BACKGROUND: The enormous burden of tuberculosis (TB) worldwide is a major challenge to human health, but the costs and risks associated with novel drug discovery have limited treatment options for patients. Patients with a history of penicillin allergy pose a treatment dilemma. Unnecessary avoidance of this relatively nontoxic class of drugs exposes the patient to potentially more toxic drugs, increases health care costs, and contributes to the development of antibiotic resistance. Yet for those who truly have allergy or other serious adverse reactions to beta-lactams, the use of alternate drugs is a must. This article reviews current management strategies to determine which patients are good candidates for reintroduction of beta-lactams and which patients should continue avoidance.

Bleeding induced by beta-lactam antibiotics was reported for the first time in 1970. The phenomenon has been confirmed in subsequent studies but has never resulted in any general recommendations for, e.g., screening and treatment. Over a period of seven months, we diagnosed in our department three cases of presumed beta-lactam-induced bleeding in patients treated with high-dose penicillin for endocarditis and septic shock.

Consecutive isolates of Haemophilus influenzae were collected by the Public Health Laboratory, Bath between 1 June 1992 and 31 May 1993. Of 379 apparently distinct isolates, 216 originated from the respiratory tract, 102 from eyes and 61 from other sites. Transmitted resistance to recent cephalosporin antibiotics of the third generation was first proved in 1983 by the authors. Approximately five years later the authors revealed during systematic monitoring of bacterial strains resistant to cephtazidime, cephtriaxone and cephotaxim also transmitted resistance to these and other more recent beta-lactam antibiotics in Czechoslovakia. The authors advocate rational use of new cephalosporins and other beta-lactams (e. g. aztreonam) as reserve antibiotics.
