Treatment of the atrophic maxilla using an onlay bone-grafting technique in combination with simultaneous insertion of endosteal implants (Branemark System) was performed in 17 patients. They were followed during a 3-year period. At the end of this period, 14 patients had stable bridges in use and one patient had an overdenture. Two patients had had to return to conventional dentures due to implant losses. The survival rate of the implants after 3 years use was 75%. It is concluded that, in spite of the implant losses, onlay bone grafting to the maxilla is a valuable method in cases of severe

This article reports on the efficacy of a technique involving expansion of edentulous ridges of less than 3 mm in width by means of a planned green stick fracture of the labial plate and simultaneous implant placement without the need for membranes. This retrospective case series includes long-term results from 21 patients with 36 sites and 37 implants with a mean follow-up of 4 years, 5 months from the date of restoration. Freeze-dried bone allograft was used in 22 sites (61%) to augment the ridge. Reentry at stage-two surgery confirmed the preservation of the displaced labial plate after imp

Several regenerative therapies have been used for maxillary sinus grafting. However, recent advances in modern bone tissue engineering techniques have been evaluated. The aim of this histologic report was to evaluate the bone obtained by a culture of autogenous osteoblasts seeded on polyglycolic-polylactid scaffolds in maxillary sinus augmentation. A 56-year-old partially edentulous male with severe atrophy of the posterior maxilla received 6 polyglycolid-polylactid disks (8 mm diameter × 2 mm depth, Oral Bone), each carrying 1.5 million autogenous osteoblasts into the depth of the sinus cavit

A 68-year-old woman was treated with an autogenous particulated bone graft from the anterior part of the mandible to elevate the right maxillary sinus floor, which was next to the alveolar ridge of an edentulous area, to facilitate dental implant placement. A rigid plate for anchorage was placed into the zygomatic bone. The maxillary right canine and the premolars were moved distally 6 months after the implant was placed and osteointegration of the bone graft had occurred. The Class II relationship was corrected. After tooth movement, the patient underwent multislice computed tomography to det

OBJECTIVES: Up to now the effect of bone-substitute materials on de novo bone formation has been tested in a variety of preclinical animal models. We hypothesized that there is no significant difference in bone regeneration after application of autogenous bone (AB) and bone substitutes in a porcine calvarial monocortical defect model and in human maxillary sinus.
STUDY DESIGN: Twenty-four weeks after application of ss-tricalcium phosphate (betaTCP), hydroxyapatite (HA), and AB in each of 3 defects on the sculls of 6 adult pigs (N = 54) and the application in 44 sinus floor elevations (11x bet

In a 40-year-old woman, a central incisor was to be replaced with an implant-supported crown. When the implant was uncovered, the soft tissue around the implant showed an unfavorable profile: The adjacent lateral incisor showed significant loss of periodontal attachment. Subsequent soft tissue and connective tissue grafts failed to correct the problem, and an alternative treatment plan was formulated. Errors in the initial treatment, including incorrect implant position and failure to analyze the cause of the periodontal attachment loss are discussed in detail; the options for further treatmen

Specific anatomic and physiologic differences between the maxillary and mandibular arches have presented challenges in restoring the fully or partially edentulous maxilla. These differences include the quality and quantity of available bone at the potential implant site. To address the limitations of the maxillary arch, attempts have been made during the last two decades to improve implant anchorage in this area. Bone grafting techniques and guided bone regeneration have been utilized to augment and improve the maxillary arch. The learning objective of this article is to review the important a

This study presents the results from ten consecutive patients who, because of insufficient bone volume for conventional implant placement in the maxilla, were treated with an interpositional bone graft and Le Fort I osteotomy. The endosteal implants were placed six months after the osteotomy. A total of 60 screw-shaped titanium implants (Brånemark) were placed, of which three failed to integrate during the six-month healing period. No further implants were lost during the follow-up period, ranging from 15 to 39 months after placement of the implants. All patients received fixed bridges and all

Insufficient alveolar height is one of the most common problems in oral implantation, and it may preclude placement of an implant or compromise the final aesthetic outcome of the restoration. To solve this problem, distraction implants (DIs) have been introduced because they can fulfill the functions of bony augmentation and implantation simultaneously and facilitate the operation, minimise the trauma, and shorten the duration of treatment. However, the high risk of complications such as device fracture from uneven distribution of stress or transport bone resorption from insufficient blood sup
