Rapid progression of a severe femoral bone loss in a stable revision hip prosthesis: causes and management.

We report a case of a severe bone loss of the proximal femur with a rapid progression, in a 72-year-old male patient with a stable total revision hip prosthesis. The patient had a persistent mechanical thigh pain. The blood laboratory values were normal. Infection disease and osteolytic bone tumor were excluded. A surgical procedure was performed. The native bone of the proximal femur was resorbed and replaced by a dense fibrous tissue with some sclerotic bone fragments. A large amount of a brownish fluid and a red-brown and friable tissue were found in the pseudojoint cavity. The proximal fem

A Survey of Retention and Support for Anterior Bounded Saddles of Removable Partial Dentures.

The treatment for the anterior bounded saddle because of missing teeth presents several challenges, primarily that of aesthetics. The choice of treatment depends on the position of the saddle, its size, the amount of available alveolar bone support, the health of the abutment teeth, and the patient's wishes. With regard to the use of a removable partial denture (RPD), while acknowledging the significant importance of aesthetics, unless the presence of adequate retention of the prosthesis is ensured, the appearance of the teeth alone will not guarantee its acceptability. This article highlights t

Biomechanics of Implant Fixation in Osteoporotic Bone.

PURPOSE OF REVIEW: The purpose of this review is to critically evaluate the current literature regarding implant fixation in osteoporotic bone. RECENT FINDINGS: Clinical studies have not only demonstrated the growing prevalence of osteoporosis in patients undergoing total joint replacement (TJR) but may also indicate a significant gap in screening and treatment of this comorbidity. Osteoporosis negatively impacts bone in multiple ways beyond the mere loss of bone mass, including compromising skeletal regenerative capacity, architectural deterioration, and bone matrix quality, all of which cou

An integrated hollow bulb obturator prosthesis with a metal framework for a soft palate defect fabricated by multiple digital techniques.

The digital workflow to fabricate an integrated hollow bulb obturator prosthesis with a metal framework for a patient with soft palate defect is described. The framework was digitally designed with an open lattice denture base connector to facilitate the assembly of the hollow bulb obturator and printed with titanium. A functional impression of the palatopharyngeal area was made, and an integrated 3-dimensional (3D) cast was obtained by aligning the data of the functional impression to the preliminary intraoral scan data. The hollow bulb obturator and a palatal cover were designed based on the

Clinical complications in fixed prosthodontics.

The purpose of this article is to identify the incidence of complications and the most common complications associated with single crowns, fixed partial dentures, all-ceramic crowns, resin-bonded prostheses, and posts and cores. A Medline and an extensive hand search were performed on English-language publications covering the last 50 years. The searches focused on publications that contained clinical data regarding success/failure/complications. Within each type of prosthesis, raw data were combined from multiple studies and mean values calculated to determine what trends were noted in the st

Effect of attachment type and implant position on the retention of mandibular implant-supported overdentures.

BACKGROUND: In implant-supported overdentures increase in the number of implants improves the retention and stability of the overdentures. A direct correlation exists between prosthesis retention and patient satisfaction. Therefore, this experimental study assessed the effect of attachment type and implant position on the retention of mandibular implant-supported overdentures. METHODS: A transparent acrylic resin model of the mandible was fabricated, and dental implants were positioned at the first molar area (position 6), between the lateral incisor and canine teeth (positions B and D), and …

Surgical technique of minor revision of a transcutaneous osseointegration prosthetic system (TOPS) with implant retention.

OBJECTIVE: Transcutaneous osseointegration prosthetic systems (TOPS) offer a stable skeletal attachment for artificial limbs post-extremity amputation, serving as an alternative to socket attachment. Press-fit osseointegration implants (OI) utilized in TOPS consistently enhance quality of life and mobility for amputees, particularly those experiencing socket-related issues. Despite notable benefits, late complications such as infection and implant loosening pose challenges unique to TOPS due to their percutaneous nature. Recent studies indicate a low risk of implant failure but highlight the p

Thumb Reconstruction Using Osseointegrated Dental Implants for Prosthetic Retention Following Gunshot Amputation: A Case Report.

Thumb amputations severely compromise hand function, particularly grip and opposition. Osseointegration, originally developed for dental implantation, has become a promising method for directly anchoring prostheses to bone. This technique uses a two-stage protocol: first, a dental implant is placed into the medullary canal and allowed to integrate with the surrounding bone; once stable osseointegration is confirmed, a skin-penetrating abutment and custom prosthesis are attached. This approach provides secure fixation even when bone stock is limited and offers several advantages over convention

Management of Periprosthetic Hip Joint Infection.

Total hip joint replacement offers dramatic improvement in the quality of life but periprosthetic joint infection (PJI) is the most devastat … tay for diagnosis of PJI. Treatment options mainly include thorough irrigation and debridement with prosthesis retention, or a two-stage prosthesis exchange with intervening placement of an antibiotic-loaded spacer. Success in treating PJI depends on extensive surgical debridement and adequate and effective antibiotic therapy. Treatment in two stages using a spacer is …

Use of the anterolateral thigh flap for reconstruction of the pediatric anophthalmic orbit.

Children with acquired anophthalmia pose unique reconstructive challenges. With implant-based reconstruction, the current standard of care, there is a tendency toward intraorbital and adnexal tissue contraction and an increasing volume discrepancy with orbital growth, resulting in enophthalmos and eyelid retraction, and also difficulty in retaining an ocular prosthesis. We describe a novel technique for secondary reconstruction of the pediatric anophthalmic orbit using a small free anterolateral thigh (ALT) flap. The technique involves release of retracted eyelids and restoration of orbital vo

The current state of facial prosthetics – A multicenter analysis.

Even though modern surgical techniques are dominating reconstructive facial procedures, the capability to use facial epitheses for reconstruction is still an important skill for the maxillofacial surgeon. We present an international multicenter analysis to clarify which techniques are used to fixate facial prostheses. We contacted all maxillofacial departments in Germany, Austria, Switzerland and Norway which were registered with the German society for oral and maxillofacial surgery (DGMKG). These centers were asked via electronical mail to provide information on the type of epithesis fixation
