Transient osteochondrosis of the capitellum in a child after humeral supracondylar fracture: a case report.

Supracondylar fracture of the humerus is the most frequently encountered fracture around the elbow in children. Avascular necrosis is an uncommon complication after supracondylar fracture, but fishtail deformity after avascular necrosis of the trochlea is well known. However, transient osteochondrosis of the capitellum after a supracondylar fracture has not been previously reported in the English literature except 1 case abstract. The authors report their experience of transient osteochondrosis of the capitellum in a 3-year-old boy after the closed reduction and percutaneous pinning for a disp

Stabilisation of the fractured fibula plays an important role in the treatment of pilon fractures: a retrospective comparison of fibular fixation methods.

Ninety-eight pilon fractures associated with ipsilateral distal fibular fracture were included in this study. The pilon fractures were treated by open reduction and plating. The 98 fractures were divided into three groups based on the treatment method of fractured fibula. Group A was composed of 50 fibular fractures treated by open reduction and plate fixation. Group B was composed of 23 fibular fractures treated by open reduction and pin fixation. Group C was composed of 25 fibular fractures treated conservatively by closed reduction. The radiographs were reviewed for adequacy of fracture red

Treatment of fracture-dislocation of the proximal interphalangeal joint using extension-block Kirschner wire.

Fourteen cases of fracture-dislocation of the proximal interphalangeal joint (PIPJ) were treated using extension-block Kirschner (K)-wire after closed or open reduction of the dislocation. The postoperative ROM of the PIPJ in the 14 patients averaged 94.4 degrees. A full ROM of the PIPJ was regained in 10 cases. This wire provides an alternative method to the use of extension block splintage in the management of such injuries.

Triquetral fracture associated with hamate dislocation: a case report.

A case of hamate dislocation associated with triquetral fracture is presented. No other carpal or metacarpal injury was involved. The mechanism of injury was volar flexion and ulnar deviation. An attempt at closed treatment failed because the proximal tip of the hamate was incarcerated between the triquetral fragments. Open reduction, K-wire fixation, and casting were done. The patient was able to use her hand nearly normally 12 weeks postoperatively.

Evaluation of Treatment of Low-energy Distal Radial Fractures in Postmenopausal Women.

BACKGROUND: Distal radial fractures are one of the most common injuries seen in traumatology. Most are treated conservatively with closed reduction and immobilisation in a plaster cast. However, this method does not always allow for achieving and maintaining normal fracture reduction in unstable fractures. The aim of this study was assess clinical and radiographic outcomes of treatment of distal radial fractures depending on the method used. MATERIAL AND METHODS: A total of 77 postmenopausal female subjects were divided into three subgroups, depending on the treatment methods: 1. closed reduc

Median Nerve Entrapment After Intramedullary Nailing of a Pediatric Both-Bone Forearm Fracture: A Rare Case Report.

CASE: We present a case of a 10-year-old boy with a distal third both-bone forearm fracture, who developed delayed median nerve dysfunction following closed reduction and intramedullary nailing. The child later developed a nonhealing ulcer and sensory loss over the middle finger. Surgical exploration confirmed the nerve trapped within the callus; neurolysis and primary repair were performed. The patient showed progressive neurological recovery and complete sensory and motor restoration at 1-year after repair. CONCLUSION: Median nerve entrapment should be considered in cases of persistent pain

Double Dislocation of Proximal and Distal Interphalangeal Joints With Volar Plate Avulsion Fracture in the Fifth Digit of an Elderly Female: Successful Conservative Management.

A 77-year-old, right-hand-dominant woman presented after a low-energy fall with dorsal displacement involving the proximal and distal interphalangeal joints of the right small finger, along with a volar plate avulsion fracture at the base of the middle phalanx. Closed reduction was performed without anesthesia, followed by static aluminum splinting and occupational therapy. At her two-month follow-up, she reported minimal pain, improved function, and maintained joint alignment. This case highlights successful conservative management of a rare double interphalangeal dislocation with associated

Pathological Distal Tibial and Fibular Fracture in a Paediatric Patient: A Case Report.

Paediatric distal tibial and fibular fractures are seen quite regularly in orthopaedic trauma practice. Most patients are managed conservatively with closed reduction or casting while only a selected few required surgical treatment. Surgical options include plating, percutaneous Kirschner wires, rigid intramedullary nails, and flexible intramedullary nailing. This is dependent upon the patient's age, fracture site, comminution, and concomitant injuries. Here, we present an interesting case of a patient with an unusual lesion seen at the fracture site. This lesion was curetted out during surger

Brachial Plexus Injury in a 6-Year-Old Boy with 100% Displaced Proximal Humeral Metaphyseal Fracture: A Case Presentation.

UNLABELLED: Posttraumatic brachial plexopathies can occur following displaced proximal humeral fractures, causing profound functional deficits. Described here is an unusual case of a displaced proximal humeral metaphyseal fracture in a young child. The patient underwent closed reduction and serial casting, but hand weakness and forearm sensory loss persisted. Needle electromyography localized the injury to the mid/proximal arm near the fracture site, resulting in damage to the posterior and medial cords of the brachial plexus with profound involvement of the radial, ulnar, and median nerves an

[Elbow dislocation and lateral epicondyle fracture in a five year-old girl. Case report].

Traumatic elbow dislocation in the pediatric population is a particularly unusual injury. It was first described by Stimson in 1900 and almost 100 years later revisited by Tachdjian in 1990. Three percent of cases are associated with lateral epicondyle fracture, so this is an infrequent injury that has been described in only a few papers as case reports. The mechanism of injury is not clearly known, nor is the best type of treatment or its complications. We report herein the case of a five year-old girl with fracture dislocation of the lateral epicondyle who was managed with closed reduction a

Posterior atlantoaxial dislocation complicating odontoid fracture without neurologic deficit: a case report and review of the literature.

Traumatic posterior atlantoaxial dislocation associated with odontoid fracture is extremely rare, with only eight cases reported thus far in the English literature. This report concerns a 47-year-old female who presented with considerable pain and stiffness in the neck without a neurologic deficit after injury due to a fall. Radiographs, computed tomography, and magnetic resonance imaging demonstrated a posterior dislocation of the atlas with respect to the axis with an odontoid fracture. No cord compression or intramedullary cord signal abnormalities were detected at the level of the atlantoa

Isolated dorsal radiocarpal dislocation: case report and literature review.

A 35-year-old woman sustained an injury to her right wrist that caused a dorsal radiocarpal dislocation with an avulsion fracture of the ulnar styloid process without radial fracture, but with a dorsiflexed intercalated segment instability (DISI) pattern of the carpus. Treatment consisted of debridement, exploration of the wound, closed reduction of the dislocation, and immobilization for 4 weeks. Six months later, she had recovered satisfactorily. Eighteen months after the injury, radiographs of both wrists showed bilateral DISI deformities of the carpus, and it is thought that the DISI patte

Displaced fracture of the lumbosacral spine with delayed cauda equina deficit: report of a case and review of literature.

A 41-year-old man with a fracture of the upper sacrum and forward and downward displacement of the superior sacral fragment and upper spine on the lower sacrum, developed partial deficit of cauda equina function. Similar fractures rarely have been reported. Signs of lumbar pain, contusion, and fractured transverse processes should lead the surgeon to carefully examine the radiologic fractures of the area of injury; otherwise, this lesion may heal unrecognized. With the spine in flexion, the fracture may create mechanical instability. Early closed or open reduction is theoretically ideal. Surgi

Intramedullary nailing using K-wires for high-energy distal humeral metaphyseal-diaphyseal fractures accompanying radial nerve palsy in a 2-year-old toddler: A case report.

RATIONALE: Pediatric distal humeral diaphyseal fractures are rare and challenging to manage due to anatomical and biomechanical complexities. In addition, humeral shaft fractures in adults are often accompanied by radial nerve paralysis (RNP), but there are no studies on the incidence or treatment of pediatric humeral shaft fractures accompanied by RNP in toddlers. The authors present the outcomes of closed reduction and intramedullary nailing using Kirschner wires (K-wires) for high-energy distal humeral metaphyseal-diaphyseal open fractures accompanied by RNP in a toddler. Four quadrant parallel peripheral screw fixation for displaced femoral neck fractures in elderly patients.

BACKGROUND: The treatment options for displaced femoral neck fracture in elderly are screw fixation, hemiarthroplasty and total hip arthroplasty based primarily on age of the patient. The issues in screw fixation are ideal patient selection, optimal number of screws, optimal screw configuration and positioning inside the head and neck of femur. The problems of screw fixation may be loss of fixation, joint penetration, avascular necrosis of femoral head, nonunion, prolonged rehabilitation period and the need for second surgery in failed cases. We hereby present results of a modified screw fixat

Case Report: Traumatic obturator hip dislocation with subtrochanteric fracture in an 8-year-old child.

BACKGROUND: Obturator-type hip dislocation is a rare condition in pediatric patients, with the simultaneous occurrence of an ipsilateral subtrochanteric femoral fracture being exceptionally uncommon. Although various treatment methods have been described for hip dislocation or subtrochanteric femoral fractures, managing these injuries remains challenging due to the potential risk of femoral head avascular necrosis and concerns about fixation stability. CASE PRESENTATION: An 8-year-old boy sustained a traumatic obturator hip dislocation and an ipsilateral subtrochanteric fracture following a f

Primary prosthetic replacement in per- and intertrochanteric fractures.

PURPOSE: Hip arthroplasty is rarely indicated in the treatment of per- and intertrochanteric femur fractures. Although the majority of fractures are amenable to closed- or open reduction and internal fixation (CRIF/ORIF), in some patients the complexity of the fracture or other patient-related factors may cause the orthopaedic surgeon to consider arthroplasty as the treatment of choice. Decision-making is challenging, and a reliable score has not yet been established. RESULTS: Reviewing literature, several predictors of inferior outcome after CRIF/ORIF in per- and intertrochanteric fractures

Modified Nicoll's Graft for Treatment of Gap Non-union of Ulna: A Rare Case Report.

INTRODUCTION: Clavicle fractures are common injuries treated by orthopedic surgeons, with most injuries managed nonoperatively. Operative fixation of clavicle fractures is indicated in specific clinical scenarios such as open injuries, ipsilateral shoulder trauma, or fractures with associated neurovasculature compromise. Operative fixation is not widely accepted for closed injuries and is typically reserved for instances of failed closed treatment with resultant nonunion or delayed union. Among the complications associated with clavicle fractures, pneumothorax has not been commonly reported. W

Treatment of type II odontoid fracture with a novel technique: Titanium cable-dragged reduction and cantilever-beam internal fixation.

Surgical methods for type II odontoid fracture can be classified into 2 main groups: anterior or posterior approach. A more effective way to achieve bone fusion with the lowest possible surgical risk is needed. Therefore, the aim of our study was to describe and evaluate a novel technique, cable-dragged reduction/cantilever beam internal fixation for the treatment of type II odontoid fracture. This was a retrospective study enrolled 34 patients underwent posterior cable-dragged reduction/cantilever-beam internal fixation surgery. Purposeful Closed Reduction and Pinning in Unstable Slipped Capital Femoral Epiphysis Results in a Rate of Avascular Necrosis Comparable to the Literature Mean.

The standard treatment of stable slipped capital femoral epiphysis (SCFE) is generally accepted to be in situ pinning. Controversy exists regarding the treatment of unstable SCFE, including the role of a purposeful closed reduction or open reduction. The objective of this study was to investigate the rate of avascular necrosis (AVN) with purposeful closed reduction and in situ pinning of unstable SCFE. The authors retrospectively reviewed 221 patients with 302 SCFE hips treated with in situ pinning between 2000 and 2014. Forty-eight patients (50 hips) presented with an unstable SCFE. All unsta
