This retrospective study was performed to evaluate the clinical and angiographic characteristics and the in-hospital complications after failed percutaneous coronary angioplasty (PTCA) that necessitates coronary artery bypass graft surgery (CABG). The study population consisted of 123 patients from January 1990 to December 1992. The failed PTCA was secondary to an acute occlusion of the dilated but dissected vessel in 36.5% of the study population. 43 patients (35%) had an emergency CABG due to hemodynamic instability and a large area of myocardium at risk for infarction. 93% of these patients

One hundred and fifty-two patients age 70 years or more underwent aortic valve replacement (AVR) at Emory University Hospital between July 1, 1974 and July 1, 1982. Of these, 98 had isolated AVR (elderly AVR group) and 54 had concomitant coronary artery bypass grafts (elderly AVR/CABG group). Results of surgery in these patients were compared to results in patients aged 20 to 69 years operated on in the same period (young AVR/CABG groups). Comparative descriptors with statistically significant differences included a higher incidence of both stable and unstable angina in patients undergoing con

BACKGROUND: Refractory cardiac arrest remains a major challenge in children, with poor survival despite advances in cardiopulmonary resuscitation (CPR). Extracorporeal cardiopulmonary resuscitation (ECPR), defined as rapid deployment of extracorporeal membrane oxygenation (ECMO) during ongoing CPR, provides circulatory and respiratory support and can serve as a bridge to definitive interventions. While increasingly reported in adults, pediatric use remains limited due to anatomical and physiological challenges, with evidence largely restricted to case reports and small series.

Bedside pressure-flow variables in cardiogenic shock (CS) incompletely characterize ventricular energetics and coupling. We prospectively derived bedside pressure-volume (PV) loop surrogates from paired pulmonary artery catheter and echocardiographic data in 68 patients (263 paired assessments) with acute myocardial infarction-related CS (AMI-CS) or heart failure-related CS (HF-CS) during microaxial support, intra-aortic balloon pump (IABP) support, or medical therapy. In AMI-CS with microaxial support, arterial elastance decreased (-1.22 mm Hg/ml) with improved coupling (ventriculoarterial co

BACKGROUND: The Fontan procedure improves survival in single-ventricle patients, but some progress to Fontan circulatory failure (FCF) with limited treatment options. Fontan-associated liver disease (FALD) further complicates management and raises uncertainty regarding the choice between isolated heart transplantation (HT) and combined heart-liver transplantation (CHLT). Data from Asian populations are limited, and this study aimed to provide additional evidence on transplantation outcomes in this population.

Post-transplant infection caused by multidrug-resistant organisms (MDROs) is a major challenge in heart transplantation, especially in recipients requiring veno-arterial extracorporeal membrane oxygenation (VA-ECMO) before surgery. We describe a 52-year-old man with non-ST-elevation myocardial infarction and refractory cardiogenic shock who required VA-ECMO, intra-aortic balloon pump support, continuous renal replacement therapy, and mechanical ventilation before urgent heart transplantation. Before transplantation, he had active pneumonia. Donor respiratory culture grew Acinetobacter baumanni

BACKGROUND: Shock associated with diabetic ketoacidosis (DKA) is commonly hypovolemic or distributive in nature, reflecting its underlying pathophysiology and precipitating factors. However, in rare cases, DKA can precipitate stress-induced cardiomyopathy, which may lead to cardiogenic shock. In addition, DKA causes metabolic derangements, increasing the risk of additional cardiac complications.
CASE DESCRIPTION: A 30-year-old woman with no known past medical history presented with acute onset of fever and altered mental status. Laboratory evaluation demonstrated hyperglycemia, metabolic acid

Venoarterial extracorporeal membrane oxygenation (VA-ECMO) is widely used for short-term mechanical circulatory support in cardiogenic shock and refractory cardiac arrest. However, increased left ventricular (LV) afterload during VA-ECMO may lead to LV distension, a complication associated with myocardial injury, pulmonary congestion, and thromboembolic risk. Various strategies for LV unloading have been described, but experience with percutaneous transseptal LV cannulation in adults remains limited. We report two adult patients with cardiogenic shock supported with peripheral VA-ECMO who deve

RATIONALE: Septic cardiomyopathy (SCM) is a severe and often reversible complication of sepsis but carries a high mortality rate when it progresses to cardiogenic shock. Differentiating SCM from acute coronary syndrome in septic patients is a major clinical challenge. This report details a case of SCM secondary to small intestinal perforation, highlighting both its life-threatening presentation and successful management.
PATIENT CONCERNS: A 62-year-old male presented with acute abdominal pain and was found to have a small intestinal perforation with peritonitis. Emergency surgery was performe

Iliac artery rupture is a rare but life-threatening complication of transfemoral vascular procedures that typically requires urgent intervention. Traditional approaches use transfemoral or transbrachial access; however, transradial access may offer a safer alternative in selected patients. We report the case of a 68-year-old man with ST-segment elevation myocardial infarction and shock who underwent primary percutaneous coronary intervention via right radial access with intra-aortic balloon pump (IABP) support through the right femoral artery. Following staged coronary intervention and IABP re

BACKGROUND: Cardiogenic shock (CS) during venoarterial extracorporeal membrane oxygenation (VA-ECMO) support carries high mortality, often due to neurological injury. Precise matching of ECMO flow to cerebral perfusion is critical. Transcranial color-coded sonography (TCCS) offers real-time, non-invasive hemodynamic assessment to guide flow titration, though its clinical integration remains limited.
CASE PRESENTATION: This study reports a 58-year-old male firefighter who suffered out-of-hospital cardiac arrest due to acute myocardial infarction, resulting in CS, right frontal lobe hemorrhage,

An electrical storm (ES) represents one of cardiology's most formidable and life-threatening crises, marked by relentless ventricular arrhythmias within a 24-hour period. While stimulant cardiotoxicity is an escalating concern, the devastating role of methamphetamine in triggering refractory ES and its deleterious outcomes in advanced cardiomyopathy, particularly within the critical care setting, remains profoundly underreported and poorly understood. We present the urgent case of a 44-year-old male with end-stage dilated cardiomyopathy and chronic, heavy methamphetamine abuse, who spiraled in

BACKGROUND: Coronary artery vasospasm following coronary artery bypass grafting (CABG) is a rare but potentially life-threatening complication that can lead to myocardial ischemia, arrhythmias, and cardiogenic shock. Nitroglycerin-resistant vasospasm, particularly involving the left main coronary artery (LMCA), poses significant diagnostic and therapeutic challenges.
CASE PRESENTATION: We report the case of a 47-year-old female with a history of hypertension, hyperlipidemia, and ADHD on dextroamphetamine-amphetamine, who presented with chest pain and was found to have a suspected severe LMCA

Background: The clinical course of cardiac sarcoidosis is typically subacute, and fulminant cases requiring mechanical circulatory support are rare. Here, we report the case of a patient with pathologically diagnosed cardiac sarcoidosis who presented with fulminant myocarditis and whose cardiac function was improved by aggressive immunosuppressive therapy based on the treatment of giant cell myocarditis. Case Presentation: A 55-year-old woman presented with progressive dyspnoea and nausea that persisted for 1 month and was eventually diagnosed with acute heart failure. Echocardiography showed

Veno-arterial extracorporeal membrane oxygenation (VA-ECMO) is used widely as part of extracorporeal resuscitation (ECPR). Prolonged VA-ECMO may be associated with the development of foot drop, often secondary to limb ischemia. We report on three patients who developed foot drop following prolonged ECMO use, and we propose an alternative mechanism to ischemia. Three male patients, mean age 46.3 years, underwent peripheral VA-ECMO support, for an average of 22.6 days (17-25 days), two patients following ECPR and one post-cardiotomy shock. All patients underwent distal femoral artery perfusion c

BACKGROUND: Left atrial dissection (LAtD) is a rare but potentially life-threatening complication of mitral valve surgery. Its management is not well stablished in the literature. However, early recognition through intraoperative TEE and attention to changes in the left atrial free wall during saline leak testing can lead to avoidance of severe complications.
CASE PRESENTATION: We report a case of LAtD detected by intraoperative transesophageal echocardiogram (TEE) following mitral valve repair for primary mitral valve regurgitation secondary to degenerative mitral valve disease with MAZE IV

A 13 years and 1 month old child was admitted to the hospital with fulminant myocarditis. After admission, the child's condition deteriorated rapidly, with a rapid drop in blood pressure, alternating malignant arrhythmias, such as ventricular tachycardia and ventricular fibrillation, and then developed rapid cardiac arrest, which lasted for 62 min. Moreover, the child developed multiple organ failure (heart, kidney, brain, lungs, liver, gastrointestinal tract, and inflammatory system) with internal environmental disturbances, indicating critical conditions. After emergency extracorporeal membr
