Total joint replacement, although considered an excellent surgical procedure, can be complicated by osteolysis induced by implant particles and subsequent aseptic loosening of the implant. The pathogenesis of implant-associated osteolysis includes inflammatory and osteolytic processes. The sustained chronic inflammatory response initiated by particulate debris at the implant-bone interface is manifested by recruitment of a wide array of cell types. These cells include macrophages, fibroblasts, giant cells, neutrophils, lymphocytes, and--most importantly--osteoclasts, which are the principal bo

One of the major causes of implant loosening is due to excessive bone resorption surrounding the implant due to bone remodelling. The objective of the study is to investigate the effects of implant material and implant-bone interface conditions on bone remodelling around tibia bone due to total ankle replacement. Finite element models of intact and implanted ankles were developed using CT scan data sets. Bone remodelling algorithm was used in combination with FE analysis to predict the bone density changes around the ankle joint. Dorsiflexion, neutral, and plantar flexion positions were consid

A central dental implant success criterion is the marginal bone response as measured longitudinally. Factors that influence marginal bone changes include osseous and soft tissue architecture, occlusal loading factors, implant position, implant design, and inflammatory processes. The evolution of implant design is multifactorial and includes the implant-abutment interface geometries. The primary objective of this study was to compare the proximal marginal bone changes following placement and loading of internal conus design implants (ICI) and external hex design implants (EXI) used in the treat

INTRODUCTION: Management of the subscapularis during shoulder arthroplasty is controversial. The purpose of this study was to compare the biomechanical performance of subscapularis peel (SP) and lesser tuberosity osteotomy (LTO) in a cadaveric model.
METHODS: The subscapularis and proximal humerus were dissected from all soft tissues in 21 fresh-frozen human cadaveric shoulders and randomized to undergo SP, LTO, or standard subscapularis tenotomy (ST, control). For SP and LTO, six #5 sutures were passed through eyelets in the implant (on lateral border and through drill holes in bicipital gro

In tooth replacement therapy with implant-supported prostheses, optimal outcomes depend on a wide range of variables, including anatomic, phenotypic, surgical, and prosthetic factors. Adequate management of the restorative interface is particularly important for achieving predictable esthetic outcomes in the anterior maxilla and for maintaining long-term soft and hard tissue health and stability. This article describes an approach based on the use of intermediate multi-unit abutments on bone-level implants. A comprehensive workflow is presented, integrating digital planning, surgical protocols

Prosthetic joint infection after joint arthroplasty has serious consequences for patients. Finding new treatments against prosthesis infection is increasingly urgent due to the growing number of prosthetic joints and increasing prevalence of antimicrobial resistance. This study investigates the feasibility of non-contact induction heating to supplement debridement, antibiotics and implant retention using an implant model. Upper temperature limits for induction heating treatment are established, based on the presence or absence of biomaterials commonly associated with joint replacement implants

PURPOSE: The biological response of dental tissues to implants placed in contact with retained roots is poorly documented. The aim of this report is to describe the histological findings of three implants that inadvertently engaged inflammation-free retained root fragments during placement and were retrieved after 10 and 12 weeks of healing in the pig mandible.
MATERIALS AND METHODS: Three implants were retrieved from a preclinical pig model in which fractured root remnants were inadvertently left in situ following tooth extraction; implants were placed after a three-month healing period and

Mononuclear osteoclast precursors are present in the wear-particle-associated macrophage infiltrate found in the membrane surrounding loose implants. These cells are capable of differentiating into osteoclastic bone-resorbing cells when co-cultured with the rat osteoblast-like cell line, UMR 106, in the presence of 1,25(OH)2 vitamin D3. In order to develop an in vitro model of osteoclast differentiation which more closely parallels the cellular microenvironment at the bone-implant interface in situ, we determined whether osteoblast-like human bone-derived cells were capable of supporting the d…

We have investigated whether the presence of polyethylene (PE) alone is sufficient to cause an aggressive periprosthetic tissue response, or whether certain mechanical interface conditions can allow bone to grow while in the presence of PE. An experimental implant was loaded in the presence and absence of particulate PE under stable and unstable conditions. Bone with a thin, discontinuous fibrous membrane formed in both groups of stable implants, either in the presence or absence of PE. By contrast, a continuous fibrous membrane consistently formed in both groups of unstable implants. The memb

Insertion of bone substitution materials accelerates healing of osteoporotic fractures. Biodegradable materials are preferred for application in osteoporotic patients to avoid a second surgery for implant replacement. Degraded implant fragments are often absorbed by macrophages that are removed from the fracture side via passage through veins or lymphatic vessels. We investigated if lymphatic vessels occur in osteoporotic bone defects and whether they are regulated by the use of different materials. To address this issue osteoporosis was induced in rats using the classical method of bilateral

Glass-ionomer cements (GIC) offer several advantages over the conventional acrylic-based bone cements. The formation of an adhesive bond with bone and metals, a low setting exotherm and no systemic or local toxicity are some of the advantages cited. This study examines the in vivo biological and biomechanical behavior of two polyalkenoate cements (LG26 and LG30) implanted for 6 wk into the submetaphyseal spongiosa of the rabbit femur. Cements were implanted as both set cement rods and unset cement dough. Implantation of set rods resulted in the formation of variably mineralized osteoid/woven b

BACKGROUND: Finger amputation may result from congenital cause, trauma, infection and tumours. The finger amputation may be rehabilitated with dental implant-retained finger prosthesis. The success of implant-retained finger prosthesis is determined by the implant loading. The type of the force is a determining factor in implant loading.
OBJECTIVE: To evaluate stress distributions in finger bone when the loading force is applied along the long axis of the implant using finite element analysis.
METHOD: The finite element models were created. The finger bone model containing cortical bone and

Three spinous processes of the cervical spine and implanted porous hydroxyapatite spacers were removed from a patient with a recurring intramedullary tumor at revisional laminectomy one year after spinous process splitting laminoplasty. Histological examination of undecalcified sections revealed direct bone bonding to the spacer at three of the six bone-hydroxyapatite interfaces. Bone ingrowth was observed up to 400 microns into the implant pores. Fibrous tissue was observed to intervene between the other three bone-hydroxyapatite interfaces. The suspected bone bonding with the spacer on CT sc

The paper presents a theory of fixation failure and loosening in cemented total hip prostheses and proceeds to investigate this using an experimentally validated finite element model and two prosthesis types, namely the Charnley and the C-Stem. The study investigates the effects of retroversion torque occurring at heel-strike in combination with a loss of proximal cement/bone support and distal implant/cement support with a good distal cement/bone interface. A 3D finite element model was validated by comparison of femoral surface strains with those measured in an in vitro experimental simulati
