Management of post-surgical cervical pseudomeningocele causing spinal cord compression: A case report.

Major neurological deficits secondary to compressive cervical pseudomeningocele are rarely encountered and only a limited number of cases have been documented in the literature. This report presents the case of a 64-year-old man who had progressive upper and lower limb weakness for the last six months. Magnetic Resonance Imaging (MRI) of the cervical spine showed stenosis from C3 to C6 level. Laminectomy of vertebral level C3-C6 with lateral mass fixation was done. On the third post-operative day, the patient developed quadriplegia along with swelling at the operative site. MRI revealed collec

Glucagon-like peptide-1 receptor agonist use for type 2 diabetes and risk of upper extremity fragility fracture.

PURPOSE: Older adults with type 2 diabetes mellitus (T2DM) face an elevated risk of fragility fractures. Glucagon-like peptide-1 receptor agonists (GLP-1RAs) have grown in use for the management of T2DM over recent years, however, their potential skeletal effects remain incompletely understood. This study evaluated the association between sustained GLP-1RA use and the risk of upper extremity fragility fractures in this population. METHODS: A retrospective cohort study was conducted using TriNetX, a global electronic health record platform. Adults of age ≥ 50 years with T2DM and active metform

The management of pink pulseless hands in paediatric supracondylar humerus fractures: A systematic review and meta-analysis.

INTRODUCTION: The management of the pink pulseless hand (PPH) following paediatric supracondylar humerus fractures remains controversial, with no clear consensus between orthopaedic management and early neurovascular surgical exploration. This study aimed to compare outcomes of these strategies through systematic review and meta-analysis. METHODS: A systematic review was conducted in accordance with PRISMA guidelines. Multiple electronic databases were searched up to December 2024. Retrospective cohort studies evaluating management of PPH in paediatric supracondylar fractures were included. P

Firearm-Related Upper-Limb Injuries in Children: An 8-Year Single Institution Analysis.

PURPOSE: Pediatric firearm-related injuries are rising, reflecting an ongoing public health concern and burden on the health care system. However, studies examining regional gunshot wound (GSW) trends among children are scarce. We evaluated upper-limb injuries from GSWs at our state's sole level-1 trauma center over 8 years. We hypothesize there would be an increase in upper limb ballistic injuries in our pediatric population and that understanding these trends may lead to improved supportive care and interventions aimed at mitigating future occurrences. METHODS: A retrospective chart review

Humerus Shaft-Antegrade Intramedullary Nailing.

This review describes intramedullary nailing of humeral shaft fractures through antegrade technique. The case involved a high-energy injury to the humerus in a patient with relevant comorbidities, and discusses the rationale for both surgery in general and nailing in specific. Indications for surgery, setup considerations unique to the lateral decubitus position, and intraoperative methods for avoiding complications are all discussed. A brief review of current relevant literature is included.

Composite inflammatory biomarkers for predicting vascular crisis after finger replantation: a two-center retrospective study identifying a promising inflammatory biomarker and its multidimensional evaluation.

BACKGROUND: Vascular crisis is a leading cause of replantation failure following finger replantation, and preoperative identification of high-risk patients is essential for improving clinical outcomes. Composite inflammatory biomarkers derived from routine blood tests have demonstrated predictive value across multiple surgical disciplines; however, their role in predicting vascular crisis after finger replantation has not been systematically evaluated. METHODS: This retrospective study enrolled 1, 744 patients who underwent finger replantation, classified into a vascular crisis group (n = 102

LMWH prophylaxis after digital replantation and early vascular compromise.

BACKGROUND: Early vascular compromise remains a major cause of failure after digital replantation, and the optimal low-molecular-weight heparin (LMWH) regimen remains unclear. This study compared intensified versus standard LMWH prophylaxis for preventing early vascular compromise after digital replantation. MATERIALS AND METHODS: In this single-center retrospective cohort study, consecutive adult patients undergoing digital replantation (2023-2025) were included. All procedures were performed by one microsurgeon. Postoperative LMWH was administered subcutaneously for 7 days as either 5000 in

Improving hand function in chronic incomplete tetraplegia by high-PAS intervention with alternative equipment: a case report.

INTRODUCTION: The reproducibility of a paired associative stimulation (high-PAS) protocol, shown to be beneficial for rehabilitation in incomplete spinal cord injury (SCI), was investigated in a different technical environment. Whereas previous studies relied on a specific technical setup for high-PAS, here the protocol was replicated using alternative devices. Differences in technical attributes could influence the replicability of specific output. As the necessary technology is available in many clinical and research settings, validating the protocol across device systems is important for br

Impact of Microscope Versus Loupes on Sensation Disturbances After Digital Nerve Repair: A Propensity Score-Matched Analysis.

BACKGROUND: Digital nerve repair is a procedure performed to restore sensation in fingers. While surgical microscopes are used to achieve optimal outcomes, their cost may limit accessibility. Loupes, in contrast, offer a more affordable alternative, particularly in resource-limited settings. This study compares the risk of developing skin sensation disturbances in patients who underwent digital nerve repair using microscopes versus loupes. METHODS: The TriNetX database was queried to identify patients who underwent digital nerve repair. Patients were classified into two cohorts: microscope an

Upper Limb Peripheral Nerve Injuries Associated With Subdermal Contraceptive Implants: A Case Series, Systematic Review, and Proposed Evidence-based Treatment Algorithm.

BACKGROUND: Subdermal contraceptive implants (SCIs) are a widely used form of long-acting reversible contraception (LARC) with high efficacy (Pearl Index 0.00, 95% CI: 0.00-0.14). However, upper limb neurovascular injuries related to SCI insertion and removal are increasingly reported. The true incidence of these complications cannot be reliably estimated from the current literature, which is largely limited to case reports and small case series, although sporadic cases are being described with increasing frequency. These injuries may result in significant morbidity, including chronic neuropat

Spinal Cord Stimulation and Improved Hand Function in a Traumatic Upper Limb Replantation: A Case Report.

BACKGROUND: Spinal cord stimulation (SCS) is commonly used to treat chronic pain that is refractory to conservative management. SCS has demonstrated efficacy in reducing paresthesia and pain, but functional motor improvements after this type of intervention are reported less often. CASE REPORT: A 70-year-old man sustained a traumatic left forearm amputation in 2015, which was followed by surgical reattachment and persistent neuropathic pain. The patient tried several interventions, including nonsteroidal antiinflammatory drugs, muscle relaxants, gabapentin, pregabalin, opioids, duloxetine, an

D-Dimer Variability Predicts Venous Thromboembolism in Postoperative Fracture Patients: A Single-Center Retrospective Cohort Study.

BackgroundVenous thromboembolism (VTE) is a potentially life-threatening complication following traumatic fractures. Although plasma D-dimer is widely used as a biomarker of coagulation and fibrinolysis, its utility as a standalone predictor of postoperative VTE remains limited due to poor specificity. This study aimed to investigate whether D-dimer variability is a more accurate predictor of VTE compared to D-dimer alone.MethodsDemographic data, fracture classification, and D-dimer levels were collected. D-dimer variability was defined as the ratio of postoperative D-dimer to preoperative D-d

Early prophylactic antibiotics effect on infection rate in blunt open fractures.

BACKGROUND: Open fractures represent a commonly encountered traumatic injury with complex challenges, the most notorious being infection. Antibiotic prophylaxis demonstrates a critical first line of defense; however, there exists some disagreement in the literature related to the efficacy of the timing of antibiotic administration in the setting of open fracture. HYPOTHESIS: This study aimed to determine if antibiotic prophylaxis administered within one hour exhibited an effect on the rate of deep infection across all open long bone fractures, as well as to investigate the role of antibiotic
