Necrobiosis lipoidica (NL) is a chronic granulomatous dermatologic condition usually associated with diabetes mellitus (DM). Early diagnosis and treatment can improve the quality of life of the patient and prevent the development of squamous cell carcinoma. Treatment options include corticosteroids, tacrolimus, cyclosporine, fumaric acid esters, biologics (including adalimumab, etanercept, and infliximab), and phototherapy with varying degrees of success. Here, we report a case of NL diabeticorum, which was effectively treated with immunosuppressants and keratolytics.

We report a case of a 47-year-old female, with strong preoperative clinical and radiological suspicious of uterine leiomyosarcoma who underwent a total abdominal hysterectomy. Despite the final histology concluded as benign uterine leiomyoma, the loss of fumarate hydratase expression of the same specimen still put her at risk of having hereditary leiomyomatosis and renal cell carcinoma syndrome. Intraoperatively, an obstetric vacuum cup was used for uterine manipulation to avoid breaching of the uterine serosa.

Three cases of contact dermatitis caused by an ethanolamine antihistamine, diphenhydramine hydrochloride (Benadryl), are described. Patients who develop contact dermatitis from it should avoid its oral or parenteral administration because they may develop a localized or generalized flare of dermatitis. Dimenhydrinate (Dramamine), clemastine fumarate (Tavist), carbinoxamine maleate (Clistin-D), and doxylamine succinate (Decapryn) are also ethanolamine antihistamines. Since they may cross-react with diphenhydramine hydrochloride, they should be avoided in sensitive patients.

We estimated list and net prices for tenofovir disoproxil fumarate (TDF) products Truvada, Complera, and Stribild, and their tenofovir alafenamide (TAF) versions Descovy, Odefsey, and Genvoya. Gilead offered discounts for Descovy that resulted into lower net prices compared to Truvada. This strategy encouraged patients switching from Truvada to Descovy before the availability of generic Truvada. Conversely, Gilead offered lower discounts for Odefsey and Genvoya, which resulted into higher net prices compared to Complera and Stribild.

Hereditary leiomyomatosis (HL) is a rare autosomal dominant syndrome resulting from a mutation in the germline of the fumarate hydratase (FH) gene. Patients with this syndrome have an increased risk of cutaneous and uterine smooth muscle tumors as well as renal cancer. Renal carcinoma associated with hereditary leiomyomatosis (HLRCC) was recognized as a subtype of independent renal tumor in the 2016 WHO classification. We present a case of HLRCC occurring in a 39-year-old man with no family history or specific skin manifestations at the time of diagnosis.

Di(tri-n-butyltin) itaconate (DTBTI) and monoethyl tributyltin fumarate (METBTF) were synthesized as organotin monomers. The organotin monomers were copolymerized with styrene (ST) and methyl methacrylate (MMA) via a free radical polymerization technique. The overall conversion was kept low (<or=15% wt/wt) for all studied samples and the copolymer composition was determined from tin analysis. The synthesized monomers and copolymers were characterized by elemental analysis, 1H- and 13C-NMR, and FTIR spectroscopy.

We describe a case of cardiomyopathy due to quetiapine. Atypical antipsychotics are known for their multiple and sometimes dangerous side effects. Various studies showed increased sudden cardiac death and sudden unexpected deaths. Quetiapine can cause tachycardia and QT prolongation, alongside a broad list of other side effects. Cardiomyopathy has been reported, but a causative relation was doubted. We report a case of a 37-year-old woman developing a cardiomyopathy under high doses of quetiapine. We report a case of priapism occurring in a patient taking a standard daily dose of the atypical antipsychotic quetiapine (Seroquel). To our knowledge, this represents the second published report associating quetiapine with priapism, and the first to associate priapism with routine dosing of the drug. Previously published studies have suggested that alpha-adrenergic blockade may be the mechanism of action for this side effect. We report successful management in this case with cavernosal aspiration followed by intracavernosal injection of phenylephrine.

BACKGROUND: Patients with a compromised immune system are at risk for converting from latent tuberculosis infection (LTBI) to active tuberculosis (TB) infection. Multiple sclerosis (MS) therapies may put individuals with LTBI at higher risk of TB.
