Clinical management of the organ donor.

There is a critical mismatch between available organs for transplant and acutely or critically ill patients with end-stage organ disease. Patients who may benefit from organ transplantation far outnumber available organs. The causes for this imbalance are multiple. One cause is family refusal to donate. A second cause is nonrecognition or delay in determination of brain death. A third cause is donor loss due to profound cardiopulmonary and metabolic instability consequent to brain-stem herniation and brain death. Family refusal may be addressed by education, public awareness, as well as close

Behavioural interventions for urinary incontinence in community-dwelling seniors: an evidence-based analysis.

UNLABELLED: In early August 2007, the Medical Advisory Secretariat began work on the Aging in the Community project, an evidence-based review of the literature surrounding healthy aging in the community. The Health System Strategy Division at the Ministry of Health and Long-Term Care subsequently asked the secretariat to provide an evidentiary platform for the ministry's newly released Aging at Home Strategy. After a broad literature review and consultation with experts, the secretariat identified 4 key areas that strongly predict an elderly person's transition from independent community living

The real cost of treating venous ulcers in a contemporary vascular practice.

BACKGROUND: Venous leg ulcers (VLUs) are a prevalent and morbid disease that consumes considerable health care resources. Estimates place the total costs for treatment of VLU at 1% of health care budgets in many industrialized countries. Unfortunately, there is little contemporary information on the total cost of treating VLU in the United States, particularly in a wound center staffed by vascular specialists. The purpose of this study was to define the actual cost of treating VLU and to identify factors influencing costs. METHODS: A cohort of 84 patients with active VLU (Clinical, Etiologic,

An update on Peginterferon beta-1a Management in Multiple Sclerosis: results from an interdisciplinary Board of German and Austrian Neurologists and dermatologists.

BACKGROUND: Interferon (IFN) beta drugs have been approved for the treatment of relapsing forms of multiple sclerosis (RMS) for more than 20 years and are considered to offer a favourable benefit-risk profile. In July 2014, subcutaneous (SC) peginterferon beta-1a 125 μg dosed every 2 weeks, a pegylated form of interferon beta-1a, was approved by the EMA for the treatment of adult patients with RRMS and in August 2014 by the FDA for RMS. Peginterferon beta-1a shows a prolonged half-life and increased systemic drug exposure resulting in a reduced dosing frequency compared to other available inte

The Comprehensive Post-Acute Stroke Services (COMPASS) study: design and methods for a cluster-randomized pragmatic trial.

BACKGROUND: Patients discharged home after stroke face significant challenges managing residual neurological deficits, secondary prevention, and pre-existing chronic conditions. Post-discharge care is often fragmented leading to increased healthcare costs, readmissions, and sub-optimal utilization of rehabilitation and community services. The COMprehensive Post-Acute Stroke Services (COMPASS) Study is an ongoing cluster-randomized pragmatic trial to assess the effectiveness of a comprehensive, evidence-based, post-acute care model on patient-centered outcomes. METHODS: Forty-one hospitals in

Learning Collaboratives in the HEALing Communities Study: An Approach to Providing Training and Technical Assistance.

BACKGROUND: Reducing opioid overdose death rates across the United States demands novel community partnerships and information sharing to implement and sustain evidence-based opioid overdose prevention strategies. During the NIH-funded HEALing Communities Study, a multisite intervention to reduce opioid overdose deaths, researchers from 4 states (MA, NY, OH, KY) used Learning Collaboratives as part of training and technical assistance. Learning Collaboratives are forums for gathering people with intersecting goals intended to promote peer-based learning and build professional networks. OBJECT

The AIMS Clinic Model: A Retrospective Analysis of Interdisciplinary Movement-Neuropalliative Care for People With Late-Stage Parkinsonism.

BACKGROUND AND OBJECTIVES: Patients with late-stage parkinsonism face complex, progressive symptoms requiring specialized interdisciplinary care, yet the unique needs of these patients and family care partners are often inadequately addressed in standard neurology practice. Evidence supporting innovative palliative care models for late-stage movement disorders remains limited. Although interdisciplinary movement-palliative clinics have expanded over the past decade, many provide consultation-only services, lack longitudinal follow-up, meet infrequently, or face sustainability challenges. An Explanatory Mixed-Methods Study of Intensive Care Unit Net Benefit: Triage and Trajectory for Sepsis and Acute Respiratory Failure.

Rationale: Patients with sepsis and/or acute respiratory failure are at high risk for death or long hospital stays, yet limited evidence exists to guide triage to intensive care units (ICUs) or general medical wards for the majority of these patients who do not initially require life support. Objectives: To identify factors that influence how hospitals triage patients with capacity-sensitive conditions and those factors that may account for observed ICU relative to ward, or ward relative to ICU, benefits for such patients. Methods: We conducted an explanatory sequential mixed-methods study. As

Participatory Design of AI-Based Clinical Decision Support Systems: Scoping Review.

BACKGROUND: AI-based clinical decision support systems (CDSS) can improve diagnostics and treatment decisions, but they are rarely implemented in practice. Barriers include limited integration into clinical workflows, lack of transparency, and insufficient involvement of end users in system design. Participatory and user-centered approaches offer ways to address these challenges by aligning development processes with the needs and routines of clinical staff. However, systematic evidence on how such approaches are applied in the development of AI-based CDSS remains limited. OBJECTIVE: Our stud

A Smart System for Remote Monitoring of Patients in Palliative Care (HumanITcare Platform): Mixed Methods Study.

BACKGROUND: Due to the complexities of advanced illnesses and their treatments, it can be difficult for patients in palliative care to maintain their quality of life. Telemedicine interventions in chronic disease management engage patients in their care, provide continuous follow-up by their health care providers, identify symptoms earlier, and allow a quick response to illness-related decline. OBJECTIVE: We aimed to detail and reflect on the design of an app and evaluate its feasibility to monitor the clinical situation of patients with advanced illnesses. METHODS: This study used a mixed m

Physical restraint and associated agitation.

BACKGROUND: Physical restraint of patients in intensive care units (ICUs) has an estimated prevalence of 50%. Many medical centres do not have specific protocols for physical restraint, and the decision of whether to physically restrain a patient is up to the nursing staff. Disadvantages of physical restraint include injuries, exacerbation of agitation and an increased risk of developing post-traumatic stress disorder (PTSD). AIM: To report prevalence and outcomes in terms of morbidity and mortality of physical restraint in general ICU patients in an 800-bed secondary medical centre. STUDY D

[Alcohol dependence syndrome and before-discharge intervention method (BDIM)--Report 1. The process to develop the constructed BDIM].

Many alcoholics come to the hospital denying their own drinking problems. So, in the initial treatment stage for alcoholics, it is very important to bring patients' attention to their denial. In those days, there were many kinds of treatment method to help them aware of patients' denial. For example, psycho-educational therapy, Japanese Naikan therapy, attendance to self-help group meetings, and so on. But I don't think that these are effective enough to help them aware of their drinking problems, especially denial. The purpose of my study is to develop the therapeutic intervention method (Bef

Development of S-IMCICA_V.2: A User-Centered Educational and Decision-Support mHealth App for Early Detection of Childhood Cancer.

Early detection of childhood cancer is a key determinant of survival, as diagnostic delays result in advanced disease, higher treatment toxicity, and increased mortality. In low- and middle-income countries (LMICs), limited cancer education and awareness among primary care providers (PCPs) contribute to diagnosis delays, highlighting the need for educational interventions that support clinical decision-making. Mobile health (mHealth) tools offer a promising cancer education strategy; however, few are designed to address learning needs in local contexts. With support from the My Child Matters p

Impact of Dedicated Nurse Time on Electronic Health Record (EHR)-Based Diabetes Quality Measures: A Pre-Post Observational Study.

BACKGROUND AND OBJECTIVE: Despite major advances in diabetes therapies, only about 60% of U.S. patients achieve recommended glycemic targets in primary care. Inconsistent electronic health records (EHR) complicate diabetes care delivery and highlight the need for strategies that support accurate documentation and patient engagement. The objective of this project is to have 50% of patients with diabetes meet six quality measures: hemoglobin A1c, blood pressure, statin use, aspirin use, tobacco status, and urine microalbumin.  Methods: In April 2019, a registered nurse (RN) was tasked with overs
