-Mag dressing in reducing postoperative oedema following limb fracture fixation surgeries. METHODOLO

Transorbital Brain Injury by a Sharp Object: A Case Report and Literature Review.

Non-missile penetrating head injuries are rare in Western countries and often present significant management challenges. We describe the case of a 39-year-old man who sustained a transorbital penetrating brain injury after an assault with a sharp object. On admission, he presented with ocular trauma and impaired consciousness. Computed tomography revealed a metallic object entering through the right orbit, traversing to the left parasagittal occipital bone, with associated intraventricular and subarachnoid hemorrhage. The foreign body was surgically removed without intraoperative complications

Efficacy and safety of combined negative pressure therapy after skin grafting on limbs: A systematic review and meta-analysis.

Numerous studies have demonstrated that negative pressure wound therapy (NPWT) can enhance wound healing; however, its use following skin grafting remains a subject of debate. Given the high rate of graft failure in extremities and the critical role these areas play in human mobility, this study aimed to evaluate whether NPWT improves skin grafts take rates after extremity skin grafting. A comprehensive literature search was conducted across multiple databases, and 18 studies involved 1098 patients met the inclusion criteria and were included in the analysis. The effectiveness of NPWT versus c

Beyond function: Aesthetic considerations in pedicled abdominal flaps for upper limb reconstruction.

BACKGROUND: Pedicled abdominal flaps (PAFs) represent an established approach for reconstructing the upper extremity, offering robust coverage and minimal donor-site morbidity. Most studies focus on functional outcomes and complications; however, there is a paucity of literature examining patient and physician satisfaction regarding aesthetic results following reconstruction. OBJECTIVE: This retrospective study aimed to assess pedicled abdominal flap efficacy and safety in upper limb reconstruction, examining flap survival, complications, aesthetic outcomes, and surgeon assessment. METHODS:

Weber Clamp Dorsal Compression Technique for Distal Radius Fracture Open Reduction and Internal Fixation With Case Series.

Open reduction and internal fixation (ORIF) of distal radius fractures (DRF) is a common procedure performed by hand surgeons. Complications include, but are not limited to, failure/loss of reduction, improper positioning of hardware, and nerve or tendon injury. Although uncommon, these risks should be considered when choosing a surgical technique. In this article, we present a novel dorsal compression reduction technique (DCRT) using a Weber clamp and surgical towel for ORIF of DRFs. The DCRT maintains efficacy and efficiency and improves safety by minimizing the risks of penetrating skin or

Limb Salvage and Reconstruction of a Complex Upper Extremity Injury: A Case Report.

INTRODUCTION: Complex upper extremity trauma poses us with significant challenges in orthopedic and reconstructive surgery due to the complex anatomy and functional importance of the extremities. These injuries are often the result of high-energy mechanisms like motor vehicle collisions, which makes their management particularly demanding. Making the decision between limb salvage and amputation in such cases requires careful consideration of multiple factors such as functionality, risk of complications, and recovery. CASE REPORT: We describe the case of a 39-year-old male who had sustained an

Use of Muscle Interpolation Flap as a Reconstructive Option in Soft Tissue Coverage of Limbs: A Novel Technique.

BACKGROUND: In injuries of the hand or forearm with vascular compromise due to a single vessel supplying blood distally or difficulty in donor vessel access, a pedicled flap is preferred. Skin flaps like the groin flap are commonly used as an interpolation flap. Muscle flaps used as interpolation flaps have scarcely been reported in the literature. However, muscle has been a component of composite flaps used as interpolation flaps like the tongue flap. The authors have used pedicled, interpolated muscle flaps successfully in hand and heel reconstructions. MATERIALS AND METHODS: Five patients
