Iatrogenic perforation of a sigmoid diverticulum following diagnostic colonoscopy.

BACKGROUND: Colonoscopy is widely performed for diagnostic and therapeutic purposes and is generally safe. However, colonic perforation remains its most serious complication, with reported incidences ranging from 0.03% to 0.2% after diagnostic procedures. The sigmoid colon is particularly vulnerable, especially in elderly patients with diverticular disease. CASE PRESENTATION: We report the case of a 73-year-old woman with hypertension and type 2 diabetes who presented with diffuse abdominal pain 5 days after an incomplete diagnostic colonoscopy performed for chronic constipation. On admission

Therapeutic strategies for the prevention of acute diverticulitis according to the Diverticular Inflammation and Complication Assessment endoscopic score: a post hoc analysis of a prospective international study.

OBJECTIVE: The onset of acute diverticulitis (AD) is a significant concern, yet the optimal preventive strategy is unclear. METHODS: This multicentre, prospective cohort study included 1945 patients with newly diagnosed colonic diverticula and scored according to Diverticular Inflammation and Complication Assessment (DICA) classification. At baseline, patients were assigned to one of five treatment strategies: no treatment, a high-fibre diet, mesalamine, rifaximin, or a combination of mesalamine and rifaximin. The primary outcome was the development of AD within 3 years. Energy balancing weig

A Case Report of Necrotizing Neutrophilic Dermatosis: A Sheep in Wolf's Clothing.

Neutrophilic dermatosis (ND) is a category of diseases characterized by trauma-induced, autoinflammatory cutaneous eruption. Comorbid systemic disease is common with a predilection for malignancy, inflammatory bowel disease, and rheumatologic disease. Rarely, it can manifest with aseptic shock, an entity referred to as necrotizing neutrophilic dermatosis (NND). NND may occur in the postoperative setting and is often misdiagnosed as a necrotizing soft tissue infection. Unfortunately, the treatment for a necrotizing soft tissue infection, namely, wide debridement, is often detrimental in the set

Gangrene of the Kidney Following Percutaneous Renal Cryoablation of a Small Tumor.

Background: In selected cases cryoablation is a valid treatment option for small renal masses. The procedure is generally considered oncologically efficient with a low rate of severe complications. We report here a case of a 62-year-old man who after percutaneous cryoablation develops severe gangrene in the treated kidney. Case Presentation: A 62-year-old man was incidentally diagnosed with a 45-mm renal cell carcinoma. The tumor was found on a CT scan performed on the suspicion of diverticulitis. An abscess in relation to the sigmoid was found and he was treated with aspiration and antibiotic

Clinical profile and surgical outcome of Meckel's diverticulum.

The aim of the study was to estimate the incidence and analyse the clinical profile and surgical outcome of the Meckel's diverticulum. This prospective study included 632 patients who were operated upon for acute abdomen during August 1999 to July 2004 in a single surgical unit. Pre-operative abdominal ultrasonography and plain x-ray abdomen erect were done depending on the necessity. These patients were subjected to laparotomy/appendicectomy depending on the case. A search for Meckel's diverticulum was done and if found, surgical resection and analysis by histopathological confirmation of the

[Results and prognostic factors in the Hartmann procedure].

INTRODUCTION: The introduction of one-stage procedures in emergency colonic surgery many years ago has relegated the use of the Hartmann procedure to the most seriously-ill patients, which has led to the high morbidity and mortality rates associated with this surgical technique. The aim of our study was to investigate our results using Hartmanns procedure and to evaluate several prognostic factors of postoperative mortality in this group of patients. PATIENTS AND METHODS: From January 1995 to December 2000, 79 patients (34 men and 45 women) with a mean age of 71.5 years underwent Hartmanns op

CAPD in patients with autosomal dominant polycystic kidney disease.

OBJECTIVE: To investigate whether there are specific complications to continuous ambulatory peritoneal dialysis (CAPD) in patients with autosomal dominant polycystic kidney disease (ADPKD) due to defects in various wall structures--causing hernia and diverticulitis--and to enlarged kidneys. DESIGN: The clinical experience of CAPD in 26 patients with ADPKD, treated for 11+/-6 months, was studied in retrospect and compared with that of 26 contemporary controls. Medical records were reviewed with respect to survival in this treatment form and any complication. Peritoneal dialysis capacity (PDC),

Laparoscopic resection of cecal duplication cyst in a 28 years old male: a rare case report.

BACKGROUND: Cecal duplication cysts are a rare congenital anomaly, particularly uncommon in adults. With no clear etiology behind it, duplication cysts typically appear before 2 years of age. Their presentation is diverse, often mimicking other right lower quadrant pathologies. We report a case of an adult cecal duplication cyst treated successfully with laparoscopic resection, highlighting diagnostic challenges and management considerations. CASE PRESENTATION: A 28-year-old man presented to our care with a one-week history of right lower quadrant pain. Initial ultrasound and CT scan suggeste

[Managing side-effects of targeted therapies in renal cancer:surgical complications].

The principal surgical complication feared in a patient under anti-angiogenic targeted therapy is gastrointestinal perforation. It is therefore important to be alert to situations "potentially at risk" of perforation: diverticulitis, carcinomatosis, intra-abdominal abscess, history of external radiotherapy, colonoscopy in the 4 weeks preceding the treatment, inflammatory disease and gastrointestinal mucosal ulceration, colonic stent, NSAID. It is essential to make an early diagnosis, and for treatment to be conservative if possible, with nasogastric aspiration without surgery. Surgical treatme

Clinical and diagnostic relevance of Meckel's diverticulum in children.

Meckel's diverticulum (MD) has varied clinical manifestations and is a diagnostic challenge for pediatricians. This study investigates the clinical and diagnostic characteristics of MD in children with different presentations. We retrospectively reviewed the medical records of pediatric patients diagnosed with MD from 1988 to 2008. The data including the age, sex, clinical features, laboratory data, sensitivity of diagnostic methods, and pathological finding were recorded. These patients were classified into three groups: diverticulitis, intestinal obstruction, and intestinal bleeding. A total

Portal vein thrombosis after laparoscopic sigmoid colectomy for diverticulitis: report of a case.

Portal vein thrombosis is a very uncommon complication after laparoscopic surgery. Although only one case of portal vein thrombosis has been reported after laparoscopic colectomy, there are several reports of mesenteric vascular occlusion after other laparoscopic procedures. We present a case of portal vein thrombosis in a patient with no other demonstrable hypercoagulable states or risk factors, who underwent an uneventful laparoscopic sigmoid colectomy. Because alteration in coagulation may occur after establishing a pneumoperitoneum, we suggest that heparin prophylaxis may be advisable to a

[Acute peritonitis and small bowel diverticula in a patient with mitochondrial neurogastrointestinal encephalomyopathy].

INTRODUCTION: Jejunal and ileal diverticula are acquired lesions that are often associated with intestinal motility disturbances like those occurring in myopathies and neuropathies with visceral affection. Mitochondrial neurogastrointestinal encephalomyopathy is characterized by gastrointestinal dysmotility, cachexia, ptosis with external ophthalmoparesis, neuropathy, leukoencephalopathy and laboratory evidence of mitochondrial alteration. CLINICAL CASE: A female patient that since 9 months age presented digestive symptoms: diarrhea, nauseous, vomits, slow digestions and abdominal pain. She h

[Perforation of colonic diverticuli].

Difficulties of preoperative diagnosis of perforations of colonic diverticula are shown by an analysis of 11 patients. The Gartman resection of the sigmoid colon was performed in 8 patients. A conclusion is made concerning advantages of resection of the intestine without a primary anastomosis for perforation of colonic diverticula over other kinds of operative procedures.

Is there a role for magnetic resonance imaging in diagnosing colovesical fistulas?

OBJECTIVES: Colovesical fistula is a devastating complication, usually secondary to diverticulitis or colon cancer. Imaging plays a crucial role in delineating the underlying anatomy and identifying an etiologic factor. Barium enema, computed tomography, and cystoscopy have been the mainstays of investigation, but they have less than ideal sensitivity and specificity. This study assessed the role of magnetic resonance imaging (MRI) in the investigation of patients with probable colovesical fistulas. METHODS: A total of 22 patients with presentations suspicious for enterovesical fistulas who u

Technically difficult colonoscopy, a non-radiologic sign of sigmoidal obstruction: value of sigmoid colectomy for intractable symptoms.

Technical difficulty in passing the colonoscope was assessed in 371 patients undergoing 627 colonoscopies during 1989-91 and were graded as 0: no difficulty and cecum reached (71.43%); 1: difficult but cecum reached (20.22%); 2: difficult and cecum not reached although lumen beyond seen (4.85%); and 3: difficult and cecum not reached as lumen beyond could not be seen (3.5%). Frequency of patients with chronic lower abdominal pain and/or disturbed bowel habits in each grade increased as grade of obstruction increased: 0 (25.66%), 1 (36%), 2 (77.77%), and 3 (100%). During 1983-91, 54 patients wi
