Demonstration of reversible complete left bundle branch block following tachycardia-induced cardiomyopathy.

A 56-year-old man with atrial fibrillation and complete left bundle branch block (CLBBB) developed heart failure refractory to the initial medical treatment. Both the CLBBB and cardiac dysfunction completely recovered only with an advanced medical regimen for rate control and heart failure. This report describes a case with reversible CLBBB following tachycardia-induced cardiomyopathy, who was not a candidate for cardiac resynchronization therapy.

Left Bundle Branch Area Pacing From a Femoral Approach in a Patient Without Superior Access.

Limited venous access and lateral left ventricular scar are impediments to traditional cardiac resynchronization therapy. We present a case where placement of an implantable cardioverter-defibrillator from a femoral approach while using left bundle branch area pacing led to clinical improvement. (Level of Difficulty: Intermediate.).

A case report on middle cerebral artery aneurysm treated by rapid ventricular pacing: A CARE compliant case report.

RATIONALE: Cerebral aneurysm is a common cause of intracranial hemorrhage, stroke, and death. It is treated with vascular surgeries, such as coil embolism and artery clipping. However, surgery itself is a risk factor that may cause rupture of aneurysm, and leads to irreversible brain damage, and even death. Rapid ventricular pacing (RVP) is a procedure that temporarily lowers blood pressure by increasing heart rate and reducing ventricular filling time. RVP has been widely used to reduce blood vessel tension in many cardiovascular surgeries. PATIENT CONCERNS: A 46-year-old man came to our hos

Dilated phase of hypertrophic cardiomyopathy caused by Fabry disease with atrial flutter and ventricular tachycardia.

We describe a case of a 60-year-old male with dilated phase of hypertrophic cardiomyopathy caused by Fabry disease. He was diagnosed to have a cardiac variant of Fabry disease by an enzyme assay and a right ventricular endomyocardial biopsy which revealed specific features of this disease and cardiac involvement was the sole manifestation. He has developed dilated cardiomyopathy with sustained atrial flutter and frequent non-sustained ventricular tachycardia requiring isthmus ablation and cardiac resynchronization therapy with defibrillator.

Incidence of ventricular arrhythmias after biventricular defibrillator replacement: impact on safety of downgrading from CRT-D to CRT-P.

BACKGROUND: Cardiac resynchronization therapy (CRT) reduces mortality and hospitalizations. It is debated whether CRT alone (CRT-P) or CRT plus defibrillator (CRT-D) is preferable, and still guidelines are not exhaustive. The aim of the study was to investigate whether to implant CRT-P or CRT-D in CRT-D patients who did not experience malignant arrhythmias at the moment of replacement. METHODS: Out of 451 heart failure patients undergoing CRT-D according to guidelines, 103 (67±10 years, 80% men) underwent device replacement with CRT-D. Every 6 months patients underwent to clinical evaluation …

Conduction system pacing in the Italian clinical practice: results of a nationwide survey.

BACKGROUND: Conduction system pacing (CSP) is an increasingly used approach for physiological ventricular pacing. We conducted a nationwide survey aimed to evaluate the adoption of CSP in the Italian clinical practice. METHODS: An online survey consisting of 20 questions, distributed to Italian centres performing cardiac pacing, was conducted from October to November 2024. RESULTS: A total of 136 centres took part in the survey (37% of all arrhythmia centres operating in Italy), 119 of which (86%) declared to perform CSP implants. The lack of trained operators was the main perceived barrier

[Cardiac remodelling and function after cardiovascular implantable electronic devices implantation].

Cardiovascular implantable electronic devices (CIED) encompass permanent cardiac pacemakers (PM) and implantable cardioverterdefibrillators (ICD). CIED play an important role in treatment of cardiac arrhythmias, including significant bradyarrhythmias and tachyarrhythmias. The conventional right ventricular endocardial leads as well as right ventricular pacing may increase valvular defects (especially dysfunction of tricuspid and mitral valve), cause adverse cardiac remodelling and lead to a decrease in left ventricular ejection fraction. These changes may be due to mechanical damage to the lea

Severe atrial fibrosis as a cause of significant intraatrial conduction delay in a patient with scleromyositis.

Scleromyositis is an autoimmune disease and an overlap syndrome of scleroderma and poly/dermatomyositis. It is characterized by frequent cardiovascular involvement including heart failure, arrhythmias and conduction disturbances. We present a case of a 73-year old female patient who required an upgrade from a DDD pacemaker to cardiac resynchronization therapy due left ventricular dysfunction and permanent ventricular pacing. Electroanatomical mapping (CARTO 3D) revealed extensive right atrial fibrosis which resulted in significant delay in intraatrial conduction. Interval from atrial paced sti…

The Role of Conduction System Pacing in Heart Failure: Current Evidence and Future Directions.

Conduction system pacing (CSP), including His bundle pacing and left bundle branch area pacing, offers a promising approach to treating heart failure (HF) with conduction system disease. This review examines the evidence supporting CSP as an alternative to traditional biventricular pacing in improving outcomes for HF patients. Studies were reviewed focusing on CSP's clinical efficacy in patients with left bundle branch block (LBBB) and those who are nonresponders to cardiac resynchronization therapy. Findings suggest that CSP enhances electrical and mechanical synchronization, improving left v…
