Management of a post-ELLKAT keratectasia with a gas permeable contact lens.

BACKGROUND: The Excimer laser keratoplasty of augmented thickness for keratoconus (ELLKAT) has been proposed for the refractive management of keratoconus. CASE REPORT: A 41-year-old man with a history of bilateral keratoconus came to the clinic for a contact lens. He had undergone an ELLKAT procedure in his right eye some months earlier in an attempt to obtain acceptable unaided vision. As the result in this eye was not positive, the surgery in the fellow eye was cancelled. On examination, I diagnosed a centrally located keratectasia (KE) in his RE. After surgery, the corneal shape presented

[mRNA expression of transforming growth factor-beta 1 in the cornea after photorefractive keratectomy in rabbits].

OBJECTIVE: To investigate the roles of transforming growth factor beta 1 (TGF-beta 1) during healing after photorefractive keratectomy (PRK). METHODS: Twenty of 24 rabbits underwent bilateral 193 nm excimer laser photorefractive keratectomy to correct 10 diopters of myopia, and the other 4 rabbits were taken as the normal group. The mRNA levels of TGF-beta 1 in the corneas of the rabbits were determined by in situ hybridization with a Digoxigenin labled probe. RESULTS: 1. Corneal haze appeared in operated eyes from the 7th day, and peaked on the 28th day after PRK. The most severe haze reach

Keratorefractive Lenticule Extraction Using a Novel Nomogram Software Tool for High Astigmatism: Refractive Outcomes and Aberrations.

PURPOSE: To develop a VISULYZE (Carl Zeiss Meditec AG)-generated nomogram for keratorefractive lenticule extraction (KLEx) in patients with high astigmatism and to compare refractive accuracy and visual outcomes between surgeries performed with and without the nomogram. METHODS: This study included 43 patients (57 eyes) with pre-operative refractive astigmatism of 1.50 diopters (D) or greater who underwent KLEx with the VISULYZE nomogram and 34 patients (45 eyes) in a matched control group who underwent KLEx without the nomogram. The primary outcomes were the predictability of residual astigm

Analysis of the impact of FS-LASIK on peripheral retinal defocus and its influencing factors in myopic adults.

PURPOSE: To assess the retinal peripheral defocus changes following femtosecond laser in situ-keratomileusis (FS-LASIK) in myopic adults and analyse influencing factors. METHODS: This study recruited 202 patients who underwent FS-LASIK with myopia at Beijing Ming Vision Eye Hospital. Participants were divided into low myopia (LM, -0.50D to -3.00D), moderate myopia (MM, -3.00D to -6.00D), and high myopia (HM, ≥-6.00D) groups according to spherical equivalent (SE). We measured axial length (AL), total retinal defocus (TRDV), and defocus values at eccentricities ranging from 0° to 53° from the f

Analysis of flap morphology and visual outcomes using a new femtosecond-laser platform: An observational study.

PURPOSE: To evaluate the accuracy in terms of thickness of LASIK flaps created using the ELITATM femtosecond laser (Johnson and Johnson Surgical Vision, Inc) compared to the intended thickness. METHODS: This retrospective, single-center study was conducted at the Institut Noemie de Rothschild (Paris, France) in patients eligible for bilateral LASIK surgery. The ELITATM femtosecond laser was used to create all flaps, with an intended thickness of 110μm or 120μm. Flap thickness was measured manually by using images from two Optical Coherence Tomography (OCT) devices: Anterion (Heidelberg Engine

A Nomogram to Improve the Predictability of High Myopic Astigmatism in Small Incision Lenticule Extraction Surgery.

PURPOSE: To evaluate a new regression-derived nomogram for high myopic astigmatism in small incision lenticule extraction (SMILE) surgery. METHODS: In this prospective study, data from 180 eyes with myopic astigmatism ranging from -2.50 to -4.50 diopters (D) at 3 months after SMILE surgery were analyzed to construct the astigmatic nomogram. Linear regression between target induced astigmatism and flattening effect formula was derived as a nomogram to adjust astigmatic treatment. The nomogram was applied to 112 eyes with astigmatism ranging from -2.75 to -4.50 D. Subsequently, 143 eyes (manife

Effects of effective optical zone and decentration on visual quality after smile for different astigmatism types.

PURPOSE: To compare the refractive outcomes and visual quality among different types of astigmatism following SMILE and evaluate effective optical zone (EOZ) features, decentration and their potential effects on visual quality. METHODS: This study included 101 left eyes of 101 patients who underwent SMILE. Patients were grouped according to astigmatism types (with-the-rule [WTR], against-the-rule [ATR] and oblique astigmatism) and decentered displacement (major axis > minor axis and major axis < minor axis). Objective Monitoring of Corneal Backward Light Scattering After Femtosecond Laser-assisted LASIK.

PURPOSE: To investigate the changes in corneal backward light scattering, as measured by a rotating Scheimpflug camera with automated corneal densitometry software, in eyes treated with femtosecond laser-assisted LASIK (FS-LASIK). METHODS: The cornea was examined preoperatively and postoperatively at 1 day, 1 week, and 1, 3, and 6 months in 23 patients who underwent myopic FS-LASIK. Local analysis of corneal backscatter was performed on four concentric radial zones across a 12-mm diameter (0 to 2, 2 to 6, 6 to 10, and 10 to 12 mm) and at a different corneal depth (anterior 120 µm, central and

Induced Higher-order aberrations after Laser In Situ Keratomileusis (LASIK) Performed with Wavefront-Guided IntraLase Femtosecond Laser in moderate to high Astigmatism.

BACKGROUND: Wavefront-guided Laser-assisted in situ keratomileusis (LASIK) is a widespread and effective surgical treatment for myopia and astigmatic correction but whether it induces higher-order aberrations remains controversial. The study was designed to evaluate the changes in higher-order aberrations after wavefront-guided ablation with IntraLase femtosecond laser in moderate to high astigmatism. METHODS: Twenty-three eyes of 15 patients with moderate to high astigmatism (mean cylinder, -3.22 ± 0.59 dioptres) aged between 19 and 35 years (mean age, 25.6 ± 4.9 years) were included in this

LASIK-induced aberrations: comparing corneal and whole-eye measurements.

PURPOSE: To compare the change in aberrations produced by laser-assisted in situ keratomileusis (LASIK) measured by either front surface corneal aberrometry or Hartmann-Shack whole-eye aberrometry. METHODS: This was a retrospective case series from consecutive patients treated with LASIK at the London Vision Clinic, London, UK. Corneal and ocular wavefront data were collected before and after LASIK for myopia and hyperopia. To control for centration of corneal and whole-eye aberration measurements, only patients with pupil offset (measured as the distance between the corneal vertex and the en

Ocular residual astigmatism's effect on high myopic astigmatism LASIK surgery.

PURPOSE: To analyze the effect of ocular residual astigmatism (ORA) on the effectiveness of LASIK for treating high myopic astigmatism. METHODS: This is an observational, cross-sectional study. We studied 116 consecutive myopic eyes with -3 diopters (D) or more of astigmatism that underwent LASIK surgery. The magnitude of uncorrected residual refractive astigmatism 3 months postoperatively was measured. RESULTS: The mean preoperative cylinder was -4.0±0.83 D (range, -7.5 to -3 D) and the mean preoperative ORA was 0.82±0.5 D. The mean residual refractive cylinder 3 months postoperatively was
