AI-Based Dose Compliance of Secondary Organs at Risk in Head and Neck Cancer Radiotherapy.

Objective: Alongside evaluating the geometric and dosimetric performance of an AI auto-segmentation algorithm on conventional (primary) organs at risk (OARs), the aim of this work was to highlight its added advantages in terms of the dosimetric evaluation of secondary OARs, which are commonly under-reported despite their influence on overall toxicity. Methods: The study included 50 head and neck cancer patients, with volumetric modulated arc radiotherapy (VMAT) plans created based on both manual contouring and auto-contouring using a commercially available auto-segmentation tool. Quantitative

Longitudinal profiling reveals immune dynamics and distinct plasma cell signatures during B-cell depletion in IgG4-related disease.

OBJECTIVES: IgG4-related disease (IgG4-RD) is a systemic fibroinflammatory disorder characterised by tissue infiltration of IgG4+ plasma cells, yet the precise roles of B cells remain unclear. This study aimed to characterise the longitudinal immune dynamics in IgG4-RD during B-cell depletion therapy to identify pathogenic cell subsets and specific biomarkers associated with relapse. METHODS: We performed a longitudinal multiomics analysis on 119 peripheral blood samples from patients with IgG4-RD undergoing rituximab treatment, spanning active, remission, and relapse phases. Integrated singl

Evaluation of parotid and submandibular salivary glands by ultrasonography in patients using antidepressants: a case control study.

BACKGROUND: Antidepressants are widely prescribed group of medications frequently associated with xerostomia. Although alterations in salivary flow among antidepressant users have been extensively investigated, there is a paucity of data regarding the structural and vascular ultrasonographic characteristics of the major salivary glands. This study aimed to evaluate the echogenicity, parenchymal structure, margin characteristics, blood supply characteristics, and dimensions of the parotid and submandibular glands in antidepressant users compared with systemically healthy controls using ultrason

Senescence-associated tertiary lymphoid structures in Sjögren's disease model nishiura mice.

We discovered a Sjögren's disease (SjD)-like state in the programmed cell death 1+ (PD-1⁺) niches of the spleen and submandibular glands of 36-week-old female Nishiura (NI) mice, characterized by the formation of tertiary lymphoid structures (TLS) by senescence-associated (SA) immune cells, production of anti-α-amylase autoantibodies, and reduced saliva secretion. Co-culturing B220⁺/PD-1⁺ cells derived from the spleens of NI mice with CD4+/PD-1+/CD153+ (SA-Tfh) cells for 48 h enhanced IgG and senescence-associated secretion phenotype production in both cell types. Conversely, transplantation o

Radiation-Induced Volume Changes in the Parotid and Submandibular Glands, Intragland Dose Distribution, and Quality of Life Scores Over a Long Follow-Up Period.

PURPOSE: Xerostomia remains a common toxicity after head and neck radiation therapy that significantly impacts patients' quality of life. Although intensity modulated radiation therapy reduces the dose to the salivary glands, many patients still experience salivary gland toxicity. METHODS AND MATERIALS: This study examined the association between radiation-induced volume changes in the parotid and submandibular glands, mean dose to the salivary glands, and intragland dose distribution, as well as its relationship to quality of life scores, over a median follow-up period of 47 months. RESULTS

Pleomorphic adenoma arising from ectopic salivary glands in the submandibular region: a case report.

Pleomorphic adenoma (PA) or mixed tumor, is the most common benign tumor of the major and minor salivary glands. However, PA arising from ectopic salivary gland tissue in the submandibular region is extremely rare with only four case reports described in the English language literature. An 80-year-old Japanese woman presented with a growing mass measuring 25 mm in diameter in the right submandibular region. Magnetic resonance imaging revealed a well-demarcated mass with low intensity on T1-weighted images and high intensity on T2-weighted images with heterogeneous signal intensity. The submand

Comment on a submandibular gland-sparing radiotherapy study: A methodological and statistical perspective.

The study by Garg et al., which employs 99mTc-pertechnetate scintigraphy to evaluate submandibular gland function following radiotherapy for head and neck cancer, provides a useful exploration of objective functional assessment and contributes to advancing the field. While acknowledging its contributions, we suggest that several methodological and analytical aspects of the study could be further explored and refined. The study faced certain challenges in maintaining the sample, and the grouping approach may have introduced potential confounding factors. There remains room to further elucidate

Microbial Fermentation Reduces Xerostomia-Related 5-Demethylnobiletin in Aurantii Fructus: A Chromatographic and Pharmacological Evaluation.

Aurantii Fructus is a widely used herb with dual-purpose food-medicine applications; however, prolonged consumption may lead to the development of xerostomia. Although traditional processing methods of Aurantii Fructus can mitigate this adverse effect, they are characterized by cumbersome procedures and significant time demands, and the key constituents and underlying mechanisms responsible for symptom alleviation remain insufficiently defined. In the present study, we propose an innovative microbial fermentation approach (FY) for processing Aurantii Fructus, which markedly streamlines the wor

Comparison of Histological Effects of LigaSure and Bipolar Cautery on the Submandibular Gland in a Rat Model.

BACKGROUND: Submandibular gland prominence is increasingly recognized as a limiting factor in achieving optimal cervicomental contour. Although energy-based devices are widely used during glandular procedures, their tissue-level effects remain insufficiently defined. OBJECTIVES: To compare the histological impact of a vessel-sealing system and conventional bipolar electrocautery on submandibular gland tissue in an experimental model. METHODS: Twenty-four male Wistar rats were randomly assigned to three groups (LigaSure, bipolar electrocautery, and cold dissection control; n=8 per group). Par

Bilateral Nonneoplastic Heterotopic Submandibular Glands Discovered Postmortem: A Case Study.

The submandibular glands are salivary glands located under the mandible that secrete saliva into the oral cavity. The occurrence of heterotopic submandibular glands is an uncommon anatomical variation that is usually found unilaterally. There are few known reports of bilateral heterotopic submandibular glands; this paper describes a rare case of bilateral heterotopic submandibular glands that were discovered incidentally during routine dissection of a cadaver for medical education purposes and subsequent histological examination. The heterotopic glands were located in the carotid triangle at t

Perturbation of the circadian rhythmic transcription contributes to radiotherapy-induced salivary gland hypofunction.

Salivary gland hypofunction is a common adverse effect in patients after radiotherapy for head and neck cancers, resulting in the dry mouth syndrome called xerostomia. Previous studies suggested that the functionality of the salivary gland is under the regulation of the circadian clock, however, the extent and scope of this regulation remains unexplored. Here, we profiled the diurnal fluctuation of gene expression in the mouse submandibular gland, and found that about 6% of the transcriptome showed rhythmic expression. We further analyzed the regulatory role of key circadian transcription fact

Exploratory comparison of immune cell compositions across organs in IgG4-related disease.

OBJECTIVES: IgG4-related disease (IgG4-RD) is a systemic fibroinflammatory disorder affecting multiple organs, but the extent to which immune microenvironments differ across tissues remains unclear. We compared immune cell compositions between submandibular gland and pancreatic lesions using transcriptome-based deconvolution analysis. METHODS: Bulk RNA sequencing data were obtained from patients with IgG4-related submandibular sialadenitis (n = 40) and autoimmune pancreatitis (n = 8). Immune cell fractions were estimated using CIBERSORTx with the LM22 immune signature matrix. Relative immune

Rapamycin Attenuates Age-Related Changes in Marmoset Submandibular Gland: A Non-Human Primate Model of Human Oral Aging.

The aging of salivary glands (SGs) leads to xerostomia, impaired oral defenses, and a compromised quality of life, yet the biology of human submandibular gland (SMG) aging remains poorly understood due to limited availability of human tissues and differences in anatomy and physiology between rodent and human SGs. The common marmoset (Callithrix jacchus) is a short-lived non-human primate (NHP) with SMG histology that closely resembles humans and has the potential to provide a translational model for investigating SG aging. In this study, we examined the age-associated structural and molecular

Submandibular gland preservation versus removal during neck dissection in oral squamous cell carcinoma: a systematic review and meta-analysis.

INTRODUCTION: Submandibular gland (SMG) resection is traditionally included in level IB neck dissection for oral squamous cell carcinoma (OSCC), although isolated SMG metastases are rare. Routine removal may contribute to xerostomia and a reduced quality of life. This systematic review and meta-analysis assessed whether SMG preservation compromises oncologic outcomes or improves functional outcomes. METHODS: This systematic review and meta-analysis followed PRISMA 2020 and Cochrane guidelines, with prospective registration in PROSPERO (CRD420251027851). The literature search included PubMed,

Synthetic CT-enabled weekly adaptive radiotherapy for nasopharyngeal carcinoma: Optimizing plan adaptation triggers through volumetric-dosimetric monitoring.

BACKGROUND AND PURPOSE: Anatomical changes during radiotherapy for nasopharyngeal carcinoma (NPC) frequently compromise target volume coverage while escalating dose delivery to organs at risk (OARs). In adaptive radiotherapy (ART), optimizing workflow efficiency to minimize clinical workload while maximizing patient benefit remains a fundamental challenge. Previous investigations inadequately tackled the critical aspects of identifying robust triggers and optimal timing for ART initiation. To bridge this gap, we quantified weekly sCT-derived volumetric and dosimetric changes on a C-arm linac w
