Chemotherapy-induced transverse ridging of the nails.

A case of Beau's lines secondary to combined chemotherapy for Hodgkin's disease is reported. These transverse nail depressions are interpreted as the result of a transient interruption of the nail matrix activity due to antimitotic drugs. Other causes of Beau's lines are reviewed.

Fetal phenytoin exposure, hypoplastic nails, and jitteriness.

In a prospective study infants born to mothers with epilepsy (n = 61) were found to have an unexpectedly high incidence of congenital anomalies (26/61, 43%) and neonatal conditions (26/61, 43%) compared with controls (0/62, and 6/62, 10%, respectively). There were two neonatal deaths in the study group but none among the controls. Hypoplasia of the finger or toenails was a common congenital anomaly in those infants whose mothers had received phenytoin alone or in combination with other anticonvulsant drugs (11 of 40, 28%). The mean serum phenytoin concentration was higher among mothers of infa…

Brown nails as clues for shock diagnosis.

A previously healthy 60-year-old Japanese male farmer presented with shock, initially unresponsive to fluid resuscitation. Upon incidental observation of brown nail discoloration, adrenal insufficiency was suspected, confirmed by elevated ACTH levels. The timely recognition of nail hyperpigmentation led to the diagnosis of tuberculous Addison's disease, allowing for appropriate treatment and resolution of the condition.

Mechanical Failures in Magnetic Intramedullary Lengthening Nails.

BACKGROUND: Magnetic intramedullary lengthening nails (MILNs) have gained popularity in recent years for the treatment of limb-lengthening discrepancies. We sought to catalog mechanical failures and their prevention and management in a large, single-institution series. We specifically assessed the rate of mechanical failures, the types of failures observed, and management strategies. METHODS: We retrospectively reviewed 377 patients (420 limbs) who underwent limb lengthening with early (P1) or later (P2) MILN iterations with ≥12-month follow-up. Mechanical failure was defined as mechanical br…

Psoriasiform Onychodystrophy Induced by Photobonded Acrylic Nails.

Multiple cases have reported onychodystrophy secondary to acrylic nails. We present a case of onychodystrophy with psoriasiform nail changes, including onycholysis, splinter hemorrhages, hyperkeratosis, and nail plate thinning, caused by gel manicures. Histopathological analysis of the nail plate and subungual debris revealed neutrophils in the absence of fungal elements. Although the presence of neutrophils in the nail plate material in conjunction with characteristic psoriatic nail changes suggested a diagnosis of psoriasis, certain key features of nail psoriasis, including oil spots, salmon…

[Phenolisation: treatment of choice for ingrown nails].

Ingrown nails are a common and painful pathology not always responding to local treatment. Destroying the matrix by application of phenol (phenolisation) has proven its superiority over the so called classic surgical methods. This is partially due to the exceptional properties of phenol: demyelinisation of the terminal nerve fibres provides long-term pain relief, the coagulation of proteins stops bleeding and the molecule has antiseptic properties. Phenolisation requires only few resources and can be widely used. Recurrence rates are particularly low at 2 %. We present this technique as the tr…

Intraoperative customization of intramedullary nails - First results.

INTRODUCTION: Intramedullary nailing is a valuable treatment option for many types of fractures. Furthermore nailing is applicable for osteosynthesis following osteotomy in deformity correction. For intraoperative fine-tuning of alignment in intramedullary (IM) nailing procedures, a bending device for customization of IM nails under sterile conditions was developed. We have performed a retrospective clinical study to analyze and describe the technical requirements, indications and limitations for intraoperative customization of IM nails. MATERIALS AND METHODS: In 41 cases of deformity correct…

Spoon nails: still seen in today's world.

Koilonychia is a nail abnormality characterized by thin, brittle, and spoon-shaped nails. It is frequently observed in chronic iron deficiency secondary to malnutrition, chronic blood loss, or malabsorption. It may also be idiopathic or related to occupation and rare systemic disorders. Presence of koilonychia should prompt investigations for iron deficiency.

Worn down nails after acrylic nail removal.

Worn-down nail syndrome is a nail disorder characterized by thinning of the distal nail plate caused by repetitive chemical or mechanical trauma. We present a previously undescribed source of worn-down nail syndrome caused by trauma from nail filing after acrylic nail removal.

Pachyonychia congenita tarda affecting only the nails.

A 42-year-old woman presented with complaints of rapidly progressing thickening and yellowish discoloration of the nails for the past 24 months. All nails were affected and fingernails were more thickened than toenails. Her palms and soles were normal. Keratosis pilaris, palmoplantar blistering, hyperhidrosis, leukokeratosis, alopecia, dental malformation, corneal abnormalities and epidermoid cysts were absent. This patient has pachyonychia congenita tarda with clinical manifestations limited to nail involvement.

Pincer nails complicated by distal phalangeal osteomyelitis.

Besides the known cosmetic implications, pincer nails are often complicated by pain, chronic inflammation and recurrent suppurative infections. We report two such cases of chronic paronychia of the thumb secondary to pincer nails. Both were complicated by osteomyelitis. Although both cases eventually healed after excisional debridement, one case required elective amputation because the significant loss of bony support resulted in a painful, floppy fingertip and poor pinch. Regular radiographic assessments may be useful in the management of patients with chronic inflammation or recurrent parony…

Prolonged paresthesia due to sculptured acrylic nails.

Allergic reactions to acrylates in sculptured nails are common. However, there are few reports of paresthesia as a consequence of acrylic exposure, and such paresthesia does not necessarily accompany the evolution of the dermatitis.
