Use of extracorporeal circulation in multidisciplinary approach and replantation strategies for traumatic limb amputations: A case report.

BackgroundTraumatic major limb amputations require urgent intervention to prevent irreversible tissue necrosis and life-threatening systemic complications due to warm ischemia. We aim to demonstrate the efficacy of extracorporeal circulation (ECC) in extending the ischemia tolerance window and stabilizing metabolic parameters during multidisciplinary replantation.Case PresentationA 23-year-old female presented with bilateral total upper extremity amputations following a high-energy motor vehicle accident. While damage control surgery addressed intra-abdominal injuries, the amputated limbs were

Major limb replantation: Current status and perspectives.

BACKGROUND: Major limb replantation is a demanding microsurgical procedure aiming to restore form and function after traumatic amputation. Although survival has improved since the 1960s, ischemia, infection, and complex reconstruction continue to limit outcomes. Emerging preservation and reconstructive strategies may help expand indications and improve results. METHODS: For this scoping review, MEDLINE, Embase, and the Cochrane Library were searched from inception through December 2024 using predefined MeSH terms related to major limb replantation. Adult case series and reports were included;

Results of innervated digital artery perforator flap and direct-flow homodigital flap application in fingertip soft tissue amputations.

OBJECTIVE: The purpose of this study is to compare the results of the innervated digital artery perforator (IDAP) flap and the direct-flow homodigital flap as reconstruction methods for fingertip soft tissue amputations. This issue is important in hand surgery, and we aim to identify the method that provides the best functional and cosmetic outcomes. METHODS: Between 2020 and 2022, 32 patients with fingertip amputations were reconstructed by the same surgeon using two different methods. The patients were retrospectively divided into two groups: those who underwent IDAP (n = 14) and those who

Upper Extremity Wood Saw Injuries: Experience at a Single Level 1 Trauma Center.

BACKGROUND: Saw injuries are common, with more 75 000 occurring each year in the United States alone. While these injuries occur frequently, management strategies are not universally agreed upon, and data regarding outcomes and complications are lacking. We propose to provide a comprehensive picture of upper extremity saw injury patterns, management strategies, complications, and outcomes. METHODS: Patients presenting to a single level 1 trauma center between 2012 and2019 with upper extremity laceration, crush, or amputation were identified. In all, 10 721 patients were reviewed, and those wi

Botulinum Toxin A Salvage of Ischemic Hand Trauma.

Posttraumatic hand injuries from crush injury, infusion, or iatrogenic vascular cannulation can cause ischemic finger damage that can progress to necrosis and digital amputation. Botulinum toxin type A (Botox) improves blood flow in chronic vasospastic disorders of the hand. Botox's efficacy in salvaging ischemic loss in digits in acute traumatic and iatrogenic injury has not been previously reported. From February of 2015 to December of 2016, 11 patients at a Level I trauma center (West Virginia University) presented to the hand surgery service with early ischemic injury and vascular compromi

Thumb reconstruction with combination of the wrap-around flap prefabricated by medialis pedis perforator flap with nail bed and phalanx banked from the amputated thumb: A case report.

Thumb reconstruction has been a very challenging issue for hand surgeons. In this report, we present a case of thumb reconstruction with combination of the wrap-around flap prefabricated by the medialis pedis perforator flap with phalanx and nail bed banked from the amputated thumb. A 22-year-old man suffered from the left thumb amputation as well as large soft tissue defect of hand and comminuted fracture in wrist due to a crush accident. The distal phalanx and nail bed of left thumb were exposed and no suitable vessels for microsurgical anastomosis could be found, resulting in the great diff

Secondary subdermal pocket procedure for venous insufficiency after digital replantation/revascularization.

PURPOSE: Venous insufficiency after microsurgical replantation/revascularization remained to be a vexing problem for finger failure, despite the level of amputation. Other than redo the venous anastomosis, alternative methods such as subdermal pocket procedure (SDP) can be used. The aim of this study was to incorporate SDP immediately at the time of replantation, when no suitable vein is available after digital artery anastomosis; or, as a secondary salvage procedure to restore venous drainage for venous insufficiency after initial digital replantation/revascularization.
