Maternal bond with cardiosurgically treated infant. Qualitative analysis of mothers' narratives.

The aim of this work is to describe the experience of being a mother by women who together with their children stay on the ward after cardiac surgical correction of congenital heart defects. The research material consisted of the narratives of mothers whose children were born with a heart defect and surgically treated. Four women aged 21-30 years were participants of this study. The age of the subjects' children ranged from 5 weeks to 1 year and three months. The heart defects with which the children were born were hypoplastic left heart syndrome (HLHS ) or atrioventricular canal defect. The r

A dynamic look at narrative change in psychotherapy: a case study tracking innovative moments and protonarratives using state space grids.

This study aims to further the understanding of how innovative moments (IMs), which are exceptions to a client's problematic self-narrative in the therapy dialogue, progress to the construction of a new self-narrative, leading to successful psychotherapy. The authors' research strategy involved tracking IMs, and the themes expressed therein (or protonarratives), and analysing the dynamic relation between IMs and protonarratives within and across sessions using state space grids in a good-outcome case of constructivist psychotherapy. The concept of protonarrative helped explain how IMs transfor

Safeguarding precarious survival: parenting children who have life-threatening heart disease.

The purpose of this study is to describe the process of parenting a child with life-threatening heart disease. Despite advances in pediatric cardiac sciences, hypoplastic left heart syndrome remains difficult and controversial to treat. The Norwood surgical approach is a developing technology, and little is known about how mothers and fathers experience parenting a child who survives this treatment. Constructivist grounded theory informed this Canadian study that involved multiple interactive interviews with 9 mothers and 7 fathers of children with hypoplastic left heart syndrome who were at v

'It's caveman stuff, but that is to a certain extent how guys still operate': men's accounts of masculinity and help seeking.

It is often assumed that men are reluctant to seek medical care. However, despite growing interest in masculinity and men's health, few studies have focussed on men's experiences of consultation in relation to their constructions of masculinity. Those that have are largely based on men with diseases of the male body (testicular and prostate cancer) or those which have been stereotyped as male (coronary heart disease). This paper presents discussions and experiences of help seeking and its relation to, and implications for, the practice of masculinity amongst a diversity of men in Scotland, as

Optimizing a Novel Smartphone App to Prevent Postpartum Depression Adapted From an Evidence-Based Cognitive Behavioral Therapy Program: Qualitative Study.

BACKGROUND: Low-income pregnant patients are at high risk of postpartum depression (PPD). Mothers and Babies (MB) is a cognitive behavioral therapy-based program that prevents up to 50% of de novo PPD when provided in person to low-income Spanish- and English-speaking people who are pregnant without depression. MB is limited by the need for trained personnel to support it. Transforming MB into a smartphone app may mitigate this key barrier. OBJECTIVE: We aimed to use qualitative data from target end users to create and optimize MBapp, a novel app centered on the MB program. METHODS: Draft wi

Comparison of video-based and presentation-based self-directed instruction for learning basic neuroangiography.

BACKGROUND: Digital tools and simulation-based learning are playing an increasingly important role in medical education. Earlier studies have shown that video and virtual reality can be just as effective for teaching clinical skills as presentation-based teaching methods. This exploratory study compared two standardized self-directed instructional formats, video-based and presentation-based instruction, for teaching basic neuroangiographic skills. In addition to comparing objective and subjective learning outcomes between instructional formats, the study explored whether prior angiography expe

People experiencing homelessness and their experiences with substance use services for alcohol treatment in England: Journey mapping based on semi-structured interviews.

BACKGROUND AND AIMS: Alcohol consumption at high levels is common among people experiencing homelessness and contributes to inequalities. In relative terms these individuals are underrepresented in substance use services. This study captured common trajectories and identified barriers and facilitators to accessing treatment among a group of people currently experiencing homelessness and who have used or have been eligible for alcohol treatment. METHODS: Qualitative study in which we conducted 32 semi-structured 'journey-mapping' interviews with individuals currently experiencing homelessness

When the profession itself is contested: Transformative learning and divergent identity pathways among first-year medical students during a macro-social crisis.

INTRODUCTION: Professional identity formation (PIF) research in medical education has largely examined micro-level dilemmas within relatively stable training environments. Less is known about how macro-social crises destabilizing the profession's public legitimacy shape early PIF. This study explored how first-year medical students narrated the impact of the 2024-2025 doctor-government conflict in South Korea on their professional identity development, using Transformative Learning Theory (TLT) as an interpretive lens. METHODS: We conducted a theory-informed qualitative study using written re

'I've been here before, but never looked at the ceiling': Narratives of medical trainees who took illness-based leaves.

INTRODUCTION: Medical training is commonly imagined as a linear, uninterrupted progression, yet many trainees experience illness that necessitates a leave of absence (LOA). Illness-based leaves may be especially fraught given stigma, opaque processes and training cultures that prioritise endurance. Little is known about how trainees experience illness and illness-based leaves. METHODS: We conducted a narrative inquiry using Riessman's narrative analysis framework. Canadian physicians who had taken an illness-based LOA during medical school or residency were recruited nationwide. Semi-structur

A qualitative exploration of the mistreatment of women during childbirth in a rural Guatemalan hospital.

BACKGROUND: In Guatemala, many women still give birth together with traditional birth attendants at home particularly in rural settings even though births attended by skilled birth attendants in healthcare facilities have been recommended. Mistreatment of women during childbirth is recognized as one of the obstacles to childbirth in healthcare facilities. However, little research has been conducted on childbirth care and the mistreatment of women during facility-based childbirth in Guatemala. Thus, this study aimed to explore women’s experiences of care during childbirth in a rural Guatemalan

Living through the war in Gaza: An autoethnographic account of psychological, humanitarian, and physical suffering of Gaza's inhabitants during the war in Gaza.

INTRODUCTION: This paper presents an autoethnography from a Gaza-based scholar who documents, in real time, the war's effects on himself and his family. It situates a personal trajectory-bombardments, repeated displacement, injury, bereavement, and the collapse of essential services-within the broader humanitarian emergency, arguing that first-person evidence is indispensable to grasp lived experience and cumulative harm. MATERIALS AND METHODS: Using an autoethnographic case-study design, the author combines contemporaneous field notes, direct observations across hospitals and shelters, and f

Unexpected placenta accreta spectrum in an unscarred uterus causing catastrophic post-partum hemorrhage: a case report and review of the literature.

BACKGROUND: Placenta accreta spectrum (PAS) disorder is a fatal condition responsible for obstetric haemorrhage, which contributes to increased feto-maternal morbidity and mortality. The main contributing factor is a scarred uterus, often from a previous cesarean delivery, myomectomy, or uterine instrumentation. The occurrence of PAS in an unscarred uterus is extremely rare, with only anecdotal cases reported so far in the literature. We document one such case of unexpected placenta increta without identifiable risk factors presenting with severe postpartum hemorrhage. The management is often

Late antenatal care booking and associated factors among pregnant women in Ethiopia: evidence from 2019 performance monitoring for action data analysis.

INTRODUCTION: Pregnancy is a crucial period for a woman, her family, and society. Early initiation of antenatal care (ANC) follow-up helps to identify pre-existing health conditions and complications arising during pregnancy. It also allows the mother to receive health promotion and disease prevention services. On the other hand, late initiation or missing of ANC follow-up may lead to transmission of the infections to their fetus, stillbirth, anemia, congenital malformation, and congenital syphilis. This study aims to determine the magnitude and determinant factors of late ANC booking in Ethio

Knowing the entire story - a focus group study on patient experiences with chronic Lyme-associated symptoms (chronic Lyme disease).

BACKGROUND: Healthcare providers frequently struggle to provide effective care to patients with chronic Lyme-associated symptoms (chronic Lyme disease, CLD), potentially causing these patients to feel misunderstood or neglected by the healthcare system. This study is the first to use a combined medical and communication science approach, and aims to assess patients' experiences with CLD & CLD-related care, identify themes and repertories in these patients' narrations, and provide potential ways to improve communication with them. METHODS: Informed by the principles of 'clean language', we con

Web-Based Behavioral Intervention Utilizing Narrative Persuasion for HIV Prevention Among Chinese Men Who Have Sex With Men (HeHe Talks Project): Intervention Development.

BACKGROUND: In the era of potent antiretroviral therapy, a high level of condomless anal intercourse continues to drive increases in HIV incidence in recent years among men who have sex with men. Effective behavior change strategies for promoting HIV-preventive behaviors are warranted. Narrative persuasion is a novel health communication approach that has demonstrated its persuasive advantages in overcoming resistance to counterattitudinal messages. The efficacy of narrative persuasion in promoting health behavior changes has been well documented, but critical research gaps exist for its appli

Health workers' disrespectful and abusive behaviour towards women during labour and delivery: A qualitative study in Durban, South Africa.

BACKGROUND: A high prevalence of disrespectful and abusive behaviour by health workers towards women during labour and delivery has been widely described in health facilities, particularly in Africa, and is a worldwide public health concern. Such behaviours are barriers to care-seeking, and are associated with adverse outcomes for mothers and newborns. This paper reports experiences of disrespectful care among informal working women in three public health facilities in Durban, South Africa. METHODS: A qualitative longitudinal study was conducted among a cohort of informal working women recrui
