Evaluation of automated pupillometry for early detection of ethambutol-induced optic neuropathy.

Automated pupillometry (AP) has been studied in established optic neuropathies. This prospective, sequential AP evaluation of 62 newly diagnosed adult patients with pulmonary/extra-pulmonary tuberculosis, showed a statistically significant change in the amplitude of contraction (p=0.018), percent pupillary contraction and velocity of dilation (p=0.003). Although there was a significant decrease in retinal nerve fibre layer thickness in three quadrants (p<0.001) and an increase in the latency of pattern visual evoked potential (p<0.001), none of these changes reached the pre-defined cut-off val

Decoding CNS granulomas: cracking the imaging mystery in tuberculosis and neurocysticercosis endemic zones.

BACKGROUND: Central nervous system (CNS) granulomas present formidable diagnostic challenges in tuberculosis and neurocysticercosis-endemic regions, where empirical therapy based on imaging alone frequently results in misclassification and suboptimal outcomes. OBJECTIVE: To evaluate the diagnostic accuracy of conventional and multiparametric MRI in differentiating CNS granulomas, correlate imaging findings with histopathological diagnoses, and assess 3-6 month clinical and radiological outcomes in a prospectively enrolled cohort. METHODS: We conducted a prospective observational study (Febru

Linezolid Toxicity in South African Children With Rifampicin-resistant Tuberculosis.

There is little evidence about linezolid-attributed toxicity in children with drug-resistant tuberculosis. In a prospective cohort of South African children treated with linezolid, anemia, thrombocytopenia and peripheral neuropathy were common, sometimes severe, and often required active management. Peripheral neuropathy and grade 3/4 events typically occurred after ≥3 months of treatment.

Sp1-Mettl14 Feedback Loop Promotes Ferroptosis in Mycobacterium tuberculosis-Infected Microglia by Activating Acsl4.

Central nervous system tuberculosis (CNS-TB) represents a critical form of extrapulmonary tuberculosis, characterized by high mortality and morbidity. The infection of microglia by Mycobacterium tuberculosis (Mtb) is a crucial factor in the progression of CNS-TB. Ferroptosis plays a significant role in various neurological disorders. However, it remains unclear whether Mtb can induce ferroptosis in microglia and what mechanisms underlie this process. This study demonstrated that Mtb H37Rv infection can induce ferroptosis in microglia, which is characterized by the accumulation of ferrous ions,

Multi-Pathogen Knee Infection, Including Treponema pallidum and Brucella: A Case Report.

BACKGROUND Infectious arthritis is an infection of the joint and joint cavities, with Staphylococcus aureus being the most common causative agent. Most cases of infectious arthritis are caused by a single pathogen, with polymicrobial infections being comparatively rare. This article describes a patient with a knee infection caused by Treponema pallidum, Brucella spp., and possibly co-infected with Mycobacterium tuberculosis and Escherichia coli (E. coli). The aim of this paper is to discuss the characteristics of atypical joint diseases caused by multi-pathogenic infections and to provide a re

Adrenal and Metabolic Hormonal Axes Shape Anti-Tuberculosis Immune Responses in Human Immunodeficiency Virus-Tuberculosis Coinfection.

BACKGROUND: Tuberculosis (TB) remains a leading cause of mortality worldwide among infectious agents, and HIV increases the risk of developing into active disease. HIV-TB coinfection impairs immune responses, while chronic inflammation and infection-associated stress activate neuroendocrine pathways that deeply impact immune homeostasis. Adrenal steroids such as cortisol, dehydroepiandrosterone (DHEA) and its metabolites, along with metabolic hormones like leptin and adiponectin, have emerged as critical regulators of immune function, although their role in TB pathogenesis, particularly in co-

Inflammation-induced Neuronal Damage in CNS Tuberculosis: Molecular Mechanism and Therapeutic Targets.

BACKGROUND: Central nervous system tuberculosis (CNS-TB), most frequently manifesting as tuberculous meningitis, is associated with high mortality and significant long-term neurological morbidity. Increasing evidence suggests that disease severity and neurological damage are driven largely by dysregulated host neuroinflammatory responses rather than direct Mycobacterium tuberculosis-mediated cytotoxicity. However, the mechanistic links between glial activation, inflammatory signaling, and neuronal injury remain incompletely defined. MATERIALS AND METHODS: A comprehensive literature review was

Application of transcranial direct current stimulation in refractory seizures in tuberculous meningitis: a case report.

Tuberculous meningitis (TBM) is a devastating form of tuberculosis with high mortality and long-term neurological sequelae. Seizures, including refractory status epilepticus, are common complications and portend a poor prognosis. While neuromodulation techniques such as vagus nerve and deep brain stimulation have been used in refractory epilepsy, there are no prior reports of transcranial direct current stimulation (tDCS) in refractory seizures related to TBM. We report a 19-year-old woman with pulmonary tuberculosis who developed TBM with multiple central nervous system tuberculomas and refra

A Case of Diagnostic Delay: Autoimmune Adrenalitis Mimicking Functional Gastrointestinal Disorder.

Autoimmune adrenalitis is an under-recognized cause of primary adrenal insufficiency in young individuals. Its nonspecific presentation can often mimic gastrointestinal or psychiatric conditions, leading to delayed diagnosis and life-threatening adrenal crisis. We report the case of a 23-year-old male having known treated tuberculous meningitis history who presented with recurrent episodes of vomiting, generalized weakness, and altered behavior. He was hypotensive, drowsy, and had metabolic derangements, including hyponatremia and hyperkalemia. Laboratory investigations revealed low serum cort

An overview on central nervous system tuberculosis (CNS-TB) focusing on cognitive impairments.

One of the most serious clinical manifestations of tuberculosis (TB) is the central nervous system (CNS) presentation, which results in neurological disorders and cognitive impairments that may lead to reduced social skills. Few studies have assessed TB neuropsychological symptoms after infection. This review article investigated the incidence and spectrum of cognitive impairment related to complications in patients with CNS-TB and compiled data on the pathophysiology, diagnosis, and treatment of the disease. An extensive literature review was performed, and a total of 286 published studies we

Brucella vs. tuberculous central nervous system infection in a tertiary medical center in Lebanon: A case series.

Neurobrucellosis (NB) and Neurotuberculosis (NT) are rare central nervous system infections, but pose significant diagnostic and therapeutic challenges. If not diagnosed and treated early, they can have high mortality. Clinical presentations for both are nonspecific, mimicking diverse pathologies. Therefore, it is crucial to identify distinguishing features to ensure timely treatment. We conducted a retrospective review of cases of NB and NT admitted to the American University of Beirut Medical Center (AUBMC) between January 1982 and December 2022, identifying 20 patients with NT, and five cas

The diagnosis dilemma of coexistence of neurosarcoidosis and tuberculosis: case reports and literature review.

Sarcoidosis is a multisystem disease pathologically characterized by non-caseating epithelioid granulomas. Its neurological form, neurosarcoidosis (NS), can affect the central nervous system (CNS) and/or the peripheral nervous system (PNS), posing a significant diagnostic challenge. The diagnostic dilemma is further complicated by the significant clinicopathological overlap between sarcoidosis and tuberculosis (TB). Although rare, sarcoidosis and TB can coexist. Our case series included three NS patients: two had CNS involvement and one had PNS involvement. Notably, two patients had confirmed
