Management of profound zonular dialysis or weakness with a new endocapsular ring designed for scleral fixation.

The endocapsular tension ring has helped in the management of patients with moderate loss of zonular support. However, the eye with profound zonular dialysis or weakness may require scleral fixation of the ring for centration and long-term stabilization. We used a new, modified endocapsular tension ring designed to provide scleral support without violating the integrity of the capsular bag in 4 patients. All patients had extreme loss of zonular support preoperatively or a significant risk of progressive zonular weakness. In each case, the new ring provided excellent support and centration of t

Revision Strategies for Complications and Failure of Trapeziometacarpal Joint Arthroplasty Based on Residual Trapezial Bone Stock.

PURPOSE: The main reasons for revision surgery after primary trapeziometacarpal (TM) joint prosthetic implantation are trapezium loosening and instability. Although secondary trapeziectomy is the gold standard, here we describe alternative surgical procedures that can be integrated in a revision strategy based on residual trapezial bone stock after implant removal. METHODS: This single-center retrospective study analyzed the clinical and radiologic results for 41 patients who underwent surgical TM joint prothesis revision from 2006 to 2021 at our institution. We applied a classification of tr

3D printing technology-assisted total hip arthroplasty for acute proximal femoral fracture and Hartofilakidis type II developmental dysplasia of the hip: a case report and literature review.

Developmental dysplasia of the hip (DDH) associated with an acute proximal femoral fracture on the same side is uncommon, and performing a one-stage total hip arthroplasty (THA) with concurrent fracture stabilization in such cases poses significant technical challenges. We present a case involving a 57-year-old female with a long history of right-sided DDH who was involved in a vehicular accident, leading to acute pain in her right hip, shortening of the limb, and restricted movement. Imaging studies indicated Hartofilakidis type II DDH along with a comminuted intertrochanteric fracture of the

Bifocal Radial Fracture/Dislocation and Distal Ulnar Fracture-A Rare Case of Proximal Forearm Instability Not Yet Classified and Literature Review.

INTRODUCTION: Monteggia fractures, first described by Giovanni Battista Monteggia, involve a fracture of the proximal ulna with anterior dislocation of the radial head. Bado's 1967 classification divides these injuries into four types. Rare mixed patterns exist, overlapping with other forearm injuries such as Galeazzi and Essex-Lopresti lesions. These complex fractures/dislocations pose significant diagnostic and therapeutic challenges and are not adequately represented in current classification systems. Methods and Case Presentation: We report the case of a 56-year-old woman with a complex fo

Management of Hip Arthroplasty for Intertrochanteric Fractures Under the Treatment Concept of Periprosthetic Fractures.

OBJECTIVE: Intertrochanteric fractures (IF) in the elderly are often complicated by osteoporosis and high rates of fixation failure. Current treatment options have limitations in providing both stable fixation and early mobilization in this fragile population. This study aimed to introduce and evaluate a novel approach, the periprosthetic femoral fracture treatment concept (PFFtc), as a surgical strategy to guide hip arthroplasty in elderly IF patients. METHODS: A retrospective analysis was conducted on 209 elderly patients (mean age: 81.6 years; range: 70-93) with IF who underwent hip arthro

Partial femoral head replacement: a new innovative surgical technique for precise minimally invasive treatment of association research circulation osseous stage III femoral head necrosis.

BACKGROUND: Association Research Circulation Osseous (ARCO) stage III is the last chance for joint preservation in patients with osteonecrosis of the femoral head (ONFH). As the femoral head has collapsed at this stage, existing hip-preserving surgical treatments show limited success. We aimed to introduce a new surgical technique called partial femoral head replacement for the treatment of ARCO III femoral head necrosis and to evaluate its safety and efficacy. METHODS: Between November 2022 and December 2022, 11 symptomatic hips from 11 patients were enrolled in this retrospective study. The

The application of 3D-printed auto-stable artificial vertebral body in en bloc resection and reconstruction of thoracolumbar metastases.

BACKGROUND: Nerve compression symptoms and spinal instability, resulting from spinal metastases, significantly impact the quality of life for patients. A 3D-printed vertebral body is considered an effective approach to reconstruct bone defects following en bloc resection of spinal tumors. The advantage of this method lies in its customized shape and innermost porous structure, which promotes bone ingrowth and leads to reduced postoperative complications. OBJECTIVE: The purpose of this study is to assess the effectiveness of 3D-printed auto-stable artificial vertebrae in the en bloc resection
