We present a 24-year-old lady who reported at the radiology department, UBTH Benin City for a chest radiograph, as part of her pre-employment medical examination. She was asymptomatic, her past medical history was not significant and physical examination revealed an apex beat located on the 5th right intercoastal space. The chest radiograph confirmed dextrocardia and also revealed the gastric air bubble on the right, which was suggestive of situs inversus totalis. A female newborn is reported with dextrocardia and a partial trisomy 20q, derived from a t(2;20) paternal translocation. The most discriminating findings of the condition include brachycephaly, bulging forehead, deep set eyes, short nose, large ears, dimpled chin, short neck and a heart defect. Previously reported patients with this rare chromosomal anomaly are reviewed.

A 73-year-old woman, who had a history of chronic sinusitis, presented to the emergency department with shortness of breath. Additional imaging demonstrated situs inversus totalis and multiple bronchiectases. The clinical triad of sinusitis, situs inversus totalis and bronchiectasis is often characteristic of Kartagener's syndrome, a subset of primary ciliary dyskinesia.

OBJECTIVE: To understand the pathological features of congenital dextrocardia and explore its diagnosis and surgical treatment.
METHOD: The clinical data from 32 cases of congenital dextrocardia were collected to understand the major cardiac anomalies and evaluate the diagnostic approaches of this disease. Analysis of the effect of surgical treatment was also conducted in 16 among the 32 cases.

INTRODUCTION: Isaacs syndrome is a disorder of unknown etiology characterized by muscular rigidity, cramps and myokymias. Described by Isaacs in 1961 and called by him as continuous muscular activity syndrome. There are few reports in Latino american countries.
CLINICAL CASE: A 31 year-old man with sustained muscular contractions in lower limbs and diffuse myokymias since he was eighteen-year old. Dextrocardia was disclosed on clinical examination.
CONCLUSION: We report the case of a patient with Isaacs syndrome and dextrocardia.
