Quinidine hepatitis.

Long-term administration of quinidine was associated with persistent elevation of serum concentrations of SGOT, lactic acid dehydrogenase, and alkaline phosphatase. Liver biopsy showed active hepatitis. Discontinuance of quinidine therapy led to normalization of liver function tests. A challenge dose of quinidine caused clinical symptoms and abrupt elevation of SGOT, alkaline phosphatase, and lactic acid dehydrogenase values. We concluded that this patient had quinidine hepatotoxicity and believe that this is the first case reported with liver biopsy documentation. This report also suggests th…

The Role of Quinidine in the Pharmacological Therapy of Ventricular Arrhythmias 'Quinidine'.

Historically, quinidine was the first medicine used in the therapy of heart arrhythmias. Studies in the early 20th century identified quinidine, a diastereomer of the antimalarial quinine, as the most potent of the antiarrhythmic substances extracted from the cinchona plant. Quinidine is used by the 1920s, as an antiarrhythmic agent to maintain sinus rhythm after the conversion from atrial flutter or atrial fibrillation and to prevent recurrence of ventricular tachycardia or ventricular fibrillation. Its value in chronic prophylaxis of relapse of ventricular arrhythmia was brought under suspic…

Quinidine-induced agranulocytosis.

A case of agranulocytosis in an 86-year-old man after 8 weeks treatment with quinidine sulfate is described. Acute phase serum from the patient demonstrated antineutrophil activity by the microgranulocytotoxicity assay. Review of the literature reveals that more than one mechanism could cause this idiosyncratic immune-mediated agranulocytosis.

Digoxin -- quinidine interaction.

A patient with chronic paroxysmal atrial fibrillation, receiving maintenance doses of digoxin, was admitted for addition of quinidine therapy. With stable serum digoxin levels, the institution of oral quinidine sulfate resulted in a rise in the serum digoxin level in less than 24 hours. The serum digoxin concentration increased more than threefold before digoxin was discontinued. The rise and fall of the digoxin serum concentration appeared to correlate directly with an increase and decrease in the PR interval. With the reinstitution of digoxin at lower doses, apparently stable therapeutic lev…

Quinidine-induced agranulocytosis.

In a 75-year-old man, agranulocytosis and septicemia developed after eight weeks of quinidine sulfate therapy. An IgG antibody requiring the presence of quinidine was shown by complement-dependent leukocytotoxicity and leukoagglutination reactions. The antibody did not cross-react with quinine and was active against WBCs obtained from normal subjects and from the patient himself.

Quinidine oxidative metabolism. Identification and biosynthesis of quinidine 10,11-dihydrodiol stereoisomers.

The isocratic reversed phase high performance liquid chromatographic method proposed for quinidine metabolic studies facilitates particularly the separation of 10(R) and (S) isomers of quinidine 10,11-dihydrodiols. The finding of each of these forms following a new synthetic pathway allows us to identify and quantify them in biological fluids. These two isomers have especially been observed in rat bile and hepatocyte secretions. The metabolic inducing effect of phenobarbital on the oxidative metabolism of quinidine is verified in rat isolated hepatocytes. Simultaneous secretion of the two dihy…

Quinidine-induced hepatotoxicity revisited.

Although quinidine has been widely used since the beginning of the century, quinidine-induced hepatotoxicity has been recently reported in the literature. We describe a reversible case of quinidine-induced hepatotoxicity. A 62-y-old male with a past medical history of atrial flutter and adult onset diabetes was admitted to the hospital with a 3-d history of diarrhea, nausea, fever, chills and palpitations. Past medications included 7.5 mg glyburide daily for 4 y, 0.25 mg digoxin daily for 3 w, 324 mg quinidine gluconate 3 times daily for 2 w, and 150 mg papaverine daily for 2 y. On admission, …

[Quinidine-induced lichenoid photodermatitis].

Quinidine-induced lichenoid photodermatitis was definitely isolated by Berger and Sesody in 1982. We had an opportunity to observe 4 cases of this striking clinical condition and encountered some particularities. Our patients (2 men and 2 women) were 60, 64, 81 and 68 years old respectively. All had a previous cardiovascular history; diabetes was also present in patient No. 2, and hypertension in patient No. 4. All patients were taking other drugs. The first patient presented with a 4-year old lichenoid eruption on the hands, associated with some degree of follicular keratosis. The second and …

Quinidine-induced rheumatic syndromes.

Quinidine is a commonly used antiarrhythmic agent that is rarely associated with rheumatologic toxicity. However, quinidine-induced lupus, antinuclear antibody negative lupus-like syndrome, polymyalgia rheumatica-like illness, muscle weakness, and isolated creatine phosphokinase elevation have all been reported. We present one case of quinidine drug-induced lupus and another of a quinidine-induced polymyalgia rheumatica-like illness, and review the English literature for rheumatologic toxicity due to quinidine. Prompt recognition of quinidine associated rheumatologic toxicity is important beca…

Quinidine-induced lupus erythematosus.

A 60-year-old white man presented with a syndrome of fever, polyarthritis, pleurisy, and lymphopenia, and had a positive antinuclear antibody test (after 5 months of quinidine therapy). Quinidine-induced antinuclear autoantibodies were of the IgM and IgG classes and were directed mainly to nuclear histones, predominantly histones H1 and H2B. Antibodies to native DNA and nonhistone nuclear antigens were not seen during the patient's clinical course. Upon withdrawal of quinidine therapy, the patient had prompt improvement of clinical symptoms followed by a gradual resolution of serologic abnorma…

THE RELATIONSHIP OF DOSAGE SCHEDULE TO THE BLOOD LEVEL OF QUINIDINE, USING ALL AVAILABLE QUINIDINE PREPARATIONS.

Blood quinidine levels obtained by single and multiple dosage schedules of all available quinidine preparations were ascertained. A maintenance blood level of 4 to 7 mg./l. of quinidine was considered a desirable range to prevent recurrence of auricular fibrillation in patients previously converted to sinus rhythm. Quinidine sulfate, quinidine gluconate, dihydroquinidine gluconate and quinidine polygalacturonate were given in dosage schedules of one tablet (equivalent to 200 mg. quinidine) every eight hours for four days, then two tablets every eight hours for three days. Blood quinidine level…
