Successful treatment of Acanthamoeba meningitis with combination oral antimicrobials.

Acanthamoeba was implicated as the causative agent of chronic meningitis in three apparently immunocompetent children. Diagnosis was established by cerebrospinal fluid wet mount examination and culture. Two children improved rapidly with combination oral therapy composed of trimethoprim-sulfamethoxazole, rifampin and ketoconazole.

Field's stain--a rapid staining method for Acanthamoeba spp.

Acanthamoeba sp. is a free-living amoeba known to cause chronic central nervous system infection or eye infection in humans. Many cases remain undetected for want of a good detection system. We report for the first time a rapid staining method to facilitate the identification of Acanthamoeba sp. using the modified Field's staining technique. A. castellanii, which was used in the present experiment, is maintained in our laboratory in mycological peptone medium (Gibco). The cultures were pooled together and smears were made on glass slides for staining purposes. Different types of stains such as…

Acanthamoeba keratitis cases in Sweden: genotypes and clinical course.

Acanthamoeba keratitis is a sight-threatening corneal infection. A delay in diagnosis and lack of amoeba-specific treatment contribute to a generally poor prognosis. Genotyping of pathogenic Acanthamoeba may offer deeper insights into the disease. Sixteen culture-positive Acanthamoeba ocular samples from 2006 to 2022 were identified and genotyped in patients from the northern and central regions of Sweden. Data from the patients' records were collected, and their respective clinical courses were reviewed. The most common Acanthamoeba genotype was T4 (11 cases), followed by T3 (4 cases) and T6 …

Acanthamoeba keratitis: Insights into diagnostic challenges and treatment advances.

Acanthamoeba keratitis (AK) is a mysterious and dangerous disease that mimics other corneal infections. AK is sight-threatening and a cause of blindness worldwide. Correct and timely diagnosis of this disease plays a decisive role in treating the disease and achieving better visual outcomes. Early diagnosis, however, is challenging due to nonspecific features easily mistaken for other infectious keratitis cases or coinfection keratitis. Currently many methods are used in diagnosis with varied efficacy, but deep learning image analysis is now considered a promising future direction. The efforts…

Adjunctive povidone-iodine for Acanthamoeba keratitis: a randomized trial.

The aim of this study was to determine the antiamoebic efficacy of povidone-iodine when used as an adjunct to topical biguanide therapy for Acanthamoeba keratitis. Patients with smear- or culture-positive Acanthamoeba keratitis received topical chlorhexidine 0.04%, half-hourly for 2 days, then hourly for days 3-6, then eight times per day for days 7-28. Participants were randomized in a 1:1 ratio to receive either adjunctive topical povidone-iodine at the same frequency as chlorhexidine or no additional therapy (i.e., control group). Povidone-iodine was initially administered as a 2.5% solutio…

Limbal Subconjunctival Abscess: A Rare Complication of Acanthamoeba Keratitis.

PURPOSE: Acanthamoeba keratitis (AK) is a rare infection affecting the cornea. Immune-mediated sclerokeratitis is a well-documented late complication often requiring systemic immunosuppression. We present an alternative clinical presentation of subconjunctival abscesses caused by direct invasion of the Acanthamoeba and a proposed management strategy. METHODS: This study was a single case report performed at a tertiary care center in the United Kingdom. RESULTS: A 42-year-old contact lens wearer with a history of swimming in contact lenses presented with 2 days of pain, redness, and photophob…

Sodium Metabisulfite Inhibits Acanthamoeba Trophozoite Growth through Thiamine Depletion.

Acanthamoeba keratitis (AK) is a severe infection of the cornea. Prevention and treatment are difficult due to the inefficacy of currently available compounds. The impact of many commonly used compounds for routine examinations of Acanthamoeba is unexplored but might offer insight useful in combatting AK. In this study, we demonstrate that sodium metabisulfite, a common preservation constituent of eye care solutions, was found to be active against Acanthamoeba trophozoites at concentrations lower than that commonly found in eye drops (IC50 0.03 mg/mL). We demonstrate that sodium metabisulfite …

Curcumin effect on Acanthamoeba triangularis encystation under nutrient starvation.

BACKGROUND: Curcumin is an active compound derived from turmeric, Curcuma longa, and is known for its benefits to human health. The amoebicidal activity of curcumin against Acanthamoeba triangularis was recently discovered. However, a physiological change of intracellular pathways related to A. triangularis encystation mechanism, including autophagy in the surviving amoeba after curcumin treatment, has never been reported. This study aims to investigate the effect of curcumin on the survival of A. triangularis under nutrient starvation and nutrient-rich condition, as well as to evaluate the A.…

Fulminant Acanthamoeba Endophthalmitis After Cataract Surgery-A Case Report.

PURPOSE: To report a case of Acanthamoeba endophthalmitis after an uneventful cataract surgery. METHODS: Description, management, and outcomes of a biopsy-proven case of Acanthamoeba endophthalmitis. RESULTS: Two days after a routine cataract surgery elsewhere, the patient presented with acute endophthalmitis diagnosed as a bacterial infection based on timing and severity. When conventional methods of management failed, the patient was referred to us. Only the presence of an atypical ring infiltrate suggested Acanthamoeba as a probable cause. Subsequent diagnostic evaluations confirmed the i…

Acanthamoeba castellanii interactions with Streptococcus pneumoniae and Streptococcus pyogenes.

Among the genus Streptococcus, S. pyogenes and S. pneumoniae are the major causes of pharyngitis, impetigo, pneumonia and meningitis in humans. Streptococcus spp. are facultative anaerobes that are nutritionally fastidious, yet survive in the environment and target the predisposed population. Antibacterial disinfectants have been partially effective only, indicating the need for novel preventative measures and to understand mechanisms of bacterial resistance. Acanthamoeba is a free-living protist that is known to harbour microbial pathogens, provide shelter, and assist in their transmission to…

Apoptosis of Acanthamoeba castellanii Trophozoites Induced by Oleic Acid.

Acanthamoeba spp. can be parasitic in certain situations and are responsible for serious human infections, including Acanthamoeba keratitis, granulomatous amoebic encephalitis, and cutaneous acanthamoebiasis. We analyzed the fatty acid composition of Acanthamoeba castellanii trophozoites and tested the inhibitory activity of the main fatty acids, oleic acid and arachidonic acid, in vitro. Oleic acid markedly inhibited the growth of A. castellanii, with trophozoite viability of 57.4% at a concentration of 200 μM. Caspase-3 staining and annexin V assays showed that apoptotic death occurred in A.…

Use of multiple immunosuppressive agents in recalcitrant ACANTHAMOEBA scleritis.

A 48-year-old woman who is a contact lens wearer presented with unilateral ACANTHAMOEBA keratitis, confirmed by PCR, which responded initially to topical polyhexamethylene biguanide (PHMB) and brolene. Three months later, despite continued treatment, she developed diffuse anterior scleritis with severe pain and marked scleral injection but without evidence of recurrence keratitis. Oral non-steroidal anti-inflammatories and oral high-dose corticosteroids were added without success. Subsequent treatment with intravenous methylprednisolone and high-dose cyclosporine led to a temporary improvement…

[Acanthamoeba keratitis--a rare and often late diagnosed disease].

In 83-93% of the cases of acanthamoeba keratitis the patients are contact lens wearers. Acanthamoeba keratitis is diagnosed--with descending order of sensitivity and specificity--through polymerase chain reaction (PCR), confocal biomicroscopy, in-vitro cultivation and histopathological examination. The typical clinical appearance of acanthamoeba keratitis includes pseudodendritic epitheliopathy, perineuritis, ring infiltrate or multifocal stromal infiltrates and in some cases limbitis with infiltration of the conjunctiva and/or sterile anterior uveitis. Information on reliability and efficacy …

Blindness due to Acanthamoeba: first case report from Sudan.

Acanthamoeba is a free-living protozoan widely distributed in soil, water, and air. It can cause corneal infection (keratitis) in man, and may lead to blindness of the affected eye in 15% of untreated cases. The main risk factors are eye trauma and contact lens wearing. Early presentation of Acanthamoeba keratitis patients usually includes ocular pain, photophobia, and a unilateral red eye. The story of a female Sudanese housewife who suffered a blind left eye due to Acanthamoeba is described in this report.
