Hyponatraemia and the syndrome of inappropriate antidiuretic hormone secretion (SIADH) induced by psychotropic drugs.

The use of psychotropic drugs has been associated with the syndrome of inappropriate antidiuretic hormone secretion (SIADH) in a number of case reports. SIADH is characterised by the sustained release of antidiuretic hormone (ADH) from the posterior pituitary. The patients have a reduced ability to excrete diluted urine, ingested fluid is retained, and the extracellular fluid expands and becomes hypo-osmolar. The cardinal signs are hyponatraemia, serum hypoosmolality and a less than maximally diluted urine. Common symptoms include weakness, lethargy, headache, anorexia and weight gain. These s

Fluid retention and cardiomegaly associated with carbamazepine in an epileptic child.

Carbamazepine (CBZ) has been reported to have an antidiuretic action, though it is not known how it produces this effect. This is a well recognized complication of CBZ therapy in adults. However this syndrome has been rarely observed in childhood. We present an epileptic child with fluid overload due to CBZ treatment who was referred with chest pain and cardiomegaly. Our patient developed fluid retention with cardiomegaly during treatment with CBZ alone at a normal dose and for a short time. To our knowledge this is the first case of the syndrome of inappropriate antidiuretic hormone secretion

Transient syndrome of inappropriate antidiuretic hormone secretion during pregnancy.

Plasma osmolality normally decreases in early pregnancy, reaching a minimum at approximately 10 weeks and remaining depressed until term. This is associated with a mean decrease of 4 mEq/L in the plasma sodium level, and with an altered threshold for arginine vasopressin (AVP) release and for thirst. We describe a patient who developed more severe hyponatremia (120 mEq/L), which accompanied the development of hypertension and edema at 37 weeks in her fourth pregnancy. Hyponatremia and hypo-osmolality were associated with marked elevation of the plasma AVP level. The hyponatremia and elevated A

Diabetes insipidus versus syndrome of inappropriate antidiuretic hormone.

Diabetes insipidus (DI) and the syndrome of inappropriate antidiuretic hormone (SIADH) are two endocrine disorders presenting a challenge to the critical care nurse. The challenge lies in the recognition and assessment of the effects on the patient's fluid balance and in the planning of interventions to prevent complications.

[Water intoxication--a little known complication in the course of mental diseases].

Acute or chronic psychogenic polydipsia is a not uncommon finding in psychiatric patients. Water intoxication either results from the intake of large amounts of fluid within a short period of time and/or from a reduction of the renal "free water clearance". The latter can be caused iatrogenically by the administration of certain drugs or by the "Syndrome of Inadequate Secretion of ADH" (SIADH). Water intoxication results in cerebral oedema and may ultimately lead to death. A case is reported of iatrogenically-induced water intoxication in a patient with chronic psychogenic polydipsia.

Desipramine-associated SIADH in an elderly woman: case report.

The syndrome of inappropriate secretion of antidiuretic hormone (SIADH) has numerous etiologies, including medical illness, stress, and various pharmacologic agents. While a number of psychotropic agents may induce SIADH, only a few reports of tricyclic-antidepressant-induced SIADH exist. A case of SIADH associated with desipramine treatment in an elderly depressed woman is described.

Syndrome of inappropriate secretion of antidiuretic hormone in association with chlorpromazine ingestion.

A patient is described who was found to have the syndrome of inappropriate secretion of antidiuretic hormone (SIADH) in association with chlorpromazine ingestion. Electrolyte abnormalities should be considered in patients who demonstrate behavioral changes while on chlorpromazine.

Radioimmunoassay of arginine vasopressin in human plasma and urine, a resin microcolumn method.

A new method was developed for the estimation of arginine vasopressin (AVP) in plasma and urine. Samples were extracted by a microcolumn of resin and assayed radioimmunologically using a highly sensitive antiserum to AVP. Ion-exchange resin, CG-50, H+ form, packed in a small column (diameter 4 mm, height 6 mm), was proved effective to remove the interfering substances and to concentrate the AVP in the sample. The application of 80% acid acetone successive to diluted HCl brought about a consistent recovery of AVP from the resin column. Recoveries were 66.4 +/- 8.5% for plasma and 85.4 +/- 9.7%

Opioid-induced hyponatremia in a patient with central diabetes insipidus: independence from ADH.

Hyponatremia can be a complication of opioid therapy, which has been postulated to occur secondary to inappropriate antidiuretic hormone secretion (syndrome of inappropriate antidiuretic hormone secretion [SIADH]). We report severe hyponatremia following wisdom teeth extraction with opioid analgesia in a 19-year-old female with diabetes insipidus (DI) and acquired panhypopituitarism that challenges this theory. As this patient has DI, we believe opioid treatment caused severe hyponatremia by the following mechanisms: (1) Opioids have a direct antidiuretic effect independent of changes in ADH,

Chlorthalidone-induced syndrome of inappropriate secretion of antidiuretic hormone.

A 60-year-old woman who had been instructed to increase her water intake because of nephrolithiasis developed the syndrome of inappropriate secretion of antidiuretic hormone when treated with chlorthalidone for mild hypertension. Serum osmolality was 235 mOsm/kg with concomitant urine osmolality of 490 mOsm/kg. When serum sodium decreased to 110 mEq/liter, plasma antidiuretic hormone (ADH) was elevated at 30 pg/ml. The syndrome resolved when chlorthalidone was discontinued together with fluid intake restriction. Plasma ADH returned to normal (less than 0.5 pg/ml) after three days of treatment.

Presentation of severe brucellosis in 5-year-old boy - challenges and results.

Brucellosis is highly contagious zoonotic bacterial disease caused by gram-negative genus. It has a wide spectrum of clinical manifestations and due to variety and nonspecificity of clinical signs the diagnostics can be very complicated. We present a clinical case of severe chronic brucellosis in a 5-years old boy with long-term course of disease and multiorgan involvement. A different complication of brucellosis including severe syndrome of inappropriate ADH secretion (SIADH) are discussed. Despite severe course of disease patient achieved significant clinical improvement due to multidiscipli

Acid base homeostasis and serum bicarbonate concentration in syndrome of inappropriate anti-diuretic hormone secretion (SIADH) with hyponatremia.

The Syndrome of Inappropriate ADH secretion (SIADH) presents with excess ADH release caused by a range of conditions; including pneumonia, brain tumors, certain lung cancers, and diseases of the hypothalamus. It presents with significant reduction in both sodium and chloride concentrations in the blood. However, reports examining the acid base status indicate a normal serum bicarbonate concentration and systemic acid base homeostasis. The mechanisms for the absence of abnormalities in acid base homeostasis remain speculative. This mini review is highlighting the recent advances in renal molecu

Syndrome of inappropriate antidiuretic hormone secretion as an adverse reaction of ciprofloxacin: a case report and literature review.

Antidiuretic hormone (ADH) is secreted by the posterior pituitary gland. Unsuppressed release of ADH leads to hyponatremia. This condition is referred to as syndrome of inappropriate antidiuretic hormone secretion (SIADH). Hereby, a case report is presented on ciprofloxacin-induced SIADH. A 67-year-old male patient was examined in the emergency room with symptoms of lethargy, headache, lack of attention, and a generally depressed mood lasting for three days. One week prior, empirical antimicrobial therapy involving ciprofloxacin for prostatitis was initiated. Laboratory analysis showed no rele
