Double hearing protection and speech intelligibility-room for improvement.

INTRODUCTION: Double hearing protection is used in many air forces around the world for protection in noisy aircraft environments, particularly in helicopters. The usual combination is foam ear plugs under headset or helmet muffs. Much of the research that spurred the introduction of foam earplugs indicated little change in speech intelligibility in persons with normal hearing. However, aircrew often complain about having to maximize intercom volume for speech understanding, causing a situation with no reserve volume and bad sound quality. In recent years, further developments have included so

Performance of continuous glucose monitoring systems (CGMs) during commercial flights in T1DM: A within-subject comparative pilot study.

AIMS: Continuous glucose monitoring (CGM) systems are increasingly used, but their accuracy under flight-conditions remains insufficiently studied. We assessed sensor accuracy of four CGMs during in-flight (mid-haul) and ground conditions. MATERIALS AND METHODS: Participants (n = 20) wore four sensors (Abbott Libre 3, Dexcom G7, Medtronic Simplera, and Sinocare iCan i3) in parallel, inserted 1 day prior to experiment. Capillary reference measurements were performed every 30 min. Accuracy evaluation included: proportion of CGM values within 10% of comparator glucose levels >100 mg/dL or within

Drone versus ground delivery of simulated blood products to an urban trauma center: The Montreal Medi-Drone pilot study.

BACKGROUND: Timely and safe distribution of quality blood products is a major challenge faced by blood banks around the world. Our primary objective was to determine if simulated blood product delivery to an urban trauma center would be more rapidly achieved by unmanned aerial vehicle (UAV) than by ground transportation. A secondary objective was to determine the feasibility of maintaining simulated blood product temperatures within a targeted range. METHODS: In this prospective pilot study, we used two distinct methods to compare UAV flight duration and ground transport times. Simulated bloo

Live 3D TEE demonstrates and guides the management of prosthetic mitral valve obstruction.

A 43-year-old woman, with a remote history of rheumatic mitral stenosis and a St. Jude prosthetic mitral valve replacement, presented with shortness of breath and palpitations, shortly after a long flight. On admission, atrial fibrillation with a rapid ventricular response was noted in the setting of a long history of noncompliance with her anticoagulation. Transesophageal echocardiography (TEE) demonstrated multiple laminated thrombi in the left atrial appendage. Live three-dimensional (3D) TEE confirmed this diagnosis and demonstrated an immobile posterior leaflet of the mitral prosthesis, w

Panic disorder in military aviators: a retrospective study of prevalence.

INTRODUCTION: The occurrence of panic disorder (PD) among military pilots and navigators poses questions with respect to medical care and waiver policy, but the prevalence of such disorders is unclear. We studied the epidemiology of PD in a USAF aircrew population. METHODS: Occurrence of PD during the period 2001-2008 was determined using the USAF Aeromedical Information Management Waiver Tracking System, a database containing medical disqualifications and waivers for the entire population of both qualified and disqualified (grounded) USAF aviators. RESULTS: The mean annual USAF pilot and na

Thrombotic risk factors: basic pathophysiology.

Although venous thrombosis has been traditionally associated with stasis and hypercoagulability, arterial thrombosis is mainly associated with heightened platelet reactivity and damage to the vessel wall. Accordingly, classic risk factors for venous and arterial thrombosis are usually considered distinct. Those for the former include cancer, surgery, pregnancy, and estrogens use, whereas risk factors of arterial thrombosis include smoking, hypertension, diabetes, the metabolic syndrome, and hyperlipidemia. However, a number of studies have recently challenged this dichotomy, and it is now reco

Motion sickness, stress and the endocannabinoid system.

BACKGROUND: A substantial number of individuals are at risk for the development of motion sickness induced nausea and vomiting (N&V) during road, air or sea travel. Motion sickness can be extremely stressful but the neurobiologic mechanisms leading to motion sickness are not clear. The endocannabinoid system (ECS) represents an important neuromodulator of stress and N&V. Inhibitory effects of the ECS on N&V are mediated by endocannabinoid-receptor activation. METHODOLOGY/PRINCIPAL FINDINGS: We studied the activity of the ECS in human volunteers (n = 21) during parabolic flight maneuvers (PFs)

Cerebral vasoconstriction precedes orthostatic intolerance after parabolic flight.

The effects of brief but repeated bouts of micro- and hypergravity on cerebrovascular responses to head-up tilt (HUT) were examined in 13 individuals after (compared to before) parabolic flight. Middle cerebral artery mean flow velocity (MCA MFV; transcranial Doppler ultrasound), eye level blood pressure (BP) and end tidal CO(2) (P(ET)CO(2)) were measured while supine and during 80 degrees HUT for 30 min or until presyncope. In the postflight tests subjects were classified as being orthostatically tolerant (OT) (n = 7) or intolerant (OI) (n = 6). BP was diminished with HUT in the OT group in b

Urban helicopter response to the scene of injury.

Metropolitan Houston with a population of four million has the nation's poorest freeway system. Its two Level I trauma centers are adjacent within a centrally located freeway loop, therefore the city is ideally suited for a trauma scene helicopter transport service. During 1981 there were 577 flights to the scene of injury (blunt, 466; penetrating, 111). Flights were requested by 60 agencies (EMS, law enforcement, etc.). All flights were manned by a surgical resident and flight nurse. The flight distances ranged from 2 to 57 miles (average, 14.4). Three hundred six flights (53%) were within th

Is virtual reality always better than 2D videos? A head-to-head randomized controlled trial for fear of flying with long-term follow-up.

Fear of flying affects up to 40 % of the global population, often leading to avoidance behaviors that significantly restrict personal and professional activities. Cognitive Behavioral Therapy is a well-established treatment, with exposure therapy as the gold-standard approach to reducing avoidance. Exposure therapy can be delivered through various methods, including virtual reality environments or traditional techniques such as two-dimensional (2D) videos, photographs, imagination, or real-life situations. While Virtual Reality Exposure Therapy (VRET) and 2D video exposure share similarities,

[CME: Fear of Flying and Flying Phobia].

CME: Fear of Flying and Flying Phobia Abstract. Fear of flying and flying phobia are very common in the general population and can lead to substantial distress. Symptoms and diagnosis of fear of flying and flying phobia will be described. Severe fear of flying can be diagnosed as specific phobia or alternatively as agoraphobia. Cognitive behavioral therapy with exposure in vivo has been shown to be the most effective treatment. The treatment with benzodiazepines is the main priority for the general practitioner and will be subsequently discussed. The treatment of flying phobia and agoraphobia

Measles outbreak associated with an arriving refugee - Los Angeles County, California, August-September 2011.

Measles is a highly communicable, acute viral illness with potential for severe complications, including death. Although endemic measles was eliminated in the United States in 2000 as a result of widespread vaccination, sporadic measles outbreaks still occur, largely associated with international travel from measles-endemic countries and pockets of unvaccinated persons. On August 26, 2011, the Los Angeles County Department of Public Health (LACDPH) was notified of suspected measles in a refugee from Burma who had arrived in Los Angeles, California, on August 24, after a flight from Kuala Lumpu

Phylo- and ontogenetic fears and the expectation of danger: differences between spider- and flight-phobic subjects in cognitive and physiological responses to disorder-specific stimuli.

To compare specific phobias with an assumed phylogenetic or ontogenetic origin in responses to fear-relevant (FR) stimuli, 17 spider- and 17 flight-phobic participants were exposed to pictures of spiders, flight accidents, or mushrooms randomly followed by either a startling noise or nothing else. While both groups showed a disorder-specific expectancy bias, only spider-phobic participants exhibited a disorder-specific covariation bias. Spider-phobic participants also showed enhanced skin conductance responses (SCRs), event-related brain potentials (ERPs), and startle responses triggered by di

Science for the community: assessing mental health after 9/11.

Reactions to the September 11 attacks across the United States were pervasive, and persons throughout the country reported experiences akin to posttraumatic stress disorder (PTSD) in the first week following the attacks. In the New York area, 2 major surveys conducted 4 to 8 weeks after the attacks found that approximately 1 in 10 persons probably met full criteria for PTSD related to September 11. Although tobacco, alcohol, and marijuana use did increase, it was largely among persons already using these substances. The greatest increase, not surprisingly, occurred among persons with PTSD and

Venous thrombosis from air travel: the LONFLIT3 study--prevention with aspirin vs low-molecular-weight heparin (LMWH) in high-risk subjects: a randomized trial.

The LONFLIT1 and 2 studies established that in high-risk subjects after long (>10 hours) flights, the incidence of deep venous thrombosis (DVT) may be between 4% and 6%, The LONFLIT3 study aimed to evaluate methods of prevention in high-risk subjects. Of 467 subjects contacted for the study, 300 were included. These 300 subjects at high risk for DVT were randomized, after informed consent, into three groups: 1) a control group that had no prophylaxis; 2) an aspirin treatment group, in which patients were treated with 400 mg (tablets of oral, soluble aspirin; one dose daily for 3 days, starting

On-scene helicopter transport of patients with multiple injuries--comparison of a German and an American system.

Hospital-based helicopter services from a German (GER) and an American (AMR) university-affiliated trauma center were reviewed. All patients with multiple injuries transported via helicopter from the scene to the trauma centers during a 1-year period were included. The patients were comparable regarding mechanism of injury, age, flight times, mean ISS, ISS distribution, and number of severe injuries per body region (patients with AIS score > 3 for head, thorax, and abdomen). Overall mortality was 21 of 221 (9.5%) for GER and 21 of 186 (11.3%) for AMR (NS). Survivor-based TRISS analysis yielded

Surgical cricothyrotomy in the field: experience of a helicopter transport team.

A retrospective analysis of 3,500 helicopter missions revealed 20 patients who required cricothyrotomy in the field for emergency airway access. Five patients who were in cardiopulmonary arrest succumbed despite cricothyrotomy, all with adequate airway control (Trauma Score, 2.8; ISS, 55.6). Seven of 12 patients with oral, maxillofacial, or cervical trauma survived (Trauma Score, 9.6; ISS, 48.25). There were no instances of bleeding, malposition, airway obstruction, or dysphonia after decannulation in the survivors. Autopsy revealed no serious airway pathology or compromise in those who expire
