Social Determinants of Health in Patients Undergoing Open Reduction and Internal Fixation for Distal Radius Fractures.

BACKGROUND: Social determinants of health (SDOH) may influence hand surgery outcomes. The Area Deprivation Index (ADI) is a validated and weighted index comprised of 17 census-based markers of material deprivation and poverty. QUESTIONS/PURPOSE: The purpose was to determine whether patients with high ADI (greater disadvantage) undergoing open reduction and internal fixation (ORIF) for distal radius fractures (DRF) were associated with differences in: (1) medical complications, (2) emergency department (ED) utilizations, (3) readmissions, and (4) costs. MATERIALS AND METHODS: Patients who und

Cervical spine fracture dislocation with mild neurological deficits.

Cervical fracture dislocation often leads to neurological deficits, manifesting with sensory and motor symptoms, which may persist even after surgical intervention. We presented two cases with mild neurological deficits following such injuries. In Case 1, the patient presented with left-hand numbness 1 month after a car accident. Despite this, he exhibited full muscle power in all four limbs. Cervical spine MRI revealed a posterior column tear, facet fracture dislocation and spinal cord indentation at the C5/C6 level. The patient underwent open reduction with anterior and posterior fixation. A

Does the screw trajectory affect surgical outcomes in percutaneous fixation for subacute scaphoid fracture?

BACKGROUND: Scaphoid fractures are the most common fracture in carpal bone fracture. However, they are often overlooked in the acute stage, leading to delayed diagnosis. There is limited research on the treatment outcomes of subacute scaphoid fractures. While many biomechanical studies have investigated the optimal trajectory for fixing scaphoid fractures, their clinical relevance remains unclear. This study aims to explore the clinical outcomes of volar percutaneous fixation for subacute scaphoid fractures and compare potential outcomes between groups with perpendicular and non-perpendicular

Can we avoid casting for suspected scaphoid fractures? A multicenter randomized controlled trial.

BACKGROUND: In suspected scaphoid fractures with normal initial radiographs, the usual care is casting, but only 10% of patients have scaphoid fractures. To reduce overtreatment, we evaluated whether bandaging, instead of casting, resulted in noninferior functional outcomes. PATIENTS AND METHODS: We included adults with suspected scaphoid fractures and normal initial radiographs at the emergency department in our multicenter randomized controlled trial. Patients were randomized to 3-day bandaging or 2-week casting. Questionnaires, physical examination, and radiographs were performed at 2 week

Site-specific volumetric skeletal changes in women with and without a distal forearm fracture: a case-control study with a mean 7-year follow-up.

UNLABELLED: Brief rationale: To assess bone dimensions in the radius over 7 years. MAIN RESULT: Cross-sectional area did not change significantly, but endosteal circumference increased, leading to decreased cortical thickness. Significance of the paper: Bone mineral density loss is associated with a decrease in cortical thickness in the forearm. PURPOSE: To assess site-specific volumetric bone and muscle differences in women with and without forearm fracture in a longitudinal study. METHODS: One hundred four postmenopausal women with a forearm fracture and 99 age-matched controls were inclu

Arthroscopic versus open surgery in perilunate injuries: a retrospective case-control study.

This retrospective case-control study compares arthroscopic surgery (AS) with open reduction and internal fixation (ORIF) in early and late follow-up outcomes for perilunate injuries. Of 74 patients treated between 2012 and 2023, 28 were matched into two groups: 14 AS and 14 ORIF. Baseline characteristics were similar. Arthroscopic surgery had a significantly shorter operative time (mean difference (MD) 71 min, 95% confidence interval (CI) 10 to 132 min, p = 0.021). At early follow-up (mean 4.1, SD 1.3 months), AS patients had significantly higher Mayo Wrist Scores (MWS) with better pain relie

Delayed rupture of extensor indicis proprius (EIP) and extensor pollicis longus (EPL) following volar plating of distal radius fracture: A case report.

Distal radius fractures are the most common upper limb fractures, with volar plating being the preferred surgical approach, particularly for intra-articular and unstable fractures. While complications primarily involve flexor tendon irritation or rupture, extensor tendon injuries associated with volar plating are rare. This report discusses a 23-year-old male who experienced limited active range of motion (ROM) in the thumb and index finger of his right hand 2 years after volar plating for a distal radius fracture. Imaging studies, including radiography and MRI, confirmed satisfactory fracture

Distal complications of Motec® total wrist arthroplasty: failure of osseointegration versus periprosthetic fracture.

Failure of osseointegration and periprosthetic fractures are common complications of total wrist arthroplasty. A recent increase in the incidence of the latter is hypothesized to be a result of using larger implants to avoid the former. In this mixed-methods, case-control study of the Motec® arthroplasty, six cases of failure of osseointegration were compared with eight periprosthetic fractures and 14 controls without either problem. Compared with the controls, periprosthetic fractures occurred in younger patients with better bone health and a narrower medullary diameter of the middle finger m

Immobilization Time for Conservative Treatment of Distal Radial Fractures in Elderly Patients: A Randomized Controlled Trial.

BACKGROUND: The management of distal radial fractures (DRFs) in elderly patients remains controversial. Although conservative treatment with cast immobilization is widely accepted, the optimal duration for immobilization is unclear. This study aimed to compare pain control, functional outcomes, and complication rates between 4-week and 6-week immobilization periods in elderly patients treated nonoperatively for displaced DRFs. METHODS: A single-center randomized controlled trial was conducted, including 150 patients who were ≥65 years of age and had displaced DRFs. Patients were randomized in

The Extended Flexor Carpi Radialis Approach for Distal Radius Fracture Fixation: A Prospective Study.

PURPOSE: The extended flexor carpi radialis (EFCR) approach for distal radius fractures and malunions was first described in 2001 by Orbay et al. This approach ensures optimal radial and dorsal exposure by releasing the radial septum and simplifies reduction by releasing deforming forces while providing access to the critical volar ulnar corner. We hypothesize the EFCR approach is safe and effective for routine use in the management of acute/subacute distal radius fractures without increased complication rates. METHODS: In total, 100 patients who underwent open reduction and internal fixation

Epidemiology, risk factor, and economic analysis of peripheral nerve injury following distal radius fractures.

PURPOSE: Distal radius fractures (DRFs) are a common orthopaedic injury with a bimodal age and gender distribution, comprising male children after high-energy trauma and older women after low-energy falls. Complications like malunion, ligament ruptures, and nerve injuries can significantly impact recovery. This study aimed to address the gap in population-based data by analyzing the prevalence and risk factors for peripheral nerve injuries associated with DRFs. METHODS: The National Inpatient Sample database was used to analyze DRFs patients with associated ulnar, median, or radial nerve inju
