Treatment of gingival recession using guided periodontal tissue regeneration.

Gingival recession is an apical displacement of the gingival margin and involves loss of connective tissue attachment and the supporting alveolar bone. Various treatment modalities have been utilized in the treatment of gingival recession in order to eliminate etiologic factors, achieve root coverage, regenerate the lost periodontal attachment, and prevent perpetuation of the disease. During the past two decades, the broad concept of guided tissue regeneration has become specific, identifying the specific treatment involved as guided bone regeneration and guided periodontal tissue regeneration…

[Treatment of gingival recession with a coronally repositioned flap].

The treatment of an exposed root surface may be indicated for an aesthetic improvement of a gingival recession. The surgical technic depends on the type of the recession, the quality and the quantity of the keratinized gingiva apically or laterally from the recession. The purpose of this study is to evaluate the minimal quantity of keratinized gingiva necessary to obtain a root coverage. 10 recessions on maxillary cuspids were treated with coronally repositioned flap. Less than 3 mm of keratinized gingiva was present apically to the exposed root surface. All patients received oral hygiene inst…

Comparison of Free Gingival Graft and Gingival Unit Graft for Treatment of Gingival Recession: A Randomized Controlled Trial.

The aim of this study was to compare the use of gingival unit graft (GUG) with free gingival graft (FGG) for treating wide gingival recession and increasing keratinized tissue. This randomized controlled trial with a split-mouth design included 30 localized bilateral recessions (Miller Classes I and II) that were randomly treated with GUG or FGG. Both grafts were fixed by cyanoacrylate glue. Probing depth, clinical attachment level, vertical recession depth, and keratinized tissue width were recorded at baseline and 1 and 6 months after surgery. The postoperative mean percentage of root covera…

Comparing polynomial regression and exponential growth model in predicting recession gain based on clinical gingival recession.

BACKGROUND: Gingival recession (GR) is a common issue causing plaque accumulation, root caries, abrasion, cervical wear, and dentin hypersensitivity. Studies show a prevalence of thick and thin gingival biotypes, with no significant association between age, gender, or recession. This study aims to use polynomial regression and exponential growth models to predict recession gain based on clinical data. GR is a common issue causing plaque accumulation, root caries, abrasion, cervical wear, and dentin hypersensitivity. Studies show a prevalence of thick and thin gingival biotypes, with no signifi…

Treatment of gingival recession in 2 surgical stages: free gingival graft plus coronally positioned flap.

Gingival recession is a common clinical condition that creates unfavorable esthetics and dentinal hypersensitivity. Treatment of gingival recession aims to create or increase keratinized tissue and provide root coverage. The aim of this article is to present 2 clinical cases of severe Miller Class II gingival recession with absence of keratinized tissue. Each patient was treated with a free gingival graft associated with a coronally positioned flap. Follow-up indicated that the use of the free gingival graft was effective in increasing keratinized tissue and in covering the recession in a sati…

Treatment of gingival recession in two surgical stages: Free gingival graft and connective tissue grafting.

This report describes a clinical case of severe Miller Class II gingival recession treated by two stages of surgery that combined a free gingival graft and connective tissue grafting. First, a free gingival graft (FGG) was performed to obtain an adequate keratinized tissue level. Three months later, a connective tissue graft (CTG) was performed to obtain root coverage. The results indicated that the FGG allows for a gain in the keratinized tissue level and the CTG allows for root coverage with decreased recession level after 16 months. Therefore, for this type of specific gingival recession, t…

[Bilaminar technic in the treatment of gingival recessions].

Bilaminar procedures represent an evolution in the treatment of marginal tissue recessions. A connective tissue graft removed from the palate and over which a pedicle flap is sutured, is used in these procedures. The advantages of these new procedures are: 1) sure predictability of graft survival to which a double-blood supply is guaranteed; 2) primary intention healing at the donor site, with minimum post-operative discomfort; 3) excellent esthetic results consisting of better color blending between graft and adjacent tissues and absence of keloid- like scars, very common after free gingival

The role of orthodontics in the repair of gingival recessions.

INTRODUCTION: The goal of this research was to assess the impact of orthodontic root movement on gingival recessions. METHODS: Twelve consecutive adult patients with a mandibular incisor presenting buccal or lingual gingival recession and with the root positioned outside the alveolar bone were enrolled. The roots were moved toward the center of the alveolar process with a goal oriented segmented appliance. The following variables were measured at baseline and after orthodontic treatment: (1) recession depth, (2) recession width, and (3) recession area. In addition, pocket probing depth, kerat…

[Gingival recessions and periodontal plastic surgery].

Periodontal plastic surgery is defined as the set of surgical procedures that are performed to prevent or correct developmental disorders and anatomical, traumatic and pathological abnormalities of the gingiva, alveolar mucosa, and alveolar bone. Root coverage procedures fall under this term and have been applied for more than fifty years with varying degrees of success. There are several indications for the treatment of gingival recessions. When the treatment of choice - a conservative approach - offers no solace (any more), gingival recessions can be treated by applying periodontal plastic s…

Gingival recession coverage using connective tissue graft and tunnel technique.

Gingival recession in the esthetic zone is a frequent clinical challenge that can be effectively managed with an autogenous palatal connective tissue graft in combination with the tunnel technique. This case report presents a 30-year-old patient with Miller Class II gingival recession involving the labial surfaces of the mandibular incisors. The primary objective was to achieve predictable root coverage and improvement in the width of attached gingiva using a minimally invasive tunneling approach. The tunnel technique offers several advantages in the management of multiple gingival recessions.…

Partly Deepithelialized Free Gingival Graft for Treatment of Lingual Recession.

INTRODUCTION: Gingival recession (GR) is an unhealthy root exposure that could result in sensitivity, abrasion, root caries and higher chance of plaque collection. The prevalence of GR is higher on the facial surfaces but could also affect the lingual tooth surfaces. Despite the etiology and location of GR, treatment is warranted to improve the long-term periodontal stability around the affected teeth. This case report describes the use of partly deepithelialized free gingival graft (PE-FGG) to augment lingual GR post orthodontic treatment. CASE PRESENTATION: The current report evaluates the …

Exploring the Gingival Recession Surgical Treatment Modalities: A Literature Review.

Gingival recessions present complex soft tissue pathology, with a multiple aetiology and a high prevalence which increases with age. They are defined as an exposure of the root surface of the teeth as a result of the apical migration of the gingival margin beyond the cementum-enamel junction, causing functional and aesthetic disturbances to the affected individuals. Aiming to ensure complete root coverage and satisfying aesthetic outcomes, a wide range of surgical techniques have been proposed through the decades for the treatment of the gingival recessions. The following literature review att…
