OBJECTIVES/HYPOTHESIS: Examine a protective crumple zone effect of paranasal sinuses and nasal cavity on skull base fractures.
STUDY DESIGN: Randomized-control, cadaveric study.
METHODS: In the experimental group (n = 4), the nasal cavity and bilateral sinuses of cadavers were obliterated with bone cement, whereas the control group (n = 4) had native sinus architecture. Increasing frontal, glabellar impacts were introduced. Each impact event was examined with a high-speed video camera and sphenoid sinus pressure sensor. After each impact, computed tomography scans were performed and fracture

PRMT1 deficiency leads to severely compromised craniofacial development in neural crest cells and profound abnormalities of the craniofacial tissues. Here, we show PRMT1 controls several key processes in calvarial development, including frontal and parietal bone growth rate and the boundary between sutural and osteogenic cells. Pharmacologic PRMT1 inhibition suppresses MC3T3-E1 cell viability and proliferation and impairs osteogenic differentiation. In this text, we investigate the cellular events behind the morphological changes and uncover an essential role of PRMT1 in simulating postnatal b

PURPOSE: When localised scleroderma occurs in the face, neck and scalp area, it is called scleroderma en coup de sabre (SCS) for its resemblance to the stroke of a sabre. Most observed characteristics: abnormal skin and dental development, facial atrophy and neurological complications. The aim was to evaluate the extent of SCS in the underlying subcutis, including teeth/bone tissues. The goal was to solve, how far the external visual skin abnormality extends in depth, and if the condition appears within and limited to craniofacial neural crest fields.
METHODS: Photographic and radiographic ma

Since the nose is at the center of the face, small changes to it can cause a big overall change. Therefore, a nasal fracture needs to be operated on when deformity is expected. Although many taxonomies have been developed for nasal bone fractures, no study has aimed to predict individual patient satisfaction. Herein, the authors tried to determine any correlation between the existing classifications of nasal bone fractures and subjective patient satisfaction. A retrospective study and telephone survey were conducted in 149 patients who underwent closed reduction for a pure nasal bone fracture.

Giant cell tumors (GCTs) of bone are rare, locally aggressive primary neoplasms that account for approximately 3%-7% of all primary bone tumors. They most commonly arise in the epiphyses of long bones in young adults and are seldom seen in the craniofacial skeleton. Involvement of the frontal bone with orbital extension is exceptionally rare, particularly in the pediatric age group. We report a case of a 9-year-old male who presented with a progressively enlarging swelling of the left orbit associated with vision loss with atypical imaging features and an intracranial component with compressio…

THE RATIONALE: Encephalocoele is a herniation of the brain formed during embryonic development, because of the incomplete closure of Neural Tube. It is a rare skull defect and most cases are located in the occipital bone. Frontal encephalocoeles are very rare and they may involve the ethmoid bone, nasal bones and/or the orbits. Surgical repair is complex and usually requires a multidisciplinary approach.
PATIENT CONCERNS: We present a case of a 6-month-old baby girl with a congenital frontoethmoidal encephalocoele.
DIAGNOSIS: The diagnosis was made by computed tomography scan that showed a d

Osteopathia striata with cranial sclerosis (OS-CS) is a bone dysplasia characterized by hypertelorism, macrocephaly, frontal bossing, broad nasal bridge, palate anomalies, hearing deficits, and mental retardation. The radiographic findings include cranial sclerosis, linear striations in the long bones and iliac wings, small poorly aerated sinuses, scoliosis, and increased bone density. The sensory deficits are disabling, but the condition generally is not life threatening. We describe 4 brothers with the characteristics of OS-CS, 3 of whom have died from more serious complications of the disor

It has been suggested that craniosynostosis is caused by abnormally located ossification centers (i.e., bony tubers) in the developing skull prior to suture formation [Mathijssen et al., 1996, 1997]. The present study was designed to test this hypothesis in a rabbit model of human familial, nonsyndromic coronal suture (CS) synostosis. Calvariae were taken from 99 New Zealand White rabbit perinates (55 normal controls, 15 with delayed-onset CS synostosis, and 29 with bilateral or unilateral CS synostosis), ranging in age from 23 to 34 days postconception (synostosis occurs at approximately 23 d

BACKGROUND: In a prior report, evidence was presented for the presence of gap junction proteins [connexin32 and connexin43 (Cx43)] in embryonic facial primordia. The purpose of the present study was, first, to examine in detail the patterns of distribution of Cx43 protein in embryonic chick facial primordia and, second, to consider the possible roles played by this protein during midfacial development.
METHODS: Chick embryo heads were serially sectioned and processed for immunofluorescent localization of Cx43. The developmental stages examined encompassed the period of formation, enlargement,

Facial fractures have formerly been classified solely by anatomic location. CT scans now identify the exact fracture pattern in a specific area. Fracture patterns are classified as low, middle, or high energy, defined solely by the pattern of segmentation and displacement in the CT scan. Exposure and fixation relate directly to the fracture pattern for each anatomic area of the face, including frontal bone, frontal sinus, zygoma, nose, nasoethmoidal-orbital region, midface, and mandible. Fractures with little comminution and displacement were accompanied by subtle symptoms and required simple …

Our experience in 125 cases of traumatic frontal skull-base and mid-face lesions is discussed. 88 patients were operated on immediately after injury whereas 37 were operated on when the presented with complications due to the trauma some time after the injury. Small aeroceles seen on the computer tomogram are a definite and accurate sign of a dural rupture. The indications for an exploration of the frontal skull-base should be rather liberal since most dura lesions are difficult to verify. The current surgical techniques to repair the posterior wall of the frontal sinus and the cribriform plat

An 8-year-old boy suffered severe craniocerebral trauma with left-sided fronto-orbitobasal fracture. The CT scan showed minor subdural air inclusions. The child recovered well and had no clinical signs of aftereffects. Eight years after the accident, symptoms of intracranial pressure developed progressively with nuchal rigidity and elevated temperature. The CT showed an extensive left fronto-orbitobasal abscess. The intraoperative finding was a brain prolapse both into the frontal sinus and into the ethmoidal cavity with a large dura-bone defect at the site of the former fracture line, which w

The treatment of combined major dislocations of the face and skull requires a craniofacial team consisting of a plastic maxillofacial surgeon, a neurosurgeon, an anesthesiologist, and radiologists. The best results can only be achieved at the primary intervention both from a functional and an esthetic standpoint. Careful preoperative clinical and radiologic examinations are mandatory to reach these high goals. In primary repair, the cranial approach aims at constructing "a monobloc bone flap" to suture meningeal tears and to reinforce them with a pericranial graft, to close the cranial base co

Craniometaphyseal dysplasia is a rare congenital sclerosing skeletal dysplasia that presents with facial dimorphism and is clinically described by prominent supraorbital bridges, severe retrognathia, and respiratory problems. Fronto-orbital cranioplasty is necessary to achieve satisfactory aesthetic outcomes. The supraorbital arches were exposed to the nasal bone through coronal access, with the help of a saw and a wear drill, and osteotomy of the frontal and orbital regions with hyperostosis was performed. Our case report describes a rare and difficult to diagnose case with a few publications

PURPOSE: To report an unusual case of an ectopic lacrimal fistula in a setting of a naso-orbito-ethmoid fracture.
CASE REPORT: The present case demonstrates a right-sided ectopic lacrimal fistula just below the glabella on the contralateral side on the outer edge of the midline. CT-Dacryocystography demonstrated a large dilated right lacrimal sac with a partly sequestered fundus traversing through the frontal process of the maxilla and frontal bone, close to the skull base and communicating with the fistula. The connection of the fistula with the sac was severed at the superior-most point, an…

BACKGROUND: Lymphoplasmacyte-rich meningioma is an uncommon variant of meningioma, and that arising in the extradural site is extremely rare.
CASE DESCRIPTION: We report a case of primary intraosseous lymphoplasmacyte-rich meningioma. A 51-year-old woman presented with left frontal scalp swelling lasting longer than 1 year. Magnetic resonance imaging revealed an intraosseous extradural mass in the left frontal region with bone destruction. Surgical resection of the mass with a cranioplasty was performed. The postoperative course was uneventful, and histopathologic findings were consistent wit

Osteomyelitis with a fistulous tract of the left fronto-nasal area. The patient received an Anzio Beachhead WW II right fronto-nasal injury repaired with a lucite cranioplasty. A staphylococcal infection involved the remnant of the left frontal sinus with a fistula at the nasion near the orbital ridge. Bone scan revealed increased activity consistent with osteomyelitis. Patient was treated with intravenous oxacillin. The lucite skull prosthesis was removed 2 weeks after admission via the bilateral osteoplastic coronal flap approach with fat obliteration of the residual frontal sinus. The intra

Parathyroid adenomas rarely weigh more than 4 grams. Our patient had a 5.3-gram adenoma causing bilateral knee pain limiting mobility, constipation, low back pain, and frontal headache. Presenting with calcium of greater than 17 mg/dl, the patient was treated with two rounds of hemodialysis, calcitonin, Zoledronate, and aggressive IV hydration to decrease calcium levels before parathyroidectomy. The patient then went on to develop the hungry bone syndrome, which was treated with calcium carbonate and calcitriol. This rare giant parathyroid adenoma presents a unique opportunity to learn about t

Progressive facial hemiatrophy (PFH) is characterized by slowly progressive atrophy of subcutaneous tissue. Bone, muscles, nerves, the eye, and the brain may be affected by atrophy. Four patients suffering from various otorhinolaryngological complications of PFH or Romberg's disease are reported. Unilateral hearing loss could be located in the inner ear of one patient by audiologic examination. Localized bone destruction and disintegration of a portion of the anterior wall of the frontal sinus were observed in a patient after more than three decades. Marked shrinking of the homolateral parotid
