Robotic Cholecystectomy Remains Safe and Effective After Regular Hours: A 3-Year Update.

BACKGROUND AND OBJECTIVES: Despite growing popularity in robotic-assisted surgery, access during evenings and weekends for nonelective operations may be restricted. A prior smaller comparison study in 2024 found no basis for safety concerns in managing acute cholecystitis in this setting. We sought to provide additional data to further evaluate whether restriction of robotic cholecystectomy during "after-hours" is justified. METHODS: We performed a retrospective analysis of 161 patients undergoing robotic cholecystectomy for acute cholecystitis according to 2018 Tokyo criteria by a single sur

Comparison of Outcomes between Supramaximal Resection and Gross Total Resection for High-Grade Gliomas: A Prospective Study from a Tertiary Care Centre.

BACKGROUND: High-grade gliomas (HGG), including glioblastomas and anaplastic gliomas, are among the most aggressive primary brain tumors with poor prognosis despite multimodal treatment. Surgical resection remains the cornerstone of management, with gross total resection (GTR) traditionally considered the standard operative goal. However, recent evidence supports supramaximal resection (SMR)-removal of the enhancing tumor along with a margin of infiltrating non-enhancing tumor-as a strategy that may improve progression-free survival and overall survival (OS) without significantly increasing ne

Role of Indocyanine Green in Laparoscopic Varicocele Ligation in a Tertiary Care Hospital.

INTRODUCTION: Varicocele is surgically managed by an open or laparoscopic approach. The complications of the laparoscopic approach, such as artery ligation, lymphatic ligation, postoperative hydrocele, and recurrence, can be reduced with adequate visualisation and identification of the spermatic vein with the help of indocyanine green (ICG). MATERIALS AND METHODS: A randomised controlled trial was conducted on 20 patients presenting with symptomatic varicocele. They were divided into two groups - group I (patients not given intraoperative ICG) and group II (patients given intraoperative ICG).

Chronological age and failure to rescue after colorectal cancer surgery: A multicentre cohort study.

BACKGROUND: Whether advancing chronological age is associated more strongly with serious postoperative events or with survival after an event remains uncertain. AIMS: To compare the association of chronological age with major postoperative events and failure to rescue after colorectal cancer surgery. METHODS: We analyzed 1,124 adults treated at five Polish centers between January 2022 and August 2025. Major events were Clavien-Dindo ≥grade IIIa; failure to rescue was cumulative 90-day death after such an event. Chronological age was analysed per 10-year increase. Multivariable logistic regre

Feasibility of primary repair in extensive gunshot-induced colon injuries with severe fecal contamination: a case series of six patients and review of literature.

BACKGROUND: The optimal management of penetrating colon injuries, particularly in the presence of high-risk features such as severe fecal contamination, hemodynamic instability, and extensive tissue damage, remains controversial. Although primary repair has gained increasing acceptance, fecal diversion is still frequently performed in complex cases due to concerns regarding anastomotic failure and septic complications. Evidence specifically addressing the feasibility of primary repair in patients with extensive destructive injuries is limited. CASE PRESENTATION: We report a case series of six

Comparative Efficacy of Pecto-Intercostal Facial Plane Block vs Transversus Thoracic Facial Plane Block Combined with Rectus Sheath Block for Post-Sternotomy Analgesia: A Prospective Randomized Controlled Trial.

BACKGROUND: Ultrasound-guided facial plane blocks are central to enhanced recovery after cardiac surgery (ERACS) protocols, yet evidence comparing pecto-intercostal facial plane block (PIFB) and transversus thoracic facial plane block (TTPB) with rectus sheath block (RSB) remains limited. METHODS: This prospective, randomized, controlled trial enrolled 100 adult patients (ASA II-III) undergoing elective median sternotomy at a tertiary care hospital in South India. Patients received standardized general anesthesia and were randomized to bilateral PIFB or TTPB (20 mL 0.25% bupivacaine + 4 mg de

Decreased Health Care Resource Utilization in Children With Planned Gastrojejunostomy Tube Exchanges.

INTRODUCTION: Gastrojejunostomy tubes (GJTs) provide enteral nutrition in infants and children with gastric feeding intolerance, but are prone to device-related complications. There are no widely accepted guidelines regarding timing of GJT exchange. We hypothesized that patients with increased scheduled GJT exchanges would have decreased overall health care resource utilization compared to patients with less planned exchanges. METHODS: A single-institution retrospective analysis of a prospectively maintained registry of children undergoing GJT exchange by a surgical service from 2018 to 2023.

Cost comparison of minimally invasive transforaminal lumbar interbody fusion: experience from Nigeria and the United States.

INTRODUCTION: The total cost of patient care for minimally invasive spine surgery is generally lower than for open spine surgeries, primarily due to shorter length of hospital stay (LOS), shorter surgery duration (SDn), lower perioperative morbidity, and reduced narcotic pain medication use. Due to prohibitive costs, neither surgical approach is readily accessible in developing countries. Objectives-to report our experience with minimally invasive transforaminal lumbar interbody fusion (MIS-TLIF) in Nigeria, a lower-middle-income country, and to compare it with data from our prior practice in

Early mobilization in surgery: a narrative historical review of its evolution and contemporary implications.

Early mobilization after surgery is widely recognized as a key component of enhanced recovery after surgery (ERAS) protocols, improving patient outcomes and reducing hospital stays. However, the historical roots of this practice remain insufficiently explored. This study aimed to examine the historical evolution of early mobilization from the 19th century to the mid-20th century and to reflect on its relevance for contemporary perioperative care. This study was designed as a narrative historical review based on primary historical sources, including medical articles, editorials, and case report

Preoperative disability is independently associated with postoperative care intensity following anterior cervical discectomy and fusion.

PURPOSE: Postoperative outcomes following anterior cervical discectomy and fusion (ACDF) are traditionally assessed using complications or long-term patient-reported outcomes (PROMs). However, determinants of immediate postoperative recovery and in-hospital resource utilization remain less well defined. This is particularly relevant with the increasing adoption of outpatient ACDF pathways, where accurate preoperative risk stratification is critical. The objective of this study was to evaluate whether preoperative PROMs are associated with postoperative care intensity following ACDF, in additio

Perioperative Anesthetic Considerations for Temporal Bone Tumor Resection in Fibroblast Growth Factor 23 (FGF23)-Related Tumor-Induced Osteomalacia: A Case Report.

Phosphaturic mesenchymal tumors (PMTs) are uncommon, predominantly benign neoplasms frequently associated with dysregulated phosphate metabolism through the overproduction of fibroblast growth factor 23 (FGF23). Elevated levels of FGF23 lead to phosphate depletion, which may impair tissue oxygen delivery, central respiratory drive, and respiratory muscle function. Systemic manifestations may include osteomalacia and neuromuscular dysfunction, with an increased risk of respiratory and cardiovascular complications. PMTs often present with nonspecific clinical findings and may remain unrecognized

Tirofiban after intravenous thrombolysis in acute ischemic stroke patients: An updated systematic review and meta-analysis of randomized controlled trials.

Early antiplatelet therapy after intravenous thrombolysis (IVT) for acute ischemic stroke remains controversial because of concerns regarding hemorrhagic complications. Tirofiban, a reversible glycoprotein IIb/IIIa receptor antagonist, may reduce platelet-mediated rethrombosis and improve reperfusion after thrombolysis. We conducted a systematic review and meta-analysis of randomized controlled trials (RCTs) evaluating the efficacy and safety of early intravenous tirofiban following IVT. PubMed, Scopus, Web of Science, and the Cochrane Library were searched from inception through May 2026. RCT

Understanding Anti-Diuretic Hormone Dynamics in Off pump Coronary Artery Bypass Grafting (OPCABG) Patients: A Case Series.

OBJECTIVES: The conundrum of Polyuria witnessed post-cardiac surgery has been attributed to altered antidiuretic hormone (ADH) release secondary to both osmotic and non-osmotic stimulus. This transient DI-like state is more often seen in on-bypass cardiac surgeries but a few off-bypass occurrences have also been reported. A lot of conflicting evidence has accumulated trying to explain this altered water balance but a broader conceptual framework is lacking, which we have attempted to provide, with an objective of designing a future prospective study. DESIGN SETTING PARTICIPANTS INTERVENTIONS:
