Anatomical outcome and visual recovery after pneumatic retinopexy: Analysis of predictive factors in a cohort including non-standard criteria.

BACKGROUND AND OBJECTIVE: Pneumatic retinopexy is a treatment for rhegmatogenous retinal detachment with standard selection criteria. There is growing interest in evaluating its outcomes in broader clinical profiles. This study aimed to evaluate anatomical outcomes and visual recovery after pneumatic retinopexy in a cohort including non-standard criteria, and to identify independent predictive factors for anatomical failure and reduced visual recovery. MATERIALS AND METHODS: A prospective cohort study of 92 eyes (92 patients), stratified into standard (n = 57) and non-standard (n = 35) criter

Intercostal Nerve Cryoablation as an Adjunct to Surgical Stabilization of Rib Fractures: A Systematic Review, Meta-Analysis, and GRADE Assessment.

INTRODUCTION: Rib fractures are common in trauma patients and are associated with significant morbidity due to pain-related respiratory compromise. Surgical stabilization of rib fractures (SSRF) improves outcomes in select cases, but postoperative pain can persist despite surgery. Intercostal nerve cryoablation (INCA) has emerged as a potential adjunct to enhance analgesia and recovery. This systematic review and meta-analysis evaluated postoperative outcomes associated with the use of INCA in conjunction with SSRF in adult patients with rib fractures. METHODS: We conducted a systematic searc

Stage 1 Registered Report:  Rationale, design and study protocol for a prospective, exploratory study Predicting Response to image-Guided Cryoneurolysis of the Greater Occipital Nerve (CryoGON) in Chronic Migraine Using a Conventional Greater Occipital Nerve Block.

BACKGROUND: Chronic migraine is frequently refractory to available preventive therapies. Image-guided cryoneurolysis of the greater occipital nerve (CryoGON) may provide longer-lasting relief than conventional greater occipital nerve (GON) blocks. This Stage 1 Registered Report describes a prospective, exploratory study evaluating whether response to a conventional GON block predicts subsequent response to CryoGON, and assesses open-label safety, tolerability, feasibility and efficacy of CryoGON. METHODS: The study is planned as a single-site, investigator-initiated, open-label clinical inves

Transbronchial lung cryobiopsy and surgical lung biopsy for interstitial lung diseases: a cost-effectiveness analysis.

OBJECTIVE: Diagnosis of interstitial lung diseases (ILDs) relies on multidisciplinary discussion. However, when uncertainty persists, histopathological evaluation is required. Surgical lung biopsy (SLB) offers high diagnostic yield but carries relevant morbidity and non-negligible mortality. Although transbronchial lung cryobiopsy (TBLC) has emerged as a less invasive alternative, its economic impact on middle-income countries remains underexplored. This study sought to evaluate the cost-effectiveness of TBLC in comparison with that of SLB for the diagnosis of ILDs from the perspective of the

Eosinophilic Bronchiolitis Diagnosed by a Transbronchial Lung Cryobiopsy and Successfully Treated with Biologic Agents: A Report of Two Cases and Literature Review.

Eosinophilic bronchiolitis (EB) is a rare airway disease characterized by eosinophilic bronchiolar inflammation. We herein report two cases of EB diagnosed by a transbronchial lung cryobiopsy (TBLC), a less invasive diagnostic procedure than a surgical biopsy. Two patients with a history of refractory asthma responded to treatment with oral corticosteroids and biologic agents. To our knowledge, these are among the first reported cases of EB that were successfully managed with dupilumab and benralizumab. These cases underscore the diagnostic utility of TBLCs and suggest the potential use of bio

Natural History and Outcome of Cryoablation for Breast B3 Fibroepithelial Lesion: A Prospectively Reported Case and Literature Review.

The use of cryoablation for B3 fibroepithelial lesions (FELs) of the breast has not been well-documented. Cryoablation offers several advantages over surgery in treating breast masses, including avoidance of general anesthesia, less invasiveness, shorter recovery, and greater cost-effectiveness. This prospective study reported the outcome of a patient with a breast B3 FEL treated with cryoablation. Post-procedure, the patient reported low pain scores, early return to work, and good cosmesis without complications. The mass also reduced in size, demonstrating that cryoablation could be a feasibl

Long-term Oncological Outcomes of Partial Gland Cryoablation of Localized Prostate Cancer with Median Follow-up of 7 Years: A Single-institution Experience.

BACKGROUND AND OBJECTIVE: Traditional radical approaches to treat clinically localized prostate cancer (PCa) have largely been effective oncologically, but associated with complications and decline in functional outcomes, arguably making them unsuitable in all cases for treatment. This necessitates an alternative option, focal therapy (FT), which has good oncological control while reducing treatment-related functional detriments. Although midterm oncological and functional outcomes of FT have been demonstrated, the long-term outcomes were lacking. We sought to report the long-term oncological

Monomorphic Ventricular Tachycardia Secondary to an Epicardial Pacing Lead.

Epicardial pacing leads are frequently used in small children and in complex congenital heart disease that precludes transvenous lead placement. While lead failure is not uncommon from epicardial pacing leads, proarrhythmic events are rare. We present an eight-year-old patient who developed spontaneous nonsustained ventricular tachycardia from a chronic unipolar epicardial pacing lead. Removal of the epicardial lead with cryoablation around the lead body resolved the ventricular tachycardia. To our knowledge, this is the first case report of a suture on epicardial pacing lead causing late vent

Inflammation and myocardial injury after pulmonary vein isolation with distinct pulsed field ablation modalities: Comparison with thermal ablation.

BACKGROUND: Pulmonary vein isolation (PVI) was performed using thermal ablation like radiofrequency catheter ablation (RFCA) and cryoballoon ablation (CBA). Pulsed field ablation (PFA) is a novel nonthermal technique that selectively ablates myocardial tissue with minimal collateral damage. However, data comparing post-ablation inflammatory responses among these modalities are limited. METHODS: Paroxysmal atrial fibrillation (PAF) patients undergoing PVI alone with RFCA, CBA, or PFA were enrolled. The PFA group was subdivided into Farapulse and PulseSelect. Inflammatory markers including whit

Coupled thermal-hemodynamics computational modeling of cryoballoon ablation for pulmonary vein isolation.

Cryoballoon ablation (CBA) is a cryo-energy based minimally invasive treatment procedure for patients suffering from left atrial (LA) fibrillation. Although this technique has proved to be effective, it is prone to reoccurrences and some serious thermal complications. Also, the factors affecting thermal distribution at the pulmonary vein-antrum junction that are critical to the treatment success is poorly understood. Computer modeling of CBA can resolve this issue and help understand the factors affecting this treatment. To do so, however, numerical challenges associated with the simulation of
