Effectiveness of a simple intervention for prevention of catheter-associated urinary tract infections in a community teaching hospital.

BACKGROUND: Catheter-associated urinary tract infection (CA-UTI) is the most common health care-associated infection. Instrumentation of the urinary tract, mainly urinary catheterization, is the most important risk factor for CA-UTI. It is believed that proper catheter management can reduce the incidence of CA-UTI. METHODS: This was a prospective preintervention and postintervention study conducted to assess the effect of an intervention aimed at preventing CA-UTI. A reminder sticker was placed on each patient's medical record binder to remind the physicians to consider discontinuing any unne…

Symptomatic candiduria in neonates, infants, and pediatric patients admitted to ICUs: epidemiology, risk factors, clinical symptoms, and outcomes of empirical antifungal therapy.

Candiduria, defined as the presence of Candida spp. in urine, is more common among hospitalized patients, particularly those admitted to int … ediatric patients. Prematurity was the most common risk factor among neonates (29, 38.15%), whereas urinary catheterization was predominant in infants (41, 53.94%) and history of bacterial urinary tract infection in pediatric patients (165, 66.53%). Hydronephrosis (176, 44%) was the most frequent ultrasound finding. Most patients had an ICU stay of 1-5 days (335, 83.75%) …

Urethral Stricture is Frequently a Morbid Condition: Incidence and Factors Associated With Complications Related to Urethral Stricture.

OBJECTIVE: To determine the frequency of complications related to urethral stricture and identify clinical factors associated with them. METHODS: Of 1851 patients with a suspected diagnosis of urethral stricture referred to a single urologist from 2005 to 2016 were retrospectively reviewed. Clinical variables included complications directly related to urethral stricture at the time of patient presentation, associated signs/symptoms, patient age, stricture length, location, and etiology. Complications considered significant were acute urinary retention or difficult catheterization requiring em

The significance of 99mtechnetium dimercapto-succinic acid renal scan in children with spina bifida during long-term followup.

PURPOSE: (99m)Technetium dimercapto-succinic acid renal scans are ideal for demonstrating renal scarring in children with spina bifida. However, doubt persists about the need for routine application. We assessed the associations among abnormal (99m)technetium dimercapto-succinic acid renal scans, vesicoureteral reflux and urodynamic findings in patients with spina bifida during long-term followup. MATERIALS AND METHODS: We retrospectively reviewed the records of 64 patients with spina bifida followed at our center. All patients were older than 10 years (mean 15.8, range 10 to 23). Dimercapto-

Acute urinary retention.

Urinary retention is most commonly caused by obstruction in men and neuromuscular dysfunction in women. A careful history, physical examination, and urethral catheterization will lead to the correct diagnosis in the majority of cases. For nonobstructive causes, a careful survey of the patient's other illnesses and medications will often lead to diagnosis. Successful treatment of nonobstructing causes can be effectively managed by the primary care physician.

Irrigation and drainage properties of three-way urethral catheters.

OBJECTIVES: To evaluate in an in vitro study the irrigation and drainage capabilities of a variety of available catheters. Three-way Foley catheters are used when irrigation of the bladder is anticipated to prevent or manage blood clots in the bladder. METHODS: Six operators applied maximal one-handed pressure irrigation with a catheter-tip syringe through the irrigation and drainage ports of the Bardex and Dover three-way catheters (16F to 26F). Maximal manual irrigation flow was recorded using a flowmeter. Continuous gravity irrigation and drainage flows were measured by suspending a 2-L ir

A modified combination technique for performing diagnostic peritoneal lavage.

Existing open diagnostic peritoneal lavage techniques (DPL) use small bore catheters, through a small lower midline incision or infra-umbilical incision. This study evaluated a modified DPL technique using a 10 mm umbilical incision, with open insertion of a 20 French peritoneal lavage catheter. The catheter has 27 radial side holes in the terminal distal 13 cm, and a female Leur lock connector. Warmed saline is infused via a cystoscopy infusion set. The time required to perform a new technique was studied in 10 patients. The median time to catheter insertion was 2.4 +/- 3.8 min, infusion time

A new endoscopic technique for non deflating Foley balloon.

OBJECTIVE: To describe a new technique to deflate impacted Foley balloon. METHOD: A total endourologic method using urethrotome sheath, ureteric catheter, stylet, sedoanalgesia needle and cold cup biopsy forceps is described. The technique involves minimal maneuverability of urethra. RESULTS: The technique is non traumatic, simple, and successful in almost all cases. This technique requires urology instruments which are easily available at any centre. CONCLUSION: The described technique is safe, effective non traumatic and can easily be used in cases of impacted Foley balloon. There is no d

Percutaneous placement of a suprapubic tube with peel away sheath introducer.

A new technique for percutaneous placement of a suprapubic tube has been developed, which allows controlled entry into the bladder over a guide wire to avoid the potential hazards of blind trocar cystotomy. A Foley style catheter can be placed, which is less likely to become dislodged than other types of percutaneous suprapubic catheters currently available.

The tricuspid valve in carcinoid syndrome. An echocardiographic study.

Carcinoid syndrome was diagnosed in a 48-year-old man by urinary 5-hydroxyindoleacetic acid test results and liver biopsy findings. Cardiac catheterization disclosed tricuspid stenosis and mild pulmonic stenosis. The distinctive echocardiographic findings included a tricuspid valve moving in unison with the annulus and thickened immobile tricuspid leaflets. Unlike in rheumatic heart disease, diastolic doming of the tricuspid leaflets was not seen.

Evaluation of urinary catheters for effective manual bladder washout.

Our purpose was to evaluate the efficiency of manual bladder washout (MBW) for bladder retention by blood clot formation using urinary catheters. Three types of 22 Fr urinary catheters, a rounded tip Foley catheter (FC) with the standard two eyes, an open-ended Nelaton catheter (NC) with a side hole, and an open-ended Foley catheter (OEFC) with a side hole closer to the tip than NC, were evaluated. An automatic irrigation device that could perform a predetermined procedure mimicking MBW under constant velocity was fabricated. The procedure using catheters and the device was performed in a pseu

[Biofilm control in urological practice].

Urinary tract infections (UTIs) have long been among the most common diseases. In the structure of the general infectious morbidity, UTIs rank second after acute respiratory viral infection. Every year, researchers note an increasing number of mutations in the genomes of bacteria that cause infectious diseases, which leads to the formation of more and more aggressive forms of pathogens. Patients with infectious diseases of the urinary system have the highest risk of biofilm formation, the frequency of which is directly proportional to the length of time the urethral catheter is located and acc

Urinary catheter management: what neurologists need to know.

Patients with neurological disorders often have lower urinary tract dysfunction, manifesting as urinary retention or urinary incontinence, and so commonly use catheters. Neurologists should therefore be aware of the different types of catheters and appliances and their risks, benefits and complications. Clean intermittent self-catheterisation is preferable to an indwelling catheter; however, if this is not possible, then a suprapubic indwelling catheter is preferable to a urethral catheter for long-term management. We review the decision-making process when selecting catheters for neurological

[Palliative long-term urinary bladder drainage: the uro-geriatric point of view].

A serious voiding disorder or urinary incontinence represent indications for long-term catheterization of the urinary bladder. Treatment by a transurethral or suprapubic bladder catheter for life-long bladder drainage is accompanied by technical short-term complications and long-term sequelae. The mortality risk associated with inserting a suprapubic catheter is approximately 2% due to an incorrect bladder puncture. Long-term consequences of life-long bladder drainage are hematuria, infection, spasms, stone formation, obstruction, catheter loss, risk of falls and automanipulation especially in
