Pre-hospital time and risk of perforation in patients with acute appendicitis in 112 countries (AlliGatOr): an international, prospective, observational cohort study.

BACKGROUND: Acute appendicitis is the most common surgical emergency worldwide. Concern that delayed treatment might increase perforation can lead to unnecessary surgery and high rates of negative appendicectomy, where a normal, non-inflamed appendix is removed. However, whether perforation is related to pre-hospital time (symptom onset to first surgical assessment) has not been explored in contemporary global data. METHODS: We conducted an international, multicentre, prospective, observational cohort study (AlliGatOr) of patients undergoing emergency appendicectomy for suspected acute append

Retrospective evaluation of sirolimus (mTOR blocker) for lymphatic malformations presenting in the neonatal period: a single-center experience.

BACKGROUND: Lymphatic malformations are rare vascular anomalies. Currently, there is no uniform approach to the treatment of these patients. Neonates with extensive disease often suffer from multiple complications, organ dysfunction, and various tissue lesions, which may be life-threatening and lead to aesthetic deformity. Treatment with sirolimus has the potential to improve symptoms and downsize the lesion, up to complete regression. This retrospective single-center study presents our experience with oral sirolimus in the treatment of newborns with lymphatic malformations, evaluating its eff

Necrotizing Enterocolitis Secondary to Congenital Syphilis in an 18-Day-Old Neonate: Successful Treatment With Intravenous Penicillin.

Congenital syphilis remains a preventable yet persistent global health challenge, particularly in resource-limited countries. Gastrointestinal involvement is uncommon, and necrotizing enterocolitis (NEC) has rarely been reported as a manifestation of early congenital syphilis. We present the case of an 18-day-old neonate who was initially admitted with a diagnosis of NEC, with abdominal distension, feeding intolerance, and characteristic radiologic signs, corresponding to Grade IIb NEC according to the Modified Bell criteria. However, the presence of desquamation on the soles, along with a mar

Intraoperative Continuous Glucose Monitor Sensor-Receiver Connectivity Is Maintained With Alternative Sensor Placement: A Pilot Study.

INTRODUCTION: Perioperative glycaemic control is associated with reduced postoperative complications. However, regulatory restrictions on point-of-care device operation and operative positioning constraints, reducing accessibility for dermal puncture, may hinder intraoperative monitoring. Although continuous glucose monitors (CGM) support real-time glucose monitoring, intraoperative patient positioning may increase compression low susceptibility, and electrosurgery interference may affect sensor-receiver connectivity. Alternative CGM sensor placement may improve intraoperative sensor-receiver

Surgery under fire: war injuries and the consequences of delayed care in Ethiopia's Tigray region.

BACKGROUND: The armed conflict in Ethiopia, particularly in the Tigray region, has caused a substantial burden of war-related injuries in the context of a fragile and disrupted health system. Restricted access to timely surgical care has resulted in delayed presentation and may contribute to secondary complications, contractures, peripheral nerve dysfunction, and long-term disability. However, the consequences of delayed surgical treatment for conflict-related extremity trauma in active war settings remain insufficiently described. This study aimed to characterize conflict-related injuries in

Development and validation of an interpretable machine learning model for predicting early acute kidney injury and altered consciousness in cardiac surgery patients admitted to the intensive care unit: a retrospective study.

BACKGROUND: Acute kidney injury (AKI) and altered consciousness are common and serious complications following cardiac surgery, both associated with increased morbidity and mortality in the intensive care unit (ICU). Early prediction of these complications is essential for timely intervention. We aimed to develop and validate an interpretable machine learning model for predicting early AKI and altered consciousness risk in this population. METHODS: We retrospectively extracted data from the MIMIC-IV database. Adult patients who underwent cardiac surgery within 24 h before ICU admission were i

Comparison of postoperative outcomes of whole liver transplantation and split liver transplantation using the Comprehensive Complication Index®.

BACKGROUND: Liver transplantation (LT) is a complex procedure that results in multiple postoperative complications. Surgeons use the Clavien-Dindo classification (CDC) to evaluate the severity of complications; however, only a few studies have reported the use of the Comprehensive Complication Index® (CCI®) to evaluate complications associated with LT and split liver transplantation (SLT). This study explored the application of the CCI® to evaluate complications after whole liver transplantation (WLT) and SLT. METHODS: During this retrospective study, 244 WLT and 104 SLT recipients were enrol

Implementation and sustainability of surgical randomized trials in gynecologic oncology: Adoption beyond publication.

Surgical randomized trials have transformed gynecologic oncology by redefining the extent and route of surgery, nodal assessment, and multimodality care. However, the effect of a surgical trial does not end at publication. Unlike medical therapies, surgical interventions depend on operator expertise, team coordination, pathology support and perioperative infrastructure. Beyond controlled trial settings, these conditions may change, creating technical drift, indication drift, and inequitable adoption. Implementation should therefore be considered a distinct, structured phase of surgical researc
