Craniofacial Analysis of Lateral Cephalograms in Obstructive Sleep Apnea-An Exploratory Case-Control Study.

Background: The clinical value of lateral cephalograms for obstructive sleep apnea (OSA) risk assessment remains controversial, largely because previous case-control studies often lacked objective exclusion of OSA in control subjects and insufficiently controlled for confounding. This age-matched case-control study evaluated whether craniofacial characteristics differ between individuals with and without OSA and whether these craniofacial measurements independently predict OSA-related outcomes after adjustment for relevant confounders. Methods: A total of 54 adults were included (27 with OSA a

Optimizing Surgical Choice in Mild and Moderate OSA: Anterior Palatoplasty vs. Radiofrequency Uvulopalatoplasty.

BACKGROUND: Surgical palatal techniques are established alternatives to continuous positive airway pressure (CPAP) in selective patients with obstructive sleep apnea (OSA) with retropalatal airway collapse. Anterior palatoplasty (AP) stiffens and advances the soft palate, whereas radiofrequency-assisted uvulopalatoplasty (RF-UPP) uses thermal ablation to reduce palatal tissue. This study aimed to compare the 6-month efficacy and morbidity of AP vs. RF-UPP in treating mild-to-moderate OSA. MATERIALS AND METHODS: We conducted a single-center retrospective cohort study (March 2023-March 2025) of

Finite Element Analysis of Upper Airway in Ansa Cervicalis Stimulation for Obstructive Sleep Apnea.

OBJECTIVE: To develop a subject-specific, three-dimensional finite element (FE) model of the human upper airway and evaluate how ansa cervicalis stimulation (ACS), a novel neurostimulation for obstructive sleep apnea (OSA), alters upper airway anatomy under physiologic loading. METHODS: Upper airway anatomy was reconstructed from a head-and-neck CT of an adult female using a semi-automated pipeline, including segmentation, smoothing, tetrahedral meshing, and registration. Linear elastic material properties were assigned from the literature. ACS was simulated as a caudal load on the anterior t

ANALYSIS OF THE EFFECTIVENESS OF SURGICAL METHODS IN THE TREATMENT OF CLEFT PALATE.

BACKGROUND: Cleft palate is one of the most significant congenital craniofacial anomalies and remains an important clinical and public health concern. The incidence of cleft palate continues to increase in many regions worldwide, highlighting the need for effective surgical management and improved treatment strategies. The primary goal of palatoplasty is the anatomical restoration of palatal integrity to enable normal speech development while minimizing the risk of oronasal fistula formation and promoting long-term harmonious growth of the facial skeleton. OBJECTIVE: This study aimed to evalu

Three-dimensional upper airway phenotyping and OSA severity: a CT-based study.

BACKGROUND: Obstructive sleep apnea (OSA) involves recurrent upper airway collapse. While anatomical factors are key, the correlation between three-dimensional (3D) upper airway parameters and OSA severity remains inconsistent, limiting personalized treatment. MATERIALS AND METHODS: 69 OSA patients underwent polysomnography and computed tomography. The upper airway was segmented into nasopharynx, retropalatal oropharynx, and retroglossal oropharynx. Parameters, including soft palate length, hyoid-to-mandibular plane distance, subregion volumes, minimum cross-sectional area (MIN-CSA), and the

Palatal Emetic Receptor Density in Evolutionary Perspective: A Synthesis from Neuroanatomy, Comparative Primatology, and Biological Anthropology.

The soft palate harbors a morphologically and functionally heterogeneous population of sensory receptors whose coordinated activation constitutes one of the most ancient and conserved protective reflexes in vertebrate biology: the emetic reflex. Despite decades of investigation into the neurochemistry and pharmacology of emesis, the quantitative topography of palatal receptor populations, comprising free nerve endings (FNEs), encapsulated mechanoreceptors, and epithelial taste receptor cells (TRCs), remains incompletely characterized, and their evolutionary significance has received comparativ

Integrating ultrasound biomechanics and clinical assessments to examine breastfeeding function: novel insights into tongue mobility, milk transfer, and reflux symptoms.

BACKGROUND: Breastfeeding is a dynamic co-occupation between mother and infant, influenced by both anatomical and functional factors. Tongue mobility plays a critical role and may impact infants' feeding efficiency and quality. Breastfeeding biomechanics, evaluated using tools such as ultrasound, and clinical aspects, assessed through questionnaires and examinations, have largely been studied independently. The aim of this study was to evaluate the association between breastfeeding biomechanics and clinical measurements of breastfeeding quality. Specifically, to assess the correlations between

Changes in Upper Airway Anatomy and Apnea-Hypopnea Index in Patients with Obstructive Sleep Apnea Undergoing Bariatric Surgery: A Pilot Study.

Background: Obstructive sleep apnea (OSA) is common in individuals with obesity, largely due to increased soft tissue causing upper airway narrowing. However, mechanisms of OSA improvement following weight loss are incompletely understood, particularly in Asian population, where craniofacial and soft tissue characteristics differ. This study aimed to evaluate changes in upper airway anatomy before and six months after bariatric surgery. Methods: We prospectively evaluated Indian obese patients with OSA undergoing bariatric surgery. Magnetic resonance imaging (MRI) and polysomnography were perf

A novel combined approach: trans-nasal endoscopic and transoral robotic-assisted resection of nasopharyngeal amyloidosis.

Amyloidosis is a condition characterized by extracellular deposition of insoluble, proteinaceous material with β-pleated ultrastructure. Localized head neck amyloidosis is rare with 41 cases of nasopharyngeal amyloidosis reported. A 56-year old male, with a history of T2 N1 M0 HPV positive left tonsillar squamous cell carcinoma, under surveillance post-treatment, had an incidental finding of a nasopharyngeal amyloidosis (AL lambda), involving the right lateral wall. This was resected with combined trans-nasal and transoral robotic-assisted surgery (TORS) approach, avoiding palatal split while

Soft palate leukoplakia as an endoscopic finding associated with esophageal squamous cell carcinoma.

BACKGROUND: We previously reported that specific endoscopic soft palate findings, including a whitish epithelium, were associated with esophageal squamous cell carcinoma (ESCC) history. In the present post hoc analysis of the same cohort, we re-evaluated endoscopic images to identify lesions corresponding to leukoplakia and further characterize their endoscopic features and clinical relevance in relation to ESCC. METHODS: This single-center retrospective study included 284 patients who underwent upper gastrointestinal endoscopy at the Osaka International Cancer Institute between January and M

Case Report: A rare case of over 45 years' survival in a patient with tonsillar adenoid cystic carcinoma.

Tonsillar adenoid cystic carcinoma (TACC) is an extremely rare malignancy with an unpredictable clinical course. We report an unusual case of TACC with a 47-year disease trajectory, characterized by prolonged indolent behavior followed by late and relatively rapid clinical progression. The patient was initially diagnosed with left tonsillar ACC in 1977 after presenting with dysphagia and underwent tonsillectomy followed by postoperative radiotherapy. A local recurrence involving the left soft palate occurred in 1996-1997 and was treated with surgical resection and tongue flap reconstruction, a
