SIGNIFICANCE: Image simulation is a useful and efficient tool to explore the impact of defocus and astigmatism combinations on visual acuity and image quality score when accommodation is taken into account.

Many people of the so-called 'baby-boom' of 1945, nowadays require presbyopic correction. A lot of them choose for multifocal contact lenses, but this is not the perfect solution. Decreased visual acuity and contrast sensitivity, and appearance of haloes, 'ghosting' and glare are a problem. We fit two multifocal soft contact lenses, Bausch & Lomb's Bi-Tech and the Unilens contact lens from Unilens Corporation, in two separate populations of presbyopes. The clinical concept of the range of zero-associated phoria is developed, and normative data for this range are presented based on a clinical study of 201 asymptomatic patients. The relation of patient age, sex, and amount of near lens addition to this range is examined. The range at far is not related to patient age or sex.
