Efficacy of topical vancomycin in preventing surgical site infection following fracture surgery : a systematic review and meta-analysis.

AIMS: Fracture-related infection (FRI) is a major cause of morbidity following limb fracture surgery. Topical vancomycin has gained popularity as an adjunct to systemic prophylaxis; however, concerns regarding nephrotoxicity, impaired bone healing, and wound complications remain. This review aims to evaluate the efficacy and safety of intraoperative topical vancomycin powder in preventing FRI in patients undergoing surgical management of upper or lower limb fractures. METHODS: A systematic review of randomized controlled trials (RCTs) was conducted in February 2026. Comprehensive searches of

Submuscular Plating, Titanium Elastic Nailing, and External Fixation for Pediatric Femoral Shaft Fractures: A Comparative Systematic Review and Meta-Analysis.

Pediatric femoral shaft fractures are commonly treated with titanium elastic nailing (TENS), submuscular plating (SMP), or external fixation (EF), but the optimal fixation strategy remains controversial. This systematic review and meta-analysis compared perioperative metrics, union, complications, and functional outcomes between SMP, TENS, and EF in skeletally immature patients. A comprehensive search of PubMed, Scopus, Web of Science, Embase, and Google Scholar (up to November 2025) identified comparative studies of diaphyseal pediatric femoral shaft fractures treated with at least two of the

Expert Tibia Nail: Is a Single Nail Enough to Fix Distal Tibia and Fibula Fractures?

Background  Management of extra-articular distal tibia fractures associated with supra-syndesmotic fibula fractures remains controversial, particularly regarding the need for fibular fixation. With the advent of the Expert Tibial Nail System (ETNS), which provides multidirectional distal locking, adequate stability may be achieved without fibular fixation, potentially reducing surgical morbidity. Objective  To compare clinical, radiological, intra-operative parameters, and complication rates between isolated ETNS fixation and conventional tibial intramedullary (IM) nailing with fibular plating

A unique case of compartment syndrome of the lower leg following hip arthroscopy: a case report.

Compartment syndrome is a serious condition typically arising from trauma, fracture, or vascular injuries but it can also occur following surgery. Recognized by symptoms such as severe pain, swelling, and neurological deficits, prompt diagnosis and intervention are crucial to prevent long-term complications. Here, we present a rare occurrence of compartment syndrome in the leg following hip arthroscopy. A healthy 25-year-old male underwent revision hip arthroscopy for cam resection and acetabular labral reconstruction. Postoperatively, the patient had progressively worsening pain and swelling

Do antibiotic bead pouches prevent infections and other complications in patients with Gustilo-Anderson Type III open lower extremity fractures?

INTRODUCTION: Antibiotic-laden beads provide high, local concentrations of antibiotics and are used to prevent infections in open fractures. This study aimed to determine if wound management with antibiotic beads was associated with fewer surgical site infections (SSI) and unplanned fracture-related operations in patients with severe lower extremity open fractures. MATERIALS AND METHODS: This cohort study included patients enrolled in the Aqueous-PREP and PREPARE Open trials with a single Gustilo-Anderson (GA) type III open fracture of the lower extremity. Our primary outcome was SSI within 9

Negative Pressure Wound Therapy as a Bridge to Definitive Reconstruction in Complex Extremity Trauma: A Systematic Review and Meta-Analysis.

Complex extremity injuries, including open fractures, particularly Gustilo-Anderson type IIIB and IIIC injuries, continue to be a major challenge for all types of institutions and levels of healthcare, a surgical challenge even in specialized trauma centers or hospitals with trained staff characterized by extensive soft tissue defects, bone exposure, and high rates of infection, flap failure, and secondary amputation. Finding the best strategy to reduce morbidity and complications of these injuries remains a challenge for plastic surgery. Negative pressure wound therapy (NPWT) has played an im

D-Dimer Variability Predicts Venous Thromboembolism in Postoperative Fracture Patients: A Single-Center Retrospective Cohort Study.

BackgroundVenous thromboembolism (VTE) is a potentially life-threatening complication following traumatic fractures. Although plasma D-dimer is widely used as a biomarker of coagulation and fibrinolysis, its utility as a standalone predictor of postoperative VTE remains limited due to poor specificity. This study aimed to investigate whether D-dimer variability is a more accurate predictor of VTE compared to D-dimer alone.MethodsDemographic data, fracture classification, and D-dimer levels were collected. D-dimer variability was defined as the ratio of postoperative D-dimer to preoperative D-d

When Coverage Isn't Enough: The Sociodemographic Impact on Weight-Bearing Progression After Flap Reconstruction for Lower Extremity Open Fractures.

BACKGROUND: Lower extremity (LE) open fractures are difficult wounds to manage, oftentimes requiring flap reconstruction. These procedures involve complex recoveries that impact daily function and quality of life, with early weight-bearing progression as a key milestone in recovery. Few studies have examined how factors influence weight-bearing progression in patients undergoing flap reconstruction after traumatic LE open fractures. METHODS: All patients > 18 years old between 2012 and 2024 who presented with a Gustilo III or higher LE open fracture and underwent flap reconstruction at our le

Clinical and radiographic outcomes of chronic anterior dislocations treated with reverse total shoulder arthroplasty.

BACKGROUND: Reverse total shoulder arthroplasty (rTSA) for chronic locked anterior shoulder dislocations (CD) remains challenging due to glenoid bone loss, soft-tissue contractures, and risk of post-operative stiffness and instability. Outcomes of rTSA for CD remain poorly defined, particularly compared with rTSA for proximal humerus fractures (PHF), another high-risk indication associated with inferior results relative to traditional rTSA indications. This study evaluated clinical and radiographic outcomes of rTSA for CD vs. PHF. We hypothesized that patients with CD would demonstrate inferio

Comparative study of the sural flap in 78 patients with a minimum follow-up of 1 year: does pedicle length matter?

PURPOSE: The sural flap is widely recognized as an effective method for soft tissue coverage in severe lower limb trauma or sequelae. This retrospective cohort study investigates the impact of vascular pedicle length in reverse sural fasciocutaneous flaps on vascular complications, specifically whether longer pedicles are associated with increased complications. METHODS: We included patients with distal third leg and foot soft tissue defects, treated between March 2008 and March 2021, with a minimum follow-up of 1 year. The cohort was divided into two groups based on the location of the flap:

Lower Extremity Reconstruction for Open Fractures with Associated Vascular Injuries: Updates in the Management of Gustilo IIIC Injuries.

Gustilo IIIC fractures represent the most severe spectrum of lower extremity trauma, characterized by high-energy skeletal injury, vascular disruption, and soft-tissue loss. Contemporary management demands early orthoplastic collaboration, rapid revascularization, and thoughtful soft-tissue reconstruction to optimize limb salvage. This review synthesizes current evidence and evolving strategies, including timing of coverage, flap selection, and adjunctive microsurgical techniques including flow through flaps, to guide decision-making in these complex injuries. Emphasis is placed on functional
