Experimental wounding models of different bullet types and diameters on extremities.

BACKGROUND: Gunshot wounds are the second leading cause of life-threatening injuries and frequently affect the extremities, accounting for 63% of combat-related cases over the past 50 years. Although extremity injuries have lower mortality rates, they still require urgent treatment to reduce complications. Wound ballistics studies often use tissue surrogates like ballistic wax and gelatin, which have limitations due to their dissimilarity to living tissues. There is insufficient data on the effects of gunshots on extremities composed of bone and muscle, which differ in resistance and elasticit

Unicortical Versus Bicortical Screw Fixation for Medial Malleolus Fractures: A Retrospective Comparative Study.

Ankle fractures are common lower extremity injuries often involving the medial malleolus, a critical stabilizer of the ankle mortise. Screw fixation is the standard treatment, but the choice between unicortical and bicortical screw techniques remains debated. This retrospective study evaluates clinical outcomes of unicortical versus bicortical screw fixation in medial malleolus fractures over a four-year period at a Level 1 trauma center. In this study, 137 adult patients were included, with 95 undergoing unicortical fixation and 42 bicortical fixation. The primary outcome was early radiologic

Wound Irrigation Prior to Closure During Routine Upper-Extremity Surgery: Is There a Difference in Wound Complications?

BACKGROUND: Presently, there is no consensus within the field of orthopedics on whether irrigation prior to wound closure in routine upper-extremity surgery reduces wound complications. Therefore, preclosure wound irrigation could provide time and cost savings. The aim of this study was to evaluate the effectiveness of wound irrigation in routine upper-extremity procedures. METHODS: We conducted a retrospective review of adult patients undergoing routine upper-extremity surgery at a single institution from 2013 to 2022. Patients were included if they underwent soft tissue upper extremity surg

The essential role of electron microscopy in the diagnosis of collagen type III glomerulopathy superimposed with membranous nephropathy or diabetic nephropathy.

BACKGROUND: Collagen type III glomerulopathy (CG) is a rare idiopathic nephropathy characterized by the massive deposition of dense, fractured, curved, band-like fibers in the mesangial and subendothelial regions. Ultrastructural pathological examination confirms that these deposits are composed of type III collagen. Here, we report two rare cases of CG: Patient I was superimposed with membranous nephropathy (MN), and Patient II was superimposed with diabetic nephropathy (DN), both confirmed by transmission electron microscopy (EM). CASE PRESENTATION: Both patients presented with bilateral lo

"Terrible Triad" Injury in an Adolescent Patient With a High-Energy Femoral Shaft Fracture: A Case Report.

Although femoral shaft fractures are typically treated with intramedullary nailing, alternative fixation methods like plating are required in select cases, particularly when nailing may be contraindicated due to respiratory complications or other patient-specific factors. Currently, there are no established guidelines for the optimal surgical fixation method of femoral shaft fractures in polytrauma patients with respiratory dysfunction. We present the case of a young patient with a diaphyseal femur fracture who underwent multiple surgical interventions within three months due to recurrent inju

Persuasive of lateral supra malleolar flap for distal leg and foot reconstruction. A cross-sectional study.

INTRODUCTION: Distal leg and foot have unique anatomical characteristics, and multiple etiologies may cause defects in various patterns that mandate reconstruction. Local flaps, such as perforator flaps, can give good outcomes with less complication. The lateral supra malleolar flap can effectively cover the distal leg and foot defect. In this article, we highlight the utility of this flap and its outcome. MATERIAL AND METHODS: This study is a descriptive, retrospective, multi-center, hospital-based performed to assess the efficacy of lateral supra malleolar flap in the reconstruction of dist

Comparison of a New Minimally Invasive Reduction Technique Using Open Wiring in the Treatment of Subtrochanteric Femur Fractures.

OBJECTIVE: Minimally invasive reduction and intramedullary nailing (IMN) are the most common strategies used to treat subtrochanteric femur fractures (SFF). We report a new minimally invasive surgical technique for the successful reduction of SFF and maintenance of fragmental position after longitudinal traction. This study compared the radiological and clinical effects of these two reduction techniques when using IMN to treat SFF. METHODS: All 43 patients who underwent SFF at our hospital were included in this retrospective study. Twenty-two patients were treated using the conservative metho

Complex Femoral Reconstruction in an Osteoporotic Elderly Patient: From Trochanteric Femoral Nailing Fixation to Hemiarthroplasty.

Hip fractures in elderly patients with poor bone quality carry a high risk of complications such as malunion and per-implant fractures, especially in the presence of pre-existing implants. We report an 82-year-old male patient with multiple comorbidities and prior distal femoral fixation in 2011 who initially underwent trochanteric femoral nailing (TFNA) for a right intertrochanteric fracture, resulting in good recovery. One year later, he sustained a new fall, resulting in a peri-implant fracture and a right distal radius fracture. CT imaging revealed an occult malunion of the prior femoral n

Clinical and Radiographic Outcomes of Distal Radius Fractures Following Dorsal Bridge Fixation to the Second Versus Third Metacarpal.

PURPOSE: Dorsal bridge plating (DBP) can effectively treat complex distal radius fractures (DRFs); however, techniques for metacarpal fixation vary. The purpose of our study was to compare range of motion (ROM), grip strength, patient-reported outcomes, radiographic parameters, and complication rates between patients undergoing DBP with fixation to the second versus third metacarpal for DRFs. METHODS: We retrospectively analyzed 432 patients with DRFs. Wrist ROM and radiographic data were calculated at the 6-month follow-up. We calculated Patient-Reported Outcomes Measurement Information Syst

A Randomized Trial Comparing Outcome of Expert Tibial Nail versus Proximal Tibial Plate in the Management of Fractures of Proximal Tibia in Adults.

BACKGROUND: Extra-articular fracture of the proximal tibia frequently results in functional impairment of the knee and lower limb if not managed properly. Any treatment method must achieve the desired anatomical reduction and stable fixation to permit early weight-bearing and joint motion. This type of fracture can be treated using a variety of methods, from conservative casting to surgical management. It is still for debate that, which surgical procedure is best for treating extra-articular proximal tibia fractures. AIM: This study aims to compare the functional and radiological outcome of e

Study protocol for the Australasian Cerebral Palsy Musculoskeletal Health Network (AusCP MSK) prospective cohort study: early detection of musculoskeletal complications in young children with moderate to severe cerebral palsy (GMFCS III-V).

BACKGROUND: Cerebral palsy (CP) is the most common physical disability of childhood, affecting movement and posture, resulting from a neurological insult during pregnancy or the neonatal period. While the brain lesion is static, the musculoskeletal sequelae in CP are often progressive and lifelong, associated with pain and can impact the lives of children with CP, their families and the healthcare system. The Australasian Cerebral Palsy Musculoskeletal Health Network (AusCP MSK) study will conduct comprehensive, population-based surveillance of children with moderate to severe functional mobil

Does Augmenting Irradiated Autografts With Free Vascularized Fibula Graft in Patients With Bone Loss From a Malignant Tumor Achieve Union, Function, and Complication Rate Comparably to Patients Without Bone Loss and Augmentation When Reconstructing Intercalary Resections in the Lower Extremity?

BACKGROUND: Extracorporeally irradiated autografting is a recognized technique in reconstruction after intercalary resections, but it has drawbacks such as nonunion and graft fracture. Because sterilized autografts lose some of their mechanical properties due to involvement of the cortex with tumor, the curettage, and the adverse effects of irradiation or other sterilization techniques, some have proposed adding vascularized fibula to augment the autograft. Because this potentially adds morbidity, we sought to address the value of adding vascular fibular grafts to reconstruction with irradiate

External fixators in austere environments under surge capacity conditions: A systematic review.

BACKGROUND: High-energy trauma causing open fractures can take place in low-resource settings ("austere" environment) and mainly affects the lower limbs. In these environments, external fixators often provide definitive surgical treatment. This systematic review identifies those external fixators for use on lower limbs as a definitive treatment that are most clinically effective in the austere environment. METHODS: Multiple databases were searched to identify studies investigating outcomes of external fixators used in austere environments. Case reports were excluded. Hand searching and expert

A Fulcrum-Assisted Reduction Technique Using a Novel "Seesaw" Plate Mechanism for Enhanced Stability in Unstable Weber B Ankle Fractures.

The management of unstable Weber B ankle fractures presents significant challenges, particularly in patients with osteoporotic bone. Conventional fixation techniques often employ a lag screw to achieve compression at the fracture site. However, in osteoporotic bone, this approach may fail to provide adequate compression and may cause complications such as irritation to surrounding tendons or the distal tibiofibular syndesmosis, sometimes necessitating additional surgery for hardware removal. We describe a novel "seesaw" technique that uses a laterally applied, obliquely oriented locking plate
