Innovating shorter, all-oral, precise, individualized treatment regimen for rifampicin-resistant tuberculosis (INSPIRE TB): study protocol for a pragmatic randomised controlled trial.

BACKGROUND: Rifampicin-resistant tuberculosis (RR-TB) poses a great threat to global health. Increasing evidence of novel regimens in trial settings has risen, but the combinations of oral shorter regimens have not been optimised. An evaluation of the efficacy and safety of short-term regimens utilizing various drug combinations is essential across different settings. METHODS: INSPIRE TB is a pragmatic, multicentre, randomised, controlled, non-inferiority open-label trial to evaluate novel regimens in pulmonary RR-TB patients of 16-75 years old with or without fluoroquinolone resistance. From trials to programmatic scale-up: treatment outcomes of rifampicin-resistant tuberculosis during transition to short oral regimens in Vietnam (2021-2022).

OBJECTIVES: Vietnam is a high tuberculosis (TB) and rifampicin-resistant TB (RR-TB) burden country. Different new RR-TB regimens were recommended in recent years. Locally-generated evidence about nationwide implementation of different RR-TB regimens is essential to inform national treatment guidelines. METHODS: A retrospective cohort study of all patients with RR-TB treated nationwide in Vietnam from 2021 to 2022. Short treatment regimens (STR) were 9-11-month standardized 7-drug regimens with either bedaquiline (BDQ_STR) or an injectable drug (inj_STR), and a modified 9-11-month 5-drug regim

Cost-effectiveness of modified fully oral 9-month treatment regimens for rifampicin-resistant tuberculosis in Belarus, Georgia, Kazakhstan and the Republic of Moldova.

INTRODUCTION: Prior to 2020, treatment options for multidrug-resistant tuberculosis (MDR-TB) were limited and typically involved long treatment durations and high financial burdens. In the eastern European and central Asian (EECA) region, traditional inpatient tuberculosis (TB) care models, alongside high MDR-TB rates, escalate nosocomial transmission risks and treatment costs. Modified, fully oral, shorter treatment regimens (mSTR) implemented in the WHO European Region under operational research conditions offered a potential reduction in the burden of MDR-TB treatment for both patients and

Using dynamic oral dosing of rifapentine and rifabutin to simulate exposure profiles of long-acting formulations in a mouse model of tuberculosis preventive therapy.

Administration of tuberculosis preventive therapy (TPT) to individuals with latent tuberculosis infection is an important facet of global tuberculosis control. The use of long-acting injectable (LAI) drug formulations may simplify and shorten regimens for this indication. Rifapentine and rifabutin have anti-tuberculosis activity and physiochemical properties suitable for LAI formulation, but there are limited data available for determining the target exposure profiles required for efficacy in TPT regimens. The objective of this study was to determine exposure-activity profiles of rifapentine a

Estimation of Bait Uptake by Badgers, Using Non-invasive Methods, in the Perspective of Oral Vaccination Against Bovine Tuberculosis in a French Infected Area.

Although France is officially declared free of bovine tuberculosis (TB), Mycobacterium bovis infection is still observed in several regions in cattle and wildlife, including badgers (Meles meles). In this context, vaccinating badgers should be considered as a promising strategy for the reduction in M. bovis transmission between badgers and other species, and cattle in particular. An oral vaccine consisting of live Bacille Calmette-Guérin (BCG) contained in bait is currently under assessment for badgers, for which testing bait deployment in the field and assessing bait uptake by badgers are req

Protection against bovine tuberculosis induced by oral vaccination of cattle with Mycobacterium bovis BCG is not enhanced by co-administration of mycobacterial protein vaccines.

Mycobacterium bovis bacille Calmette-Guérin (BCG) delivered to calves by the oral route in a formulated lipid matrix has been previously shown to induce protection against bovine tuberculosis. A study was conducted in cattle to determine if a combination of a low dose of oral BCG and a protein vaccine could induce protective immunity to tuberculosis while not sensitising animals to tuberculin. Groups of calves (10 per group) were vaccinated by administering 2 × 10(7)colony forming units (CFU) of BCG orally or a combination of 2 × 10(7)CFU oral BCG and a protein vaccine comprised of M. bovis cu

Lipid-formulated bcg as an oral-bait vaccine for tuberculosis: vaccine stability, efficacy, and palatability to brushtail possums (Trichosurus vulpecula) in New Zealand.

Bovine tuberculosis (Tb), due to infection with virulent Mycobacterium bovis, represents a threat to New Zealand agriculture due to vectorial transmission from wildlife reservoir species, principally the introduced Australian brushtail possum (Trichosurus vulpecula). An oral-delivery wildlife vaccine has been developed to immunize possums against Tb, based on formulation of the human Tb vaccine (M. bovis BCG) in edible lipid matrices. Here BCG bacilli were shown to be stable in lipid matrix formulation for over 8 mo in freezer storage, for 7 wk under room temperature conditions, and for 3-5 wk

Oral delivery of BCG Moreau Rio de Janeiro gives equivalent protection against tuberculosis but with reduced pathology compared to parenteral BCG Danish vaccination.

There is a need for an improved vaccine to better control human tuberculosis (TB), as the only currently available TB vaccine, bacillus Calmette-Guerin (BCG) delivered parenterally, offers variable levels of efficacy. Therefore, recombinant strains expressing additional antigens are being developed alongside alternative routes to parenteral delivery. There is strong evidence that BCG Moreau (RdJ) is a safe and effective vaccine in humans when given by the oral route. This study compared the efficacy of a single oral dose of wild type BCG Moreau Rio de Janeiro (RdJ), or a recombinant RdJ strain

Durable three-year outcomes and persistent neuropathy following nine-month all-oral regimens for rifampicin-resistant tuberculosis: a multicentre cohort study in China.

BACKGROUND: Rifampicin-resistant tuberculosis (RR-TB) remains a major public health threat. Nine-month, all-oral regimens containing bedaquiline (Bdq) and linezolid (Lzd) have shown high cure rates, but evidence on the durability of results beyond 24 months and long-term linezolid-induced neurotoxicity is limited. METHODS: We conducted a multicentre, prospective cohort study (MDR-Chin) involving 124 adults with culture-confirmed MDR/RR-TB or pre-extensively drug-resistant tuberculosis (pre-XDR-TB) enrolled between May 2020 and July 2022. Patients received one of four nine-month, all-oral regim

Inhaled corticosteroids compared with oral prednisone in patients starting long-term corticosteroid therapy for asthma. A controlled trial by the British Thoracic and Tuberculosis Association.

Inhaled beclomethasone dipropionate and inhaled betamethasone valerate have been compared with oral prednisone in the treatment of 75 patients with asthma who were starting long-term corticosteroids for the first time. Both of the inhaled corticosteroids controlled asthma as well as did oral prednisone in those who had responded to therapy in the initial period of the trial. A daily dose of 400 mug of inhaled drug was approximately equivalent to 7-5 mg daily of prednisone. Prednisone suppressed the adrenal response to tetracosactrin, whereas the mean responses in the groups receiving inhaled c

Comparative Treatment Outcomes of Shorter Regimen in Modified All Oral Regimen with Injectable in Patients Treated for Rifampicin-resistant/Multidrug-resistant Tuberculosis in Tanzania.

BACKGROUND: Drug-resistant tuberculosis (TB), especially rifampicin-resistant/multidrug-resistant TB (RR/MDR-TB), remains difficult to treat due to toxic aminoglycosides. The World Health Organization recommended all-oral bedaquiline-based regimens, but evidence comparing their effectiveness to injectable-containing regimens is limited. This study evaluated treatment success between both approaches. METHODS: This was a retrospective study, which included 114 adults aged 18 years and above with RR/MDR-TB treated at Kibong'oto Infectious Diseases Hospital with either a 9-11-month injectable-con

Effects of Oral Liposomal Glutathione in Altering the Immune Responses Against Mycobacterium tuberculosis and the Mycobacterium bovis BCG Strain in Individuals With Type 2 Diabetes.

The World Health Organization (WHO) has identified type 2 diabetes (T2DM) as a neglected, important, and re-emerging risk factor for tuberculosis (TB), especially in low and middle-income countries where TB is endemic. In this clinical trial study, oral liposomal glutathione supplementation (L-GSH) or placebo was given to individuals with T2DM to investigate the therapeutic effects of L-GSH supplementation. We report that L-GSH supplementation for 3 months in people with T2DM was able to reduce the levels of oxidative stress in all blood components and prevent depletion of glutathione (GSH) in
