Active fixation of bipolar left ventricular lead through a persistent left superior vena cava.

A left superior vena cava persistence was found in a 61 year-old patient affected by dilated and hypokinetic cardiopathy with severe dysfunction of the left ventricle (left ventricular ejection fraction of 32%) and valvular disease. After a negative coronary angiography, he was implanted with a cardiac resynchronization therapy with defibrillation function device (CRT-D). The present case describes the successful implantation of a biventricular defibrillator in this challenging congenital abnormality of cardiac venous system.

Minimally invasive video-assisted thoracoscopic left ventricular epicardial lead implantation for biventricular pacing in a patient with persistent left superior vena cava.

Cardiac resynchronization therapy (CRT) by biventricular pacing reduces symptoms and improves left ventricular function in many patients with heart failure due to left ventricular systolic dysfunction and cardiac dyssynchrony. Implantation of the biventricular pacing lead in association with persistent left superior vena cava is technically challenging. We report a successful case of minimally invasive video-assisted thoracoscopic left ventricular epicardial lead implantation for biventricular pacing in a patient with persistent left superior vena cava.

Isolation of the phrenic nerve to suppress diaphragmatic contraction induced by cardiac resynchronization.

Left phrenic nerve stimulation is a well-known complication of cardiac resynchronization therapy (CRT). We report a case where electronic or anatomical repositioning of the left ventricular (LV) electrode (within the coronary sinus tributaries) was not feasible/effective. Surgical graft interpositioning was performed to successfully isolate the phrenic nerve from the previously implanted coronary sinus epicardial LV lead.

Reversible left bundle branch block induced congestive heart failure. A case report.

Intraventricular conduction delay in the form of left bundle branch block plays an important role in the genesis and the progression of congestive hart failure. We report on the clinical course of a patient and the improvement in functional status after the disappearance of left bundle branch block, despite withholding cardiac resynchronization therapy.

Assessment of ventricular mechanical dyssynchrony by short-axis MRI.

Nowadays, patients with symptomatic heart failure and intraventricular conduction delay can be treated with a cardiac resynchronization therapy. Electrical dyssynchrony is typically adopted to represent myocardial dyssynchrony, to be compensated by cardiac resynchronization therapy. One third of the patients, however, does not respond to the therapy. Therefore, imaging modalities aimed at the mechanical dyssynchrony estimation have been recently proposed to improve patient selection criteria. This paper presents a novel fully-automated method for regional mechanical left-ventricular dyssynchro…

Device-related infection among patients with pacemakers and implantable defibrillators: incidence, risk factors, and consequences.

BACKGROUND: Device-related infection is a major limitation of device therapy for cardiac arrhythmia. METHODS: The authors analyzed the incidence and risk factors for cardiac device infection (CDI) among consecutive patients implanted with pacemaker (PM) or implantable cardioverter defibrillator (ICD) (including cardiac resynchronization therapy devices) at a tertiary health center in Hamilton, Ontario, Canada. Most patients with device-related infections were identified by an internal infection control system that reports any positive wound and blood cultures following surgery, between 2005 a…

Recurrent acute pulmonary oedema and cardiac arrest secondary to intermittent electrical dyssynchrony: a role for cardiac resynchronization despite preserved left ventricular function.

Recurrent pulmonary oedema with no obvious precipitant can prove difficult to treat in patients with preserved left ventricular (LV) systolic function. This report describes the novel use of cardiac resynchronization therapy (CRT) in the prevention of acute pulmonary oedema precipitated by intermittent electrical dyssynchrony in a patient with preserved LV systolic function.

[New pacing strategies for heart failure].

In patients with a reduced left ventricular (LV) systolic function (ejection fraction < 35%) and a left bundle branch block with a QRS duration > 130 ms, cardiac resynchronization therapy (CRT) can contribute to an improvement in the quality of life and a reduction in mortality. The resynchronization is mostly achieved by pacing via an epicardial LV lead in the coronary sinus; however, this approach is often limited by the patient's venous anatomy and an increase in the stimulation threshold over time. In addition, up to 30% of patients do not respond to the intervention. New treatment approac…

Keeping pace with the competition: His bundle versus biventricular pacing in heart failure.

PURPOSE OF REVIEW: His bundle pacing (HBP) has emerged as a novel method to achieve electrical resynchronization in bundle branch block and as an alternative means to deliver cardiac resynchronization therapy (CRT). There are now data on HBP in CRT-eligible patients from cohort studies and a single pilot randomized controlled trial (RCT). RECENT FINDINGS: Early clinical data regarding HBP in heart failure have demonstrated echocardiographic and functional improvement similar to traditional biventricular pacing (BiV), mostly when utilized as a bailout to traditional BiV-CRT. A single pilot RCT…

Left Bundle Branch Pacing: Unexpected Resynchronization Effect of Anodal Capture.

Anodal capture can provide a resynchronization effect on biventricular pacing. However, evidence of such an effect remains unclear in left bundle branch pacing (LBBP). We report a patient with a baseline sinus rhythm with complete atrioventricular block and a ventricular escape rhythm with a right bundle branch (RBB) block QRS morphology and left axis deviation who received LBBP for cardiac resynchronization therapy (CRT), and its anodal capture stimulation (ANS) rendered a near-normal QRS morphology. The resolution of the RBB pattern during bipolar pacing in this patient resulted from a fusio…

Cardiac CT Angiography in Congestive Heart Failure.

Cardiac CT angiography has become an important tool for the diagnosis and treatment of congestive heart failure. Differentiation of ischemic from nonischemic cardiomyopathy; evaluation of myocardial perfusion; characterization of hypertrophic cardiomyopathy, left ventricular noncompaction, and arrhythmogenic right ventricular dysplasia; and delineation of congenital heart defects and valvular abnormalities are the primary diagnostic applications. Therapeutic use includes visualization of the coronary venous anatomy for optimal implementation of cardiac resynchronization therapy and evaluation …

Collateral approach for LV lead implantation in a case with abnormal venous anatomy.

A 75-year-old man, 8 years after CABG, with ischemic cardiomyopathy underwent cardiac resynchronization therapy (CRT) for refractory heart failure. Retrograde occlusion venography revealed absence of lateral vein. A functionally occluded middle cardiac vein with branch to anterolateral vein was used for left ventricular lead implantation. Using a collateral route for left ventricular lead implantation is a new technique. Lead position was stable with excellent threshold. Follow-up at 6 months reveals continued stable lead position.

Devices in the management of advanced, chronic heart failure.

Heart failure (HF) is a global phenomenon, and the overall incidence and prevalence of the condition are steadily increasing. Medical therapies have proven efficacious, but only a small number of pharmacological options are in development. When patients cease to respond adequately to optimal medical therapy, cardiac resynchronization therapy has been shown to improve symptoms, reduce hospitalizations, promote reverse remodelling, and decrease mortality. However, challenges remain in identifying the ideal recipients for this therapy. The field of mechanical circulatory support has seen immense …
