Mechanical CPR - Assisted Percutaneous Coronary Intervention During Cardiac Arrest: Feasibility and Outcomes.

Cardiac arrest complicating acute coronary syndromes requires immediate coronary reperfusion; however, performing percutaneous coronary intervention (PCI) during active manual cardiopulmonary resuscitation (CPR) presents significant technical challenges. The Lund University Cardiac Arrest System (LUCAS) provides automated mechanical chest compressions and enables uninterrupted CPR during coronary angiography and PCI. Existing evidence is primarily limited to case reports and small observational studies, mechanical CPR has emerged as a practical adjunct in catheterization laboratories where man

Pathological capillary analysis of ischemia with non-obstructive coronary arteries in a Fabry disease patient receiving enzyme replacement therapy.

Fabry disease is a lysosomal disorder caused by an α-galactosidase A deficiency, which often causes angina in the absence of epicardial coronary artery stenosis. It is conceivable that epicardial and microvascular coronary dysfunction due to globotriaosylceramide and related glycoshingolipid accumulation within the vasculature causes ischemia with non-obstructive coronary arteries (INOCA). However, the precise histology remained unknown. We present the case of a 44-year-old male diagnosed with Fabry disease and treated with enzyme replacement therapy (ERT), who was referred to our cardiology d

Detection of periodontal microorganisms in coronary atheromatous plaque specimens of myocardial infarction patients: A systematic review and meta-analysis.

BACKGROUND: Microbial translocation from inflamed periodontal pockets into coronary atheroma via systemic circulation is one of the proposed pathways that links periodontitis and myocardial infarction (MI). The purpose of this systematic review is to determine the reported prevalence of periodontal microorganisms in coronary atheroma and/or aspirated clot samples collected from MI patients with periodontal disease. METHODOLOGY: The "Preferred Reporting Items for Systematic Reviews and Meta-Analyses" (PRISMA) guidelines were followed. Six databases were systematically searched using Medical Su

Uncommon cause of complicated myocardial infarction with normal coronary arteries in a Saudi patient.

A case of a young Saudi patient with a previous diagnosis of bronchial asthma, nasal polyps, and chronic smoker, presented with atypical chest pain, elevated serum troponin and borderline ischemic electrocardiogram (ECG) changes, with no significant regional wall motion abnormalities at bedside echocardiography is reported. The patient was admitted to the coronary care unit for continuous monitoring as possible acute coronary syndrome, non-ST elevation myocardial infarction (STEMI). One hour after admission, the patient had ventricular fibrillation (VF) cardiac arrest that required three DC sh

Kounis syndrome.

Kounis syndrome is characterised by a group of symptoms that manifest as unstable vasospastic or non-vasospastic angina secondary to a hypersensitivity reaction. It was first described by Kounis and Zavras in 1991 as the concurrence of an allergic response with an anaphylactoid or anaphylactic reaction and coronary artery spasm or even myocardial infarction. Since then, this condition has evolved to include a number of mast cell activation disorders associated with acute coronary syndrome. There are many triggering factors, including reactions to multiple medications, exposure to radiological

Coronary vasomotility and myocardial blood flow early after heart transplantation.

Serotonin constricts coronary arteries with endothelial dysfunction, a common abnormality in cardiac transplant recipients. To assess whether endothelial dysfunction is associated with myocardial blood flow (MBF) abnormalities, 24 patients were studied 1 to 12 months after transplantation. Serotonin in increasing doses (1, 10, and 20 micrograms/min for 2.5 min each) was infused into the coronary circulation. Diameters were measured by quantitative angiography. Fourteen patients (group A) had a pronounced artery constriction (diameter reduction > 40%), while in 10 other patients (group B), such…

Effect of Rotablator atherectomy and adjunctive balloon angioplasty on coronary blood flow.

BACKGROUND: The purpose of this study was to assess serial changes in coronary blood flow velocity before and after Rotablator atherectomy and after adjunctive percutaneous transluminal coronary angioplasty (PTCA). Since Rotablator atherectomy results in luminal enlargement by plaque pulverization and distal embolization, improvement in coronary blood flow could be attenuated despite luminal enlargement. METHODS AND RESULTS: Intracoronary Doppler blood flow velocity measurements were obtained with a Doppler Flowire. Basal average peak velocity (bAPV), hyperemic APV (hAPV), diastolic/systolic

[Dissection of the left coronary main branch by balloon rupture during stent implantation. A case report].

A 68-year-old female patient was admitted at our hospital because of unstable angina pectoris. Coronary angiography revealed an 85% diameter stenosis in the proximal segment of the left anterior descending coronary artery (LAD) and a 70% diameter stenosis in the mid segment of the right coronary artery (RCA). Elective implantation of a Palmaz-Schatz stent in the LAD stenosis and conventional angioplasty of the RCA stenosis was decided for. Positioning of the Palmaz-Schatz stent was uneventful, whereas deployment of the stent, using a pressure of 10 atmospheres was complicated by balloon ruptur

Recent developments in modulating atherogenic lipoproteins.

PURPOSE OF REVIEW: Randomized clinical trials have assessed the effects of several classes of drugs on plasma cholesterol levels in patients with coronary artery disease. Agents including niacin, fibrates and statins significantly lower LDL-cholesterol, but tolerance issues and undesirable side-effects are common. Residual risk may also be present in patients with persistently low HDL-cholesterol despite a reduction in LDL-cholesterol. Recent trials of drugs that increase circulating HDL-cholesterol have also been disappointing. RECENT FINDINGS: Ongoing efforts target the development of new p

Coronary flow and resistance patterns indexed by subtended myocardial mass in coronary microvascular dysfunction.

BACKGROUND: Patients with coronary microvascular dysfunction (CMD) exhibit impaired vasodilatation of the microcirculation. This manifests as reduced microvascular resistance reserve (MRR) due to either increased resting flow (Qrest; functional CMD) or decreased hyperaemic flow (Qhyper; structural CMD). However, coronary flow is intimately linked to myocardial mass, potentially confounding the interpretation of flow and resistance measurements. AIMS: We investigated the relationship between subtended myocardial mass, microvascular resistance, and coronary flow to determine whether the disturb

Absolute coronary blood flow across different endotypes of ANOCA.

BACKGROUND: Intracoronary continuous thermodilution is a novel technique to quantify absolute true coronary flow and microvascular resistance. However, few data are available in patients with angina with non-obstructive coronary arteries (ANOCA). AIMS: This study aimed to investigate the diagnostic potential of hyperaemic absolute coronary flow (Qmax) and absolute microvascular resistance (Rμ,hyper) among different ANOCA endotypes, and to determine the correlation between continuous - and bolus - thermodilution indexes. METHODS: A total of 222 patients were scheduled for clinically indicated

Coronary microvascular dysfunction is a hallmark of all subtypes of MINOCA.

INTRODUCTION: Myocardial infarction without obstructive coronary artery disease (MINOCA) is a heterogeneous clinical condition presenting with myocardial necrosis not due to an obstruction of a major coronary artery. Recently, a relevant role of coronary microvascular dysfunction (CMD) in the pathogenesis of MINOCA has been suggested; however, data on this are scarce. Particularly, it is unclear if CMD is equally present in all subtypes of MINOCA or differentially identifies one or more of these conditions. Therefore, the aim of this study was to assess CMD in all three coronary vessels of MIN

Characterizing Mechanisms of Ischemia in Patients With Myocardial Bridges.

BACKGROUND: Myocardial bridges (MBs) are prevalent and can be associated with acute and chronic ischemic syndromes. We sought to determine the substrates for ischemia in patients with angina with nonobstructive coronary arteries and a MB in the left anterior descending artery. METHODS: Patients with angina with nonobstructive coronary arteries underwent the acquisition of intracoronary pressure and flow during rest, supine bicycle exercise, and adenosine infusion. Coronary wave intensity analysis was performed, with perfusion efficiency defined as accelerating wave energy/total wave energy (%

Reversible Cerebral Vasoconstriction Syndrome Associated with Coronary Artery Vasospasm.

Reversible cerebral vasoconstriction syndrome is a rare disorder associated with neurologic symptoms secondary to diffuse cerebral vasospasm. Cardiac involvement in this disease is exceedingly rare. A 50-year-old woman was admitted to our hospital for evaluation of chest pain. During a 3-year period, she had been admitted multiple times because of chest pain and elevated serum cardiac enzymes. Transthoracic echocardiograms showed transient wall-motion abnormalities; however, coronary angiograms revealed no coronary artery disease. At the current admission, she had a thunderclap headache, and c

Noninvasive assessment of myocardial bridging in the left coronary artery by transthoracic Doppler echocardiography.

Myocardial bridging is a common congenital coronary abnormality recognized primarily with systolic narrowing or 'milking effect' shown by coronary angiography. We report the case of a 58-year-old man with signs and symptoms of myocardial ischemia who underwent transthoracic echocardiography and coronary angiography. The present case suggests the clinical role of transthoracic echocardiography in demonstrating characteristic coronary flow abnormalities in patients with muscular bridge in the region of the mid LAD.

Mechanical protection of cardiac microcirculation during percutaneous coronary intervention of saphenous vein grafts.

Saphenous vein bypass grafts permeability is one of the most important limitations of open heart surgery. The risks associated with surgical re-intervention are greater than those associated with the initial procedure. While native coronary arteries usually have fixed, fibrotic or calcified atherosclerotic plaques, the disease in the vein grafts contains soft material. When this material is compressed during percutaneous angioplasty, there is an unfavorable immediate outcome due to distal embolization of thrombus and plaque debris. In addition, the risk of post-procedure adverse events are hig

A precise, three-dimensional atlas of myocardial perfusion correlated with coronary arteriographic anatomy.

To map precise myocardial perfusion anatomy, we correlated detailed coronary arteriographic anatomy for every coronary artery and all secondary branches in the heart that had flow-limiting stenosis with corresponding specific, circumscribed, myocardial perfusion defects by positron emission tomography. Eight hundred ninety-five patients with abnormal coronary arteriograms showing any visible coronary artery narrowing of greater than 10% diameter stenosis underwent positron emission tomography perfusion imaging at rest and after dipyridamole stress; the data obtained were processed automaticall

Stenting within a stent for treatment of residual dissection.

We describe the management of residual dissection protruding through the struts of a Wiktor stent (Medtronic Inc., Minneapolis, MN) implanted in bailout conditions. In general, negotiating a stent through an expanded Wiktor stent is not recommended, but using this strategy, an AVE (Advanced Vascular Engineering, Santa Clara, CA) Micro stent was successfully implanted inside the Wiktor stent, leading to an excellent final angiographic result.
