How to avoid iatrogenic lingual nerve injury in the retromolar area: an anatomical study of retromolar pad and lingual nerve.

PURPOSE: This study aimed to investigate the relationship between the retromolar gland and pad, and the relationship between the LN and retromolar gland/pad to establish a new landmark for avoiding LN injury. METHODS: Sixty-two lingual nerves from fresh-frozen cadavers were used for this study. The age of the specimens at the time of death ranged from 57 to 98 with a mean of 76.5 years. The mucous incision was made into the medial border of the retromolar pad and the submucosal tissue depth of the initial incision was bluntly dissected to expose the lingual nerve. When the LN was identified,

Neuroepithelial Structures of the Oral Soft Tissues Including the Juxtaoral Organ of Chievitz: A Literature Review and Audit of Diagnosed Cases.

The juxtaoral organ of Chievitz (JOOC) is a part of microanatomy composed of bland epithelial islands closely associated with small nerves and usually described within the soft tissue on the lingual aspect of the posterior mandible. Similar structures are documented in the posterior tongue. There is a risk of misinterpretation as carcinoma showing perineural invasion. An audit was undertaken of diagnosed cases of the JOOC and similar neuroepithelial structures identified within the oral soft tissues of surgical specimens. Nineteen cases were identified. Epithelial islands ranged < 0.5-4 mm in

Incidence and Anatomical Properties of Retromolar Canal in an Iranian Population: A Cone-Beam Computed Tomography Study.

OBJECTIVES: Retromolar canal (RC) is an anatomic structure, and due to increasing demand for surgical procedure in the retromolar area of the mandible, the identification of the retromolar canal has become an issue of clinical concern. It can innervate the third molar and some of the muscles around the posterior segment of the mandible, complicating surgical procedures in the retromolar area and root canal treatment of third molars. The aim of this study was to evaluate the incidence and anatomical properties of RC in a western Iranian population using cone-beam computed tomography (CBCT) imag

Primary retromolar malignant melanoma in a Latin American patient.

Primary malignant melanoma of the oral mucosa is rare and has scarcely been reported in patients from Latin America. The most frequently affected sites include the palate and gingiva of the maxilla. In this report, we describe primary malignant melanoma in a Latin American patient. A 3 × 3.5-cm black tumor was observed on the upper left retromolar area. The lesion exhibited a sessile base, irregular edges, soft consistency, and pain on palpation. Immunohistochemistry staining results were positive for S100 and HMB-45; the Ki-67 index was 20%. Contrast tomography of the skull, face, abdomen, an

Distinct differences in hypoxic responses between human oral mucosa and skin fibroblasts in a 3D collagen matrix.

The differences between oral mucosa and skin wound healing involving hypoxic responses of fibroblasts are poorly elucidated. In this study, we aimed to study the different hypoxic responses between oral and skin fibroblasts embedded in a three-dimensional (3D) collagen matrix to address the early stage of wound healing. Primary oral mucosa fibroblasts (OMFs) obtained from the retromolar area and skin fibroblasts (SFs) obtained from the abdomen were cultured in the 3D 'floating model' under either 21%, 5% or 1% O2 for 2 days. Cell viability under hypoxia was higher in the OMFs than in the SFs.

A rare case of trifid mandibular canal with bilateral retromolar foramina.

There are many reported anatomical variations of the mandibular canal. Consequently, there is great variation in the retromolar area, such as the quantity, size, and location of the retromolar foramen (RMF), the bony entrance of the retromolar canal (RMC). These variations allow for different accessory innervations to the mandibular molars and their adjacent buccal tissue because the RMC contains neurovascular bundles. Consideration of these anatomical variations is crucial for avoiding complications in anesthesia, implant placement, and surgery. However, the rarer canal types are often only i

Traumatic Impaction of Unusual Foreign Body in a 10-year-old Boy's Mouth: A Case Report.

UNLABELLED: Placing objects in the mouth by children get accidentally implanted in the oral cavity, ingested, or aspirated. The incidence of foreign bodies is usually higher in the zone of head and neck than other regions of the human body. Usually, foreign bodies are symptomatic and signs of inflammation pain and purulent discharge are shown. This paper describes an unusual case of traumatic foreign body (pen cover) accidently moved in the retromolar area causing recurrent facial swelling, purulent discharge, and reduced mouth opening. Occult Nodal Metastasis in Oral Cavity Cancers.

INTRODUCTION: In squamous cell carcinoma (SCC) of the oral cavity, there is always a risk of occult metastasis to neck nodes in the clinically and radiologically negative neck (N0). Therefore, elective neck dissection (END) has ever been under discussion since the beginning of their routine use for the management of neck for oral carcinomas. The purpose of the current study is to identify the percentage of occult nodal metastasis to neck levels I-V in the cases of oral carcinoma who were treated for the N0 with END. METHODS: Patients who were treated between June 2005 and May 2010 with END fr

Mandibular Retromolar Foramen and Canal - A Systematic Review and Meta-Analysis.

INTRODUCTION: The retromolar fossa is a small triangular area posterolateral to 3rd molar region in the mandible. The retromolar fossa often contains the retromolar foramen (RMF) as an anatomical variant. When the RMF is present, the foramen is connected with the mandibular canal (MC) through another canal known as the retromolar canal (RMC). RMC contains neurovascular bundle, which gives additional supply to the mandible. Although few studies have been conducted in past, a lacunae in comprehensive review is lacking. Although, these variations posed challenging situations for the practicing su

Failure rates of mini-implants inserted in the retromolar area.

OBJECTIVE: Anchorage reinforcement is an important issue in orthodontic treatment. There is a lack of evidence regarding the failure rate of mini-implants inserted in the retromolar (RM) area, therefore the purpose of this present study was to evaluate the failure rates of mini-implants inserted in the RM area and to evaluate the factors affecting their stability. MATERIAL AND METHODS: This retrospective cohort study of 102 patients (52 female, 55 male; mean age: 18.6years; SD: 5.2years) that had received 110 RM mini-implants for orthodontic treatment from 1.2.2012 to 1.6.2017 was conducted a

Buccal Nerve Dissection Via an Intraoral Approach: Correcting an Error Regarding Buccal Nerve Blockade.

PURPOSE: The buccal nerve (BN) supplies sensation to the skin over the anterior part of the buccinator, buccal mucosa, and buccal gingivae corresponding to the second and third molar teeth. Some dental anatomy books depict the main trunk of the BN coursing over the buccal shelf of the mandible, which leads to a serious misunderstanding of this anatomy. Therefore, this study aimed to show the course of the BN on the retromolar area and establish new evidence-based anatomy for appropriate BN blockade. MATERIALS AND METHODS: Twenty sides from 10 fresh-frozen Caucasian cadaveric heads were used i

Solitary plasmacytoma of mandible: A rare entity.

Plasmacytoma is a discrete, unifocal, monoclonal neoplastic proliferation of the plasma cells. It may present as one of the three distinct clinical entities: multiple myeloma (MM), solitary plasmacytoma of bone and extramedullary plasmacytoma. Solitary bone plasmacytoma accounts for 3% of all plasma cell neoplasms with approximately 50% of the cases transforming into MM. It is frequently seen in vertebrae and secondarily in long bones. Its presence in jaws is extremely rare and when it is seen, retromolar area, angle and ramus of the mandible are most common sites of occurrence. A comprehensiv

A new treatment for lingual nerve injury: an anatomical feasibility study for using a buccal nerve pedicle graft.

PURPOSE: Lingual nerve (LN) palsy is a serious complication in dentistry and repaired by direct suture or a free graft technique. To our knowledge, there has been no study using a (long) buccal nerve (BN) graft as a donor for LN repair. Therefore, we aimed to clarify the location of the BN and investigate if it is feasible to reroute the BN to the LN. METHODS: Twenty-four sides from 12 fresh-frozen Caucasian cadaveric heads were used in this study. The mean age at death was 73.9 ± 13.4 years. The LN was dissected on the floor of the oral cavity medial to the third molar tooth. Next, the mucos
