Treatment of popliteal artery injuries: a comparison of application patterns and short-term outcomes between open surgical and endovascular repair.

BACKGROUND: After femoral artery injury, popliteal artery injury (PAI) is the second most frequent form of arterial injury in the lower extremities. Its exceptionally high risk of limb loss has attracted a lot of attention in the field of acute lower extremity vascular damage. Nowadays, open surgical repair or endovascular surgical repair are the main methods used to treat popliteal artery damage due to developments in medical technology. The effectiveness and complication rates of these two surgical techniques are still hotly debated, though, and there is a lot of room for advancement in this

Management of post-surgical cervical pseudomeningocele causing spinal cord compression: A case report.

Major neurological deficits secondary to compressive cervical pseudomeningocele are rarely encountered and only a limited number of cases have been documented in the literature. This report presents the case of a 64-year-old man who had progressive upper and lower limb weakness for the last six months. Magnetic Resonance Imaging (MRI) of the cervical spine showed stenosis from C3 to C6 level. Laminectomy of vertebral level C3-C6 with lateral mass fixation was done. On the third post-operative day, the patient developed quadriplegia along with swelling at the operative site. MRI revealed collec

Effectiveness of indocyanine green in assessing tissue perfusion in lower limb open fractures: A prospective pilot study.

BACKGROUND: Lower limb open fractures (LLOFs) represent complex injuries in which accurate intraoperative assessment of tissue viability is crucial to prevent infection, necrosis, and delayed healing. Clinical evaluation of perfusion is subjective and may underestimate ischemic areas, especially in high-energy trauma. Indocyanine green (ICG) fluorescence angiography provides a real-time, objective visualization of tissue perfusion and may optimize surgical decision-making during debridement. MATERIALS AND METHODS: This prospective, single-center case series included patients with Gustilo-Ande

Pilot Study: Soft-tissue Healing After Augmented Closure of Traumatic Injuries With Synthetic Electrospun Fiber Matrix.

BACKGROUND: Primary closure of traumatic musculoskeletal soft-tissue wounds is often complicated by soft-tissue edema or compromised skin integrity. Tenuous primary skin closure is associated with an increased incidence of major wound complications. The aim of this retrospective case series was to evaluate soft-tissue healing outcomes following incorporation of a synthetic electrospun fiber matrix (SEFM) to provide soft-tissue reinforcement of primary surgical closure of musculoskeletal injuries. METHODS: A retrospective medical record review of patients treated at a single trauma site for mu

Carotid endarterectomy for postradiation carotid stenosis in patient with hypopharyngeal cancer: a case report.

BACKGROUND: Radiation therapy for head and neck tumors is associated with radiation-induced carotid injury, which increases the risk of carotid artery stenosis and cerebrovascular events. Despite the well-established risks, routine screening for these complications has not been incorporated into clinical practice. This case emphasizes that, despite severe fibrosis and adhesion in the neck tissues resulting from radiation therapy, a well-structured surgical evaluation can still lead to favorable outcomes. It highlights the importance of timely surgical intervention in such cases. CASE PRESENTA

D-Dimer Variability Predicts Venous Thromboembolism in Postoperative Fracture Patients: A Single-Center Retrospective Cohort Study.

BackgroundVenous thromboembolism (VTE) is a potentially life-threatening complication following traumatic fractures. Although plasma D-dimer is widely used as a biomarker of coagulation and fibrinolysis, its utility as a standalone predictor of postoperative VTE remains limited due to poor specificity. This study aimed to investigate whether D-dimer variability is a more accurate predictor of VTE compared to D-dimer alone.MethodsDemographic data, fracture classification, and D-dimer levels were collected. D-dimer variability was defined as the ratio of postoperative D-dimer to preoperative D-d

Early prophylactic antibiotics effect on infection rate in blunt open fractures.

BACKGROUND: Open fractures represent a commonly encountered traumatic injury with complex challenges, the most notorious being infection. Antibiotic prophylaxis demonstrates a critical first line of defense; however, there exists some disagreement in the literature related to the efficacy of the timing of antibiotic administration in the setting of open fracture. HYPOTHESIS: This study aimed to determine if antibiotic prophylaxis administered within one hour exhibited an effect on the rate of deep infection across all open long bone fractures, as well as to investigate the role of antibiotic

Immediate Versus Delayed Repair for Lower Extremity Arterial Injury: A Propensity Score-Matched Analysis.

INTRODUCTION: Delayed repair is often used for lower extremity arterial injuries, typically following temporary intravascular shunting. However, supporting evidence remains limited. We hypothesized that delayed repair is not associated with inferior outcomes compared to immediate repair. METHODS: Adult patients (aged ≥16 y) undergoing open lower extremity vascular repair between 2017 and 2021 were identified using the Trauma Quality Improvement Project database. Repairs performed within 4 h of arrival were classified as immediate, whereas those performed thereafter were considered delayed. Co
