PURPOSE: We studied the effects of experimental neurolysis on ectopic firing in a rat chronic constriction nerve injury (CCI) model.
METHODS: Sixteen Wistar rats were used as CCI models; 8 in a neurolysis group and 8 in a sham operation group. Eight additional Wistar rats were used in a normal control group. The CCI model was created by loosely ligating the sciatic nerve with 4-0 chromic gut sutures. Seven days after surgery the same portion of the sciatic nerve, now embedded in scar tissue, was exposed. For the neurolysis group all sutures and scar tissue around the nerve were removed, and f

BACKGROUND: Cranial nerve abnormalities occur frequently in both focal and diffuse neurologic disorders and can be evaluated by electrophysiological techniques available in most clinical neurophysiology laboratories.
REVIEW SUMMARY: The optic nerve is evaluated by visual evoked potentials. Measurements of latency, amplitude, and waveform morphology are especially useful in detecting demyelinating lesions. Brain stem auditory evoked potentials evaluate the auditory portion of the eighth cranial nerve. Using an auditory stimulus, a number of waveforms are generated, and changes in the normal pa

Hereditary neuropathy with liability to pressure palsies (HNPP) is an autosomal-dominant, slowly progressive neuromuscular disorder, which is characterized by recurrent acute peripheral nerve palsies. Electrophysiological studies show decreased motor and sensory conduction velocities in both clinically affected and unaffected nerves. Focal thickening of myelin sheath with sausage-like formation, also called tomacula, is seen in nerve biopsies. In genetic studies, 1.5-Mb deletion on chromosome 17p11.2 is detected in approximately 85% of HNPP cases and point mutations are determined in some case

The effects of endothelin-1 were studied in vitro on neurons in the hamster submandibular ganglion, using the intracellular microelectrode technique. Endothelin (1 microM) caused a depolarization (5.5 +/- 1.2 mV) followed by a hyperpolarization (8.5 +/- 2.8 mV) of the membrane potential. Membrane conductance was increased during the endothelin-induced depolarization and was decreased during the endothelin-induced hyperpolarization. The endothelin-induced depolarization was depressed (mean 43.6%) in a Krebs solution containing zero calcium and high magnesium. The results suggested that the pred

Increased muscle extensibility and passivity characterize infantile hypotonia. It may reveal a peripheral neuromuscular disease as well as a disorder of the central nervous system. Electrodiagnostic studies in newborn and young infants are useful to guide the indication of other complementary investigations. Signs of denervation on needle electromyography strongly suggest infantile spinal muscular atrophy. Electrodiagnostic findings can distinguish rare conditions mimicking spinal muscular atrophy that are obstetrical tetraplegia and severe congenital neuropathies. Nerve conduction velocities

The purpose of the present review is to assess the evidence published in scientific literature that industrial organic solvents as a generic group have the ability to induce long-term nervous system damage in workers that can be detected by techniques other than neuropsychological testing. The main body of evidence considered in this review was 40 studies involving the use of brain imaging, neurophysiological testing, gross autopsy or histopathology in groups of workers with long-term solvent exposure. Case reports involving both solvent abuse and occupational exposure, and experimental animal

MAIN PROBLEM: Nucleus pulposus and/or chronic compression can induce spinal nerve root injury. Inflammation has been proposed as having major importance in the pathophysiologic mechanisms involved in the induction of such injuries. Corticosteroids, potent anti-inflammatory drugs, have been demonstrated to reduce nucleus pulposus-induced spinal nerve root injury. The aim of the present study was to assess the effects of two potent non-steroidal anti-inflammatory drugs (NSAIDs), diclofenac and ketoprofen, in experimental nucleus pulposus-induced spinal nerve root injury in a pig model.

The anatomy, pathophysiology, and clinical evaluation of radiculopathies are discussed. Defining whether root injury is present and which roots are involved can be difficult but critical for patient management. In conjunction with clinical and radiological information, studies that establish physiological abnormalities of roots should be helpful and important. Clinical neurophysiological studies for radiculopathies are performed frequently but have yet to achieve a universally accepted role in the evaluation of these patients. Electrophysiological techniques for the evaluation of radiculopathi

Neuromuscular blocking agents are routinely used as an adjunct therapy for critically ill patients. Unlike many neuromuscular blocking agents, vecuronium does not cause significant histamine release, which may lead to bronchoconstriction. Due to a short duration of action and limited accumulation, vecuronium has been widely used. Prolonged ventilatory dependence due to persistent neuromuscular blockade has been reported in patients treated with vecuronium. We report a case of an 8-year-old girl who had a primarily motor axonopathy presenting with weakness after extended vecuronium administrati

This study investigated where leprosy affects the posterior tibial nerve and whether neurolysis is beneficial. Nine patients with bilateral posterior tibial leprous neuropathy with no sensorimotor recovery were studied. Preoperative sensory-muscle and nerve conduction velocity testing revealed the tarsal tunnel to be the site of a severe lesion in all cases. During surgery, the most proximal site of the nerve lesion was detected by electrically stimulating the spinal roots from the second lumbar nerve to the fourth sacral nerve, evoking efferent mixed nerve compound action potentials that were

Peripheral nerve regeneration after traumatic injury and standard repair with a nerve autograft is usually incomplete. This study tested the influence of graft vascularity and pharmacological intervention with GM-1 ganglioside on nerve regeneration in a rat sciatic nerve model. Controls included an unoperated contralateral side and sham-operated groups either with or without the GM-1. During the 5 months of recovery, locomotion was tested by the sciatic function index (SFI). At killing, anesthetized animals were prepared for nerve conduction velocity (NCV) studies, followed by the wet weight o

AIMS: The effect of local anaesthetics on optic nerve function can be investigated by quantifying the relative afferent pupillary defect (RAPD).
METHODS: The study compared the depth of induced RAPD following posterior sub-Tenon's, retrobulbar, and peribulbar local anaesthetics using crossed polarising filters before cataract surgery (time 1 = 5 minutes), immediately after surgery (time 2 = 42 minutes (av)), and once again on the ward (time 3 = 107 minutes (av)).
RESULTS: All patients developed a RAPD. There was no significant difference in the depth of RAPD between the groups at any one tim

In motor nerve conduction studies we sometimes encounter a small initial positive deflection (IPD) of the compound muscle action potential (CMAP). This potential represents a volume conduction from nearby muscles other than the objective muscle. We demonstrated recordings of motor nerve conduction studies from two patients with carpal tunnel syndrome (CTS). In patients with CTS IPDs can be recorded from a surface electrode above the abductor pollicis brevis when intense stimuli to the median nerve provoked a stimulus spread to the ulnar nerve. However, without this stimulus spread to the ulnar

The finding of conduction block (CB) within short consecutive segments along a motor nerve is a key feature of multifocal motor neuropathy (MMN). Despite their different pathogenesis, this may be the only clinical difference between some cases of MMN and the pure spinal muscular atrophy form of motor neuron disease (MND). In 12 patients with distal atrophy and fasciculations and electrophysiological evidence of CBs in the upper limbs, we measured the peripheral and central motor conduction times (PMCT and CMCT) to hand muscles. We reasoned that patients with MMN should show an abnormally prolo

A 15-year-old male developed symmetrical weakness of the limb muscles. He had not had any previous developmental disorders except delayed initiation of walking. Flexion contraction of ankle joint and pes cavus deformity were seen. The cerebrospinal fluid protein concentration was elevated. Nerve conduction study showed severe conduction block and temporal dispersion. A sural nerve biopsy revealed remarkable onion bulb-like changes and perivascular infiltration of inflammatory cells. After high-dose corticosteroid treatment, he showed improvement in muscle strength. Although there were no abnor

Normal flexion of the fingers involves an involuntary contraction of the pronator and lumbrical muscle. In individuals whose profession involves constant flexing of the fingers those muscles become hypertrophied, impinging on the carpal tunnel. The narrowing of the carpal tunnel yields well to ultrasonography. The objective of this investigation was to find an ultrasonographic index of occupational carpal tunnel syndrome. Thus "Index M" denotes the variation obtained in the "M Space" before and after flexion-extension of the fingers. The study included 45 subjects performing tasks which involv

A 58-year-old woman developed painful, bilateral ulnar neuropathy in conjunction with small cell lung carcinoma and high serum titer of anti-Hu antibody. An incidental stage I plasma cell dyscrasia, with immunoglobulin G kappa monoclonal protein, was also present. Electropysiological assessment excluded a generalized neuropathy, and nerve biopsy showed marked loss of myelinated and small unmyelinated fibers, without inflammatory changes or amyloid deposition. High titers of circulating anti-Hu antibody can be associated with symptoms resembling a paraneoplastic mononeuropathy.
